Unfavorable prognosis for children with malignant liver tumors, is caused not only because of late diagnosis in this children, but because of imperfet methods of treatment. These methods taken from adults oncology have to be adapted for pediatric patients in front of their differences in biology, pharmacodynamic reactivity and tumor-host relationships in developing organism. Some methods of treatment can be done the same way as in adults, but another (intraarterial treatment) can be used in children with much better results then in adults. Primary malignant tumors in children are hepatoblastoma and hepatocelullar carcinoma (minority) and metastatic tumors are nephroblastoma or neuroblastoma. That makes probably basic difference with liver tumors in adults, as well as absence of hepatic cirrhosis in children. 42 children with primary and metastatic liver tumors were treated by the author in Clinical Department of Pediatric Oncology, Institute of Mother and Child, Warsaw. 19 of them was given intra-arterial chemotherapy for unoperable primary tumors, 6--systemic chemotherapy for the same reason, 5--radiotherapy (all of them neuroblastomas) and 15 was submited to surgery-From this group 13 was operated radicaly by means of right extended or left lobectomy. Only in one case, middle lobectomy was done. 3 children operated radicaly was previously treated with intra-arterial chemotherapy and only after significant remission, surgery was done. As a result 6 children is RFS for more than 3 years. One of them exclusively grace to intra-arterial treatment. Experimental investigation done in monkeys shown no long term morphological and/or functional disturbances after long time hepatic infusion with chemotherapeutic drugs. In conclusion it is to state that surgery is the best method of treatment in primary liver tumors, some time preceded with intra-aterial chemotherapy that is safe for normal liver tissue. In metastatic neuroblastoma in liver, radiotherapy with systemic chemotherapy can give 60% of RFS (recurrent free survival).

The main purpose of this paper is presentation of chosen methods of reintegration into main stream from the point of psychological, social development and physical capacity, in children with cancer. These methods concern to: investigation of parents' attitude towards children with cancer, group psychological therapy, evaluation of general physical capacity of the children and their locomotion function. There were done preliminary analysis of parents' attitude of children with cancer in diagnostic period. It consists a trial in verification of chosen methods.
The material basic for this paper consists of 24 children with soft tissue tumors, aged 1-16 years which were diagnosed in the Clinical Department of Child Oncology of the Institute of Mother and Child Warsaw. The diagnostic methods included: interview, physical examination, radiological examination (including CT), bone marrow punction histological investigation by means of fine needle punction and/or biopsy. These methods were sufficient in making a precise diagnosis and staging. The most frequent was stage III (16 cases). It comes from a fact of late diagnosis caused by non characteristic clinical picture. All common localisation was represented in this group (stage III including a cases of causel localisation in the heart.
Fine needle punction proposed by Martin and Ellis in 1931, is a safe method of diagnosis by means of cytology making possible tumoral verification. In group of 24 children with RMS treated in Clinical Department of Child Oncology, Institute of Mother and Child Warsaw during last 2 years, fine needle punction was made in 7 cases. Precise diagnosis was possible after cytological examination in 2 cases. Non-precise diagnosis confirming malignant tumor only, was done in 3 cases. In resting 2 cases there were no material for evaluation, by means of cytology. Anyhow there was tissue material for histopathological examination. No complications in our material was observed. Fine needle punction is a safe method but requires personal experience of the surgeon and pathologist who performed cytological examination. Method seems to useful in advanced tumors when surgery is limited. Precise diagnosis is possible in short time, and early treatment can be done.
24 children with rhabdomyosarcoma were treated in Clinical Department of Pediatric Oncology of the Institute of Mother and Child, Warsaw during last 2 years. Complex therapy consisted of surgery, chemotherapy, radiotherapy and rehabilitation. Choice method of the treatment depends of stagging by Intergroup RMS Study. Basic method is surgery (radical excision with margin of neighbouring tissue). Modern multi-drug chemotherapy has changed strategy of the treatment by means of less extended surgery. Radiotherapy in RMS has to be mega-voltage what gives short time of irradiation and less complication. Rehabilitation as a component of complex therapy of RMS has to preserve function of treated organ and in cases after mutilated surgery becomes rehabilitated procedure.
Forty children with life-endangering tumors localized in the ENT region were treated during the last 10 years at two clinics in Poland: the Oncological Clinic in Warsaw and the Otolaryngological Children's Clinic in Lódź. In this group there were 29 malignant tumors at different pathology, and 11 non-malignant tumors that became life-endangering because of their size and localization. These children were treated, dependent on the nature of the tumor, with surgery, chemotherapy and X-ray therapy.
We studied 116 benign and 18 borderline malignant mucinous ovarian cystadenomas. Their epithelial lining was compared with normal epithelium of the uterine cervix and that of the small and large intestines, by both light and electron microscopy as well as histochemically. This morphological and histochemical analysis enabled us to subdivide mucinous cystadenomas into cervical, mixed and intestinal epithelial types. The epithelial cells of intestinal type tumours exhibited histochemical reactions very similar to, or identical with, those found in both immature and mature cells of intestinal mucosa. Malignant transformation was found in intestinal type mucinous cystadenomas only. Ultrastructural investigations of six neoplasms (four of intestinal and two of cervical epithelium type) confirmed the results found with the light microscope.
The authors present their own clinical material of 74 cases of rhabdomyosarcoma in children, treated in the years 1962-1980. The analysis consists of generally used methods of diagnosis and treatment. Diagnosis was made on the base of clinical, radiological and pathological examination. Treatment was established after stagging qualification and after actually used methods. The treatment based on surgery, radiotherapy, and simple drug chemotherapy was giving bad prognosis and good results was obtained in single cases only. Significant improvement in the results was obtained after multidrug chemotherapy introduced into combined treatment (40%).
In 56 cases of children with tumours, the level of alpha-fetoprotein (AFP) was established with radio-immunological method. There were 15 children with neuroblastoma, 13 with ovarian tumours, 14 with sacro-coccygeal tumours, 7 with tumours of liver and 7 with tumours of testis. The level of AFP was elevated in all children with hepatoblastoma, carcinoma embryonale, and malignant teratoma. Monitoring was performed in 15 children with tumours mentioned above. Usefulness of evaluation of AFP level in blood serum in these tumours has been confirmed by diagnosis. In effective treatment of these tumours, AFP level has fallen down to normal. Its rising up again prognosed tumour's relapse several months before.
In the years 1962-1976, 168 children with nephroblastoma (Wilms Tumour) were treated in the Clinical Department of Child Oncology of the Institute of Mother and Child in Warsaw. In 150 cases, independently from surgery and radiotherapy, chemotherapy was performed. Our material was divided into 5 groups: a) without chemotherapy, b) chemotherapy with different drugs, c) chemotherapy with 1 course of Actinomycine D, d) chemotherapy with several courses of Actinomycine D, e) chemotherapy according to SIOP Trial. The results with reccurency free survival are as follows: a) 22,2%, b) 24,1%, c) 28,1%, d) 46,6%, e) 56,6%. In our material the best results were received with SIOP Trial chemotherapy.
In advanced stages of neuroblastoma, surgical management is limitedly indicated. In majority of cases (25/26 in own material) surgical procedures were performed for the sake of receiving samples for pathological examination. Surgical treatment was very rarely indicated. The only one case of neuroblastoma in stage IV that was indicated to surgical treatment after complete remission of metastases, the child died in spite of radical excision of the primary tumour. The reason of that was new metastases in distant lymphonodes which appeared when complex therapy was finished. A little larger indications for surgery exist in stage IV-S of neuroblastoma, which gives better prognosis than stage IV. Anyhow indications for surgery in stages IV and IV-S have to be given very cautiously. Histopathological evaluation can be done on the basis of fine needle biopsy, that is much safer for the children.

The authors present a group of 10 children with neuroblastoma treated with multidrug chemotherapy. 5 children were treated according to VCAD protocol and 5 children according to COMP protocol. The VCAD protocol seems to be useful in less advanced stages of neuroblastoma (stage I and II after incomplete excision) as a prophylactic chemotherapy and in stage III with parallel irradiation of tumoral mass. Chemotherapy with COMP protocol can provoke palliation and prolongation of survival time, and sometimes significant remission. Having support of the treatment with adequate irradiation it is possible to restrict tumoral mass and consolidate the result od chemotherapy.
