[Cochlear implantation and auditory feedback].

The aim of the present study was to evaluate the short term effect of cochlear implantation on the fundamental frequency and the articulation of 13 patients. This investigation objectives for the first time the changes of fundamental frequency and articulation in cochlear implant patients. All postlingual deaf patients were provided with a Combi 40+ cochlear implant. Voice recording was performed pre- and three months post-implantation. The voice data was analyzed using the "X-Tool" software. The results show that 38% of the patients had a significant (p < 0.05) decrease of F0 mean three months after implantation. A postoperative decrease of F0 towards the normal range was observed generally in all patients. Spectographs pre- and post-implantation showed that the articulation improved already 3 months after partial restoration of auditory feedback. A large variability of F0 was noticed among the deaf subjects, but no correlation to the duration of deafness could be established.

Reference states and reversals: undoing actions with verbs.

The purpose of these studies is to characterize children's conception of reversal and its relation to a reference state. A reversal is the move from one state to some prior state of affairs. For example, shoes that have been TIED can be UNTIED, parcels WRAPPED then UNWRAPPED, and dishes COVERED then UNCOVERED. The present studies were designed to find out how children (aged 1;0 to 5;0) describe reversals of action that restore objects to a prior, less constrained, state. In English, the prefix un- offers the most productive device for this, but, initially, children rely on a verb like open, on general purpose undo, and on particles like out and off. As they acquire un-, English-speaking children must learn that this prefix applies primarily to verbs for change-of-state, often for enclosing, covering and attaching. In German, there is no reversal prefix, but there are productive particles. German-speaking children also begin with a verb like open and then turn to verb particles on a course similar to that in English to express reversals.

Surgical voice rehabilitation after laryngopharyngectomy. Functional results of tracheo-hypopharyngeal shunts by jejunal transplantation.

For surgical voice rehabilitation after pharyngolaryngectomy we use the technique of placing a siphonlike jejunal interposition in order to create a tracheohypopharyngeal shunt. In this paper we present the functional benefits of this method of voice rehabilitation. The best results achieved are compared with a normal voice using the following test parameters: electroacoustic sound analyses and voice status. The findings show that the siphon voice is adequate for everyday purposes and can achieve useful social verbal communication.

The influence of genotypes and social ranks in the clinical course of an experimental infection with Mycoplasma pulmonis (MRM) in inbred rats.

Social ranks of healthy, adult male inbred rats housed in groups of four per cage were determined by their relative differences in intromission and ejaculatory patterns observed repeatedly under competitive copulatory conditions. Once ranked they were inoculated with M. pulmonis by contact with castrated females suffering clinically from M.pulmonis.--Body weight reduction and scores for laryngo-bronchotracheal breathing sounds and dispnoea or sneeze or wipe and the titers of specific antibodies were continuously observed during 123 days of illness. Studies were done in five series concerning the inbred strains AS/Ztm, LEW/Ztm, BH/Ztm. Genotypic differences accounts for 60-80% of the variability of the illness. LEW/Ztm was the most affected strain. Only in strain LEW/Ztm week correlations were shown between social rank before the infection (characterized by sexual activity under competitive conditions) and the scores for breathing sounds, dyspnoea and sneeze and wipe. Animals high in social rank showed a milder clinical course of the infection.

Malignant metastatic disease of the eye: management of an uncommon complication.

Twenty-four patients with metastatic cancer of the choroid were seen in the Department of Therapeutic Radiology, Rush-Presbyterian--St. Luke's Medical Center, Chicago, between 1964 and 1975. One fourth of the patients had involvement in both eyes, for a total of 30 eyes involved with metastatic cancer. The most common site of the primary carcinoma was the breast. Most of the patients had inadequate vision, proptosis, pain, heaviness, and double vision. Retinal detachment was a common finding. All the patients received a palliative course of radiotherapy to the posterior chamber of the eye; between 2100--3000 rad in 7/10 fractions were given. Of the patients, 33% had complete recovery of symptoms, and 37% had only partial recovery; 30% showed no response to therapy. Survival period from the time of radiotherapy to death varied from one to 21 months, with a median of four months and a mean of 6.6 months. All patients responding to therapy showed marked improvement in their quality of life until death.

Movement phosphenes in optic neuritis: a new clinical sign.

Positive visual phenomena (phosphenes) elicited by eye movements have been described in normal individuals and in myopes with vitreous opacities. In the present paper we describe eye movement-induced phosphenes that appear to be related to optic nerve involvement in patients with optic neuritis and multiple sclerosis. This phenomenon is not associated with vitreous or retinal defects, nor is it similar to phosphenes observed in normal individuals. Instead, it shares many characteristics with the well-known Lhermitte sign and is believed to represent a similar phenomenon.

Short-term effect of auditory feedback on fundamental frequency after cochlear implantation.

The present study evaluates the short-term effect of cochlear implantation on the fundamental frequency (FO) of 13 deaf patients. All patients were provided with the Combi 40+ cochlear implant. Voice recording was made pre- and 3 months post-implantation. The FO was analysed using X-Tools software. The results showed that 38 per cent of our subjects had a statistically significant decrease of their mean F0 (p=0.001) at 3 months following implantation. It was also observed that the patients tended to have a lower F0 postoperatively approaching the normal range of F0. A large variability in F0 was noticed among the deaf subjects but no correlation with the duration of deafness was seen. There was also no correlation between speech recognition and speech production.
