[Congenital poikiloderma of the verrucous type in Thomson syndrome with associated myopathy].

This report deals with a 15-year-old girl suffering from the verrucous type of Thomson's syndrome. Initial poikilodermatous skin changes developed on both cheeks at the age of 3 months. Subsequently, rapid generalization of typical skin findings was observed. The clinical heterogenity of this syndrome is discussed with reference to the existing literature and the present case. Up to now, very few comparable cases of associated neurological symptoms have been described.

[Videomicroscopy in differential diagnosis of skin tumors and secondary prevention of malignant melanoma].

During a 12-month period, 824 pigmented skin lesions were examined, and the clinical, videomicroscopic and histological diagnoses were compared. Patients with skin lesions that were difficult to assess were recruited from the outpatient clinic of the department of dermatology within the Technical University of Munich. The study reveals that videomicroscopy as a variation of epiluminescence microscopy much improves diagnostic accuracy, especially of early malignant melanoma, but also with regard to the differentiation between melanocytic and non-melanocytic lesions, is achieved. Furthermore, patients with a high relative risk of developing malignant melanoma, such as patients with multiple naevi, can be scheduled for thorough microscopical controls. The possibility of uncomplicated photographic documentation of a large number of naevi after excision of the suspicious lesions allows valuable periodic follow-ups with macroscopical and microscopical comparison.

The effect of glucose and insulin on vagally induced gastrin, bombesin-like immunoreactivity and somatostatin secretion from the perfused rat stomach.

Previous studies in the isolated perfused rat stomach have shown that elevated glucose and insulin concentrations modulate BLI and somatostatin release during arterially administered peptidergic stimuli. In the present study the effect of elevated levels of glucose or insulin was examined on vagally induced changes of gastrin, somatostatin and BLI secretion. The lumen of the stomach was perfused with saline pH 7 or pH 2. Vagal stimulation (5 Hz, 1 msec, 10V) increased gastrin and BLI secretion and inhibited somatostatin release. The increase of the perfusate glucose concentration from 100 mg/dl to 150 or 300 mg/dl or the addition of insulin (100 microU/ml) augmented vagally stimulated gastrin release at luminal pH 7 but not pH2. Vagally induced inhibition of somatostatin was attenuated by both concentrations of glucose at either luminal pH while insulin had no effect. BLI secretion was affected neither by elevated glucose nor by insulin. On the other hand, the noncholinergic component of vagally induced BLI secretion in the presence of atropine was augmented by insulin. These data demonstrate that glucose and insulin can modulate vagally activated gastric neuroendocrine functions which could be of relevance during the ingestion of carbohydrate containing meals.

[Evaluation of the efficacy of concomitant special psychotherapeutic methods by patients in an internal medicine clinic].

Patients of a medical clinic whose complaints are diagnosed above all as functional are suitably at once also treated by the clinical psychologists. For this purpose the authors created an effective combination of methods. It consists of a group and individual therapy and contains rational and emotional, in addition suggestive and behaviour-therapeutic elements which for each patient are united to an individual concept. For the purpose of the control of efficiency in a particular examination was inquired how the various methods have become effective in the individual patients and which were used preferably, respectively. The inquiry is performed at the earliest half a year after the discharge. Out of 149 answers the most important results are represented and conclusions for practice are derived.

[Psychodiagnostic characteristics of patients with functional cardiovascular disorders].

As a contribution to the early recognition in 368 patients with functional disturbances of heart and circulation is examined, whether there are relations between distinguishable groups of complaint syndromes and different degree of neurotisation . Apart from this the characteristics of the symptoms for the adequate complaint syndromes found in the individual diagnosis are summarized to rank series, in which cases different distributions of frequency reveal. Certain parallels to similar investigations of other authors are discussed and conclusions for prophylactic and therapeutic measures are derived.

[Behavior therapy self control in chronic compulsive organismic defective habit development].

Patients with chronic compulsive organismic mishabituations, which are rather difficult to treat, can be helped by self-control methods of behavioural therapy combined with suggestive measures. The author currently regards this therapeutic approach to be superior to aversion therapy, which was occasionally used for such cases in the past. The contribution presents a scheme of therapy which can be generalized and is based on the work of Meichenbaum and Leonhard. The scheme consists of two parts containing four phases each. The actual approach and the modification necessary to tailor it to the individual case is explained by means of an example involving the treatment of psychogenic polydipsia.

[Concept of the management of sex deviation (methodology and current results)].

The problem of bettering sexually deviant persons who have already incurred several penalties is of major importance in social as well as forensic respects. Since the departure from accepted sexual norms is believed by scientists to be due not only to an abnormal disposition, but to distinctive traits acquired at a very early age as well, attempts at treatment with suitable means should not at once be regarded as useless. The authors present a concept that integratevely combines suggestive and behavior-therapeutical methods, that is standardized to a certain extent, and that has yielded encouraging results in addition to being anything but time-consuming. This paper is aimed at colleagues interested in trying to help perfect this concept.

[Apparative aversive therapy in combination with verbal suggestions in special obsessional syndromes (initial investigation)].

The author restricts the use of aversion therapy by means of deliberate production of pain to obsessional, especially therapy-resistant disturbances of a permanent nature, with consideration being, of course, given to ethical factors. Experiences worthy of generalization are derived from methodically varied courses of treatment, bringing out suggestive moments subliminally involved in any therapeutical situation and also specifically used by the therapist. In addition, the author emphasizes the need for simultaneously developing, besides aversion therapy, positive attitudes and behavior patterns.

Granulomatous cheilitis and Crohn's disease in a 3-year-old boy.

Granulomatous cheilitis and Crohn's disease are disorders of unknown etiology. There are case reports describing their coincidence and pointing out the necessity of ruling out systemic disorders once the diagnosis of granulomatous cheilitis is made. However, such reports are few and the causal association of both diseases is controversial in the literature. We report the youngest patient so far, a 3-year-old boy, who had granulomatous cheilitis and Crohn's disease simultaneously. This coincidence so early in life strongly suggests that both represent manifestations of the same disease.
