A technique for the difficult peripheral arterial aneurysm.

Complete external control of the vessels feeding some aneurysms may be very difficult and occasionally only achieved at the cost of damage to surrounding structures. A method is described in which Foley catheters are introduced into the feeding vessels from within the aneurysm and then cut distal to their clamped side branch to enable Dacron grafts to be passed over them and sutured in place. This technique has been used in 3 patients with very satisfactory results and is offered as an alternative when standard control is hazardous, or impossible.

Smoking and other factors affecting the conservative management of peripheral vascular disease.

Three hundred and ninety patients with peripheral vascular disease, who were neither eligible for, nor fit to undergo, surgery, have been studied in a prospective trial to assess the effectiveness of adequate active conservative therapy. These patients were followed up from 4 to 72 months, the duration of the average follow-up being 24 months. There was a 72% over-all improvement rate. The most striking feature of the trial was the correlation between smoking and failure to improve.

Cytomegalic virus infection of bowel in adults.

Cytomegalic virus infection of the bowel in 2 adult patients is reported. The controversies related to this rare pathology are evaluated, particularly the role of cytomegalic inclusion virus (CMV) as a primary or secondary pathogen in bowel.

Carcinoma of the thoraco-abdominal oesophagus.

A new approach in the management of carcinoma of the thoraco-abdominal oesophagus, by means of pre-operative chemotherapy, in the form of Methotrexate and irradiation, followed by a one-stage resection, using a thoracic/abdominal approach, is described. This resulted in a marked tumour regression without leading to difficulty with operative management.
