The MODEL project: a scoring system to manage demand for cataract and joint replacement surgery.

Cataract surgery and joint replacement are highly cost-effective procedures; however, their availability is inequitable and doctors lack guidance on whom to refer and treat. We investigated the feasibility of using a scoring system to assess patients' suitability for operation. Wefound acceptable correlations between the scores ofgeneral practitioners and surgeons for both procedures. Scoring systems can be used in a consistent way bygeneralpractitioners and surgeons, and can make a contribution towards matching patients' needs with available treatment capacity.

Trochanteric osteotomy in total hip arthroplasty: comparison of 2 techniques.

A study of 100 cemented total hip arthroplasties was performed in 2 different hospitals. Group 1 hips (50 hips in 42 patients; mean follow-up period, 5.4 years) had been operated on with the modified Liverpool technique involving an anterior osteotomy of the greater trochanter. These were compared with group 2 (50 hips in 41 patients; mean follow-up period, 6.2 years), in whom a Charnley transtrochanteric osteotomy approach was used. The 2 groups showed no significant difference with respect to hip abductor muscle power (MRC grading), trochanteric union, Trendelenberg gait, heterotopic ossification (Brooker grading), and patient satisfaction assessed with the help of a questionnaire. Group 2 had a larger number of trochanteric problems, whereas group 1 had a greater increase in mobility scores after surgery.

Assessment of thumb to index pulp to pulp pinch grip strengths.

In this study of fifty men and women with normal hand function, the strength of thumb to index finger pulp to pulp pinch grip was assessed with the remaining fingers either flexed or extended. When the pinch grip was performed with the remaining fingers flexed the value was up to 92% higher than when the remaining fingers were extended. In future studies where thumb to index finger pinch grip is used as an objective test of function then the position of the remaining fingers must be defined to aid interpretation of the results.

The early experience of silastic trapezium implants.

This paper reports the early results of thirteen total trapezium replacements and seven partial trapezium replacements in the treatment of pantrapezial disease and first carpometacarpal arthritis. The results showed satisfactory relief of pain with good function of the thumb. This review highlights the difficulty in achieving implant stability and suggests the cause of this problem.
