Surgical treatment of congenital breast asymmetry.

Congenital breast asymmetry of sufficient severity to prompt surgical consultation is rare. The present study evaluated 40 patients who were treated for breast asymmetry at the Indiana University Medical Center. Objective measurements have been correlated with a subjective evaluation of symmetry to elucidate the goals of successful treatment. The surgical experience of the entire group is summarized and the patients who returned for follow-up are assessed.

Measurement of breast volume: comparison of techniques.

A system of graduated disks has been previously described as a method to determine breast volume. The accuracy and limitations of this system have not previously been reported. Comparison of these disks with a standardized volumetric method shows this simple system to be accurate. Standard linear correlation analysis was good for the entire group of breasts measured (r = 0.718). Conversion to true volume M is represented by the least-square line M = 20.33 + 1.37G. The correlation is expected to fail at volumes greater than 425 cc (greater than the calibration on the largest Grossman disk). Elimination of volumes greater than 425 cc, however, produced no improvement in correlation coefficient. Firm breasts (i.e., capsular contractures) displace less uniformly into the conical restraint of the disk and cause an overestimation of their volume. Three breasts in this series were evaluated with Baker grade III and IV capsular contractures. Elimination of these values improved the correlation between the two volume analyses (r = 0.853 and M = 1.56G - 4.48). We suggest that when confronted with very large or firm breasts that volumes determined by the Grossman disk be interpreted with care. A formula for calculation of the true breast volume from the Grossman measurement has been derived and presented.

Breast volume and anthropomorphic measurements: normal values.

Fifty-five consecutive female volunteers were evaluated. Linear measurements of the breast and nipple position were determined to fixed points on the chest wall. Volume determination was done by a standard chest-wall casting technique. A highly significant difference was found between left and right breasts in the axilla-to-nipple distance and nipple-to-midline measurements. A significant difference was also found in the distance from the lowest point of the breast (patient upright) to the nipple. Analysis of the volumetric difference between right and left breasts did not achieve statistical significance.

Experimental and clinical observations of the effects of cytotoxic chemotherapeutic drugs on wound healing.

The administration of perioperative doxorubicin HCl (Adriamycin) had profound effects on wound healing for 5 out of 7 breast cancer patients and 5 of 5 melanoma patients following intravenous and intra-arterial infusional chemotherapy, respectively. The clinical observation of significant reduction in wound tear strength (WTS) and wound tear energy ( WTE ) in the group of patients with cutaneous melanoma initiated this experimental analysis. A study of WTS ( kNm -2) in nontumor-bearing (non-TB) and Morris Hepatoma (MH)-7777 (TB) rats treated with therapeutic doses of Adriamycin (ADR) and methotrexate (MTX) was compared with saline-treated controls. Mean tumor volume (cm3) was unaffected by MTX, while significant tumor inhibition (p less than 0.01) was evident for ADR-treated TB animals. A correlation (r = 0.516, p less than 0.01) was observed for tumor volume and WTS. Separate analysis of TB and non-TB animals identified a significant correlation (r = 0.6259, p less than 0.01) between advancing cachexia in TB rats and WTS. A 21-day analysis was done for 160 animals to determine the effect of MTX and ADR on WTS ( kNm -2) and WTE ( Ncm -1). The presence of MH-7777 significantly (p less than 0.01) reduced WTE for TB animals not treated with chemotherapy. TB animals treated with ADR had significant (p less than 0.01) improvement in WTE at day 21 compared with TB controls. This enhancement in WTE was not observed in rats treated with MTX. These clinical and experimental observations suggest significant retardation of the early phases of wound fibroplasia as determined by WTS and WTE following operative trauma and subsequent administration of therapeutic dosages of cytotoxic agents.
