Proteolytic activity of human tumor cell lines deriving from bronchial squamous cell carcinoma, pulmonary metastasis of rhabdomyosarcoma and pleural metastasis of mesothelioma.

The proteolytic activities of human tumor cell lines deriving from bronchial squamous cell carcinoma, a lung metastasis of an embryonal rhabdomyosarcoma and a pleural mesothelioma were measured by use of chromogenic substrates. N-acetyl-alanine aminopeptidase activity, plasminogen activator activity, H-D-Ile-Pro-Arg-p-NA splitting activity as well as plasmin-like activity, cathepsin G-like activity and plasma-kallikrein-like activity were found in cell lysates. The enzymatic activity of N-acetyl-alanine aminopeptidase, plasminogen activator and H-D-Ile-Pro-Arg-p-NA splitting activity changed during culturing. Plasminogen activator and H-D-Ile-Pro-Arg-p-NA splitting activity decreased to very low values, whereas N-acetyl-alanine aminopeptidase activity leveled at 1 x 10(-5) mU/cell. Unlike other proteolytic activities, plasminogen activator was released into the medium. Plasminogen activator activity could be measured in culture medium which contained no fetal calf serum.

Defense mechanism and macroscopic tumor growth in lung tissue.

A total of 126 resection specimens from malignant lung tumors were cut into serial sections, and tumor volume and macroscopic growth pattern were computed. Four characteristic tumor growth patterns could be separated: Tumors growing in bizarre, irregular shapes; Tumors growing in spheroid shapes; Tumors growing in ellipsoid shapes; Tumors growing in mixed growth pattern. The immunologic response of the host tissue was analyzed grading the number of lymphocytes, plasma cells, macrophages in and at the boundary of the tumor tissue. Lymphocytic subpopulations were analyzed in 46 cases using monoclonal antibodies (BS3/BS4; T3, OKT4, OKT8, OKT11, OKT14). The majority of lymphocytes were T-lymphocytes and monocytes in cases with inflammatory response of host tissue. The ratio of inducer/helper subset (OKT4+) compared to suppressor/cytotoxic subset (OKT8+) was similar in expression as reported for circulating peripheral T-lymphocytes. The different growth patterns depend upon cell type of tumor, immunologic response of the host tissue, and tumor volume. The findings indicate that tumor progression into lung tissue is partly due to "localized metastatic growth" of different tumor cell subpopulations.

N-acetylalanine aminopeptidase activity in normal and tumour cells.

The catalytic concentration of N-acetylalanine aminopeptidase was determined in erythrocytes, polymorphonuclear leukocytes, lymphocytes, alveolar macrophages, human lung fibroblasts, human lung cancer cells, human umbilical vein endothelial cells, mice Leydig cells, rat tumour cells, and endothelial cells from hog and bovine lung. The catalytic concentration ranged from 0.5 +/- 0.1 nU/cell (erythrocytes) to 35 nU/cell (rat tumour cell line B Sp 73 ASML). Almost all tumour cells showed higher activity levels. No activity was found in human plasma.

Local tumor inflammation, lymph node metastasis, and survival of operated bronchus carcinoma patients.

Three hundred and fifty-three resection specimens with primary lung carcinomas were cut into serial sections, and the tumor volume was computed. Resected lymph nodes were cut into 0.3-mm serial sections and analyzed for metastasis. The inflammatory reaction of lung tissue was analyzed by grading the amount of inflammatory infiltrations of a complete tumor cross section. Survival of patients was evaluated by consulting the house physician every 3 months after surgical treatment. Percentage of specimens with severe inflammatory reaction of host tissue increased remarkably in tumors with a volume of 35-60 cm3. Percentage of patients with detectable lymph node metastasis increased with tumor volume but decreased at the tumor volume of 35-45 cm3. Mean tumor volume in patients with no detectable lymph node metastasis was increased if severe inflammatory response of host tissue existed. Survival of patients with severe inflammatory infiltrations was superior to survival of patients with no inflammatory infiltrations if grouped for tumor volume. Data indicated that inflammatory infiltrations in primary lung carcinoma may partly be related to the immunologic response of host tissue to tumor growth. Inflammatory infiltrations may delay tumor cell propagation into lymph nodes or may be even able to destroy small tumor cell agglutinations.

[Dose-response relationship and serum concentrations of methohexital and hydroxymethohexital following rectal anesthesia induction with 1% and 5% methohexital solutions in children].

Dose-response-curves for rectal induction of anaesthesia in children with 1%- or 5%-methohexitone-solutions and dosages of 5, 10, 15, 20 and 25 mg/kg body wt. were obtained in 10 groups of 20 children. Methohexitone and hydroxy-methohexitone serum-levels were compared in another 23 children after application of 1%- or 5%-methohexitone-solutions at dosages of 15 and 20 mg/kg body wt. The sleep induction quota after 1%-methohexitone-solution in the 20 and 25 mg/kg body wt.-dosages was significantly higher, 20% and 25% respectively, and the mean sleep induction time shorter, 37% and 45% respectively compared with results after 5%. The change after 15 mg/kg body wt. was not significant. A study of dosages 5 and 10 mg/kg body wt. was discontinued due to insufficient effect. Methohexitone serum-levels ranged from 0.7-8 mg/l. All of the children after the use of 1%-methohexitone-solution, and only 60% after 5%-methohexitone had serum concentrations above the sleep inducing "borderline" concentration of 2 mg/l. The differences between mean methohexitone and hydroxy-methohexitone-serum-levels were not significant due to the small groups and the wide range of results. We conclude that individual dosages of 15 or 20 mg/kg body wt. 1%-methohexitone-solution should be applied according to clinical criteria such as physical and psychic status of the child.

[Possibilities for the use of exfoliative urine cytology in invasive and noninvasive bladder cancer].

In invasively growing tumours of the urinary bladder specificity and sensitivity of the urine cytology are sufficiently high with 100 and 66%, respectively, it is, however, not successful. Endoscopy and histological ascertainment are now as ever prevailing (TUR). Cytology is indispensable to the diagnosis of the carcinoma in situ of the urinary bladder. In non-invasive urothelial carcinomas the classification of the WHO after Friedell stands the test. It is also shown a better correlation to the cytological findings which more frequently become positive with growing degree of malignity. High degree of malignity, multiple tumours and positive cytology are to be valuated as particular risk factors concerning frequent relapses and their appearance of invasively growing tumours. The domain of cytology is the control of the course with curative aim of patients who were operated on in order to sustain the organ. Relapses are frequently more dedifferentiated than the primary tumour so that the recidivation is often indicated cytologically. The endoscopic demonstration of the recidivation may still be absent at the beginning. Also for controls under local chemotherapy cytology stands the test, in which cases we may not sufficiently judge the value of this treatment method due to the small number of cases.

[Urogenital bilharziasis in Germany].

Urogenital schistosomiasis was confirmed in four patients, three Egyptians and one from Guinea. They were admitted with typical complaints, some chronic some acute, and successfully treated with praziquantel (Biltrizide). Since a single oral dose is sufficient, this is a greatly simplified form of treatment, without significant side effects.

[Frequency of tuberculosis in autopsy material of the Institute of Pathology of the Friedrich Schiller University Jena].

Steinbrück (1974), Müller and Ebert (1979), Martin and Schulze (1975) as well as Kühne (1982) reported on a number of cases of tuberculosis which remained undetected until autopsy though, according to analyses of the National Register of Tuberculosis in the GDR, the number of lethal cases has decreased (Steinbrück 1970, 1979). Analysing our post-mortem records we found 165 cases of tuberculosis (4.44%) among 3 714 autopsies. AS proposed by Seelinger an Gebhard (1977), they were subdivided: 1. tuberculosis considered the main cause of death; 2. tuberculosis was a secondary finding. Tuberculosis was shown to be the main cause of death in 50.9% of the cases including consequences of tuberculosis, for instance cor pulmonale. In 49,1% of the cases tuberculosis was a secondary finding. An analysis of age and sex distribution revealed that most of the cases occurred in the age group of 70 to 80 years. Males were 2.1 times more frequently affected than females. Among our 165 cases we found 124 cases of lung tuberculosis (72.2%) and 41 cases of extrapulmonary tuberculosis (24.8%). These included 17 cases of active tuberculosis of the lymph nodes. As 118 (72.9%) of these tuberculosis cases remained undetected by physicians during the patients' lifetime, each physician should pay more attention to tuberculosis in his diagnostic considerations.

A new human monoclonal cold agglutinin Sa recognizing terminal N-acetylneuraminyl groups on the cell surface.

A human homogeneous IgM/K cold agglutinin (CA) Sa is described, whose corresponding antigen on erythrocytes (RBC) was abolished by neuraminidase. This indicated that the antigen was related to N-acetylneuraminic acid, similar to Pr and Gd antigens. In contrast, this antigen was only partially destroyed by proteases, whereas Pr antigens are completely destroyed and Gd antigens are not influenced by proteases. Sa antibody activity was inhibited by sialyllactose NeuAc (alpha 2 leads to 3) (alpha 2 leads to 6) Gal (beta, 1 leads to 4) Glc like anti-Gd but in contrast to anti-Pr. The corresponding antigen was associated with an RBC membrane glycoprotein fraction like Pr, Sa is one of a spectrum of human monoclonal CA against cell surface neuraminyl groups.

Leiomyomas of the larynx.

In a now 65 years old male patient a monochorditis of the left vocal cord was clinically observed over several years. Multiple exploratory excisions finally demonstrated a leiomyoma with atypically growing histological structures. The complaints of the patient permanently increased, and a hazel-nut-sized leiomyoma with atypically growing tissue structures, originating in the left vocal cord, was removed by laryngectomy. The exploratory specimen, which was diagnosed in our laboratory as proliferative pseudosarcomatous tissue 11 months before manifestation of the leiomyoma with atypical structures, is compared with analogous findings reported by Lane (1957) and Sherwin et al. (1963). Our observations, completing the results of these publications, allow the conclusion that pseudosarcomatous tissue structures do not only occur in the surroundings of carcinomas but also in the immediate neighbourhood of mesenchymal tumours. Choice of the adequate site for exploratory excision is decisive for correct histological diagnosis. A comparison with the literature concerned shows that leiomyomas and leiomyosarcomas rarely occur in the laryngeal region.
