Radiation effects of iodine-125 and iodine-131 on the overactive rat thyroid.

The effects of graded doses of iodine-125 and iodine-131 on rat thyroid glands under prolonged stimulation with oral thyrotropin-releasing hormone were assessed by histological and autoradiographic studies. No evidence for a difference in effect between iodine-125 and iodine-131 was found. The reason for this is suggested.

Zinc metabolism and thyroid status.

Urinary zinc excretion was found to be increased in patients with thyrotoxicosis when compared to euthyroid individuals. This increase did not appear to be due to an increase in creatinine clearance and was found to be strongly correlated with an index designed to assess the severity and acuteness of the metabolic response of the body to hyperthyroidism. The potential clinical significance of this finding is discussed.

The assessment of 125I treatment of thyrotoxicosis.

257 patients have been reviewed 1-5 years (mean 3 years 2 months) after receiving one of five dose regimes of 125I for thyrotoxicosis. The cumulative incidence of hypothyroidism was 34% and of persistent thyrotoxicosis 17%. The group receiving doses between 351 and 500 muCi/g had the highest proportion of euthyroid patients (65%) with the lowest requirement for repeat therapy (46%). In the euthyroid patients, increasing dose of 125I was associated with progressive decline in mean thyroxine (T4) level and free thyroxine index (FTI) within the respective normal ranges, and increase in mean thyroid stimulating hormone (TSH) level to above the normal range. Euthyroid patients with elevated TSH levels had significantly lower T4 and FTI values compared with those with normal TSH, and showed a 3-4-fold increased rate of development of hypothyroidism over 1 year. Euthyroid patients with elevated T3 levels remained euthyroid during the subsequent year and mean T3 levels declined significantly, suggesting that abnormally elevated T3 levels after 125I do not generally indicate impending relapse of thyrotoxicosis. It is concluded that the potential admantages of 125I therapy for thyrotoxicosis in reducing the incidence of hypothyroidism have not been realized in practice.

Systolic time intervals in thyroid disease.

The systolic time intervals; total electro-mechanical systole (QS2), left ventricular ejection time (LVET), pre-ejection period (PEP), and ratio of PEP/LVET were measured from external readings in twenty hyperthyroid and fifteen hypothyroid patients. The hyperthyroid subjects showed shortening of the PEP and reduction in the PEP/LVET. The hypothyroid group showed lengthening of the PEP, shortening of the LVET and increased PEP/LVET. These changes returned towards normal in ten patients rendered euthyroid with appropriate therapy. The value of these measurements to predict the thyroid status was examined in a mixed group of seventy-four patients. There was close agreement between diagnosis predicted from systolic time intervals and that made independently from clinical and biochemical assessment, suggesting that the systolic time intervals may have been a useful place in the clinical assessment of thyroid patients.
