[Pathologic enzyme patterns in sonographically verified liver metastases].

The serum enzyme pattern, consisting of GOT, GPT, lactate dehydrogenase, gamma glutamyl transpeptidase and alkaline phosphatase, was investigated in 128 patients with sonographically verified liver metastases. Gamma glutamyl transpeptidase and alkaline phosphatase turned out to be the most sensitive enzymes, being elevated in 89% and 88%, respectively. The GOT was elevated in 45% GPT in 37.5% and lactate dehydrogenase in 56% of all cases. The enzyme elevation did not correlate with the degree of liver involvement. In conclusion, pathological serum enzyme patterns are useful for the detection and follow up of liver metastases. Normal serum enzyme levels do not rule out the presence of liver metastases.

[Pneumatic cold cap for the prevention of doxorubicin-induced alopecia].

Since 1973 several methods were described in the literature to prevent Doxorubicin induced alopecia. Besides the application of pressure scalp hypothermia gained increasing importance. Various modifications of scalp hypothermia were developed which are expensive and not easy in the handling so far. We are introducing a method in which a pneumatic cap is used for the application of ice on the scalp. With this method which is comperatively cheap and easy to be handled we got a protective effect against doxorubicin-induced hair loss which was at least as efficient as the current results with other methods referred in the literature.

[Emergencies in oncology].

In clinical oncology emergency situations are either manifestations of the underlying disease or effects of the treatment. The successful treatment of emergency situations is not only important in diseases in which cure is possible. It contributes also essentially to an optimal palliation for not curable disease states in oncology. Some special examples from the point of view of internal medicine are briefly discussed.

[Acute disorders of hemostasis].

Acute acquired hemostatic disorders are of widespread origin. Coagulation disorders (severe liver damage), platelet disorders (impairments of platelet function and number, immunologic and/or toxic myelosuppression) or various combinations (e.g. disseminated intravascular coagulation). According to the various etiologic factors different diagnostic and therapeutic approaches are appropriate.

[Iron and iron binding proteins in inflammations and tumors].

Iron and iron binding proteins are involved in various regulatory mechanisms in infections and tumors. Chronic infections and tumors are the most common reasons of anemias in hospitalized patients in industrial countries . Although a lot of investigative work has been done to identify the underlying mechanisms many details are still poorly understood. This paper gives a review of our present knowledge from experimental work, reported in the literature together with own results. The main emphasis is laid on new findings about the importance of a possible regulatory role of iron and iron bindings proteins on the surface of cells of the immune system in the immunosurveillance as well as in non immune functions. Using a cytotoxic assay with a monoclonal antiplacental ferritin antibody which was developed by C. Moroz, surface ferritin was not only detected on peripheral mononuclear cells of patients with early stages of breast cancer but also in patients with rheumatoid arthritis which has not been reported so far. Possible connections of inflammatory states with tumors are discussed regarding the results with ferritin bearing lymphocytes together with other findings about the role of cell bound iron binding proteins in conditions of tumor and inflammation.

[Refractory and sideroblastic anemias].

Refractory and sideroblastic anemias are heterogenous syndromes which are characterized by a hyperblastic bone marrow with dyserythropoietic features and disturbances in iron- and hemoglobin metabolism. Various pathogenetic and therapeutic aspects as well as connections with the preleukemic syndromes are discussed.

[High-dose gestagen therapy in breast cancer].

38 patients with metastasizing breast cancer who were stratified according to prognostic criteria were treated with high-dose medroxyprogesterone acetate (MPA). The response rate in our series was nearly 50 percent. The indication for high-dose gestagen therapy as well as anti-tumor activity and side effects are discussed by comparing our results with recent communications in the literature.

[Clinical features and therapeutic management of male breast cancer (author's transl)].

Only between 0.5 and 1.2% of all breast cancers are found in men. Over the past 20 years 17 male breast cancers were treated in the University Clinic of Graz/Austria. Seven of these cases had localized disease and primary surgical treatment was performed; three out of the seven are still alive and show disease-free intervals of 2, 6 and 10 years, respectively. The metastatic cases were treated by various combinations of surgical treatment, radiotherapy, hormonal treatment and chemotherapy. Our results confirm the numerous communications in the literature that the therapeutic approach to male breast cancer is not essentially different to the management of female breast cancer at potentially curable stages. Primary orchidectomy still holds a dominant position in metastatic stages. Nevertheless, hormonal treatment with anti-oestrogens achieved an overall response rate of nearly 50%, which is comparable to the results with primary orchidectomy. Hence, this treatment is becoming of increasing importance as the primary form of treatment in hormone-sensitive metastatic male breast cancer.

[Therapy of paraproteinemic hemoblastoses].

Chemotherapy with alkylating agents and Prednisone can achieve a prolonged median survival time in patients with multiple myeloma which lasts as long as the remission endures. Aggressive therapeutic regimens could not achieve a further tumor cell reduction as soon as a stable phase is reached. Waldenström's macroglobulinemia requires a therapeutic approach, which is adjusted to the individual case. Patients with small tumor mass and therapy responders have a significant prolonged median survival time compared to patients with large tumor mass and non-responders.

[Chemotherapy of non-hodgkin's lymphomas (author's transl)].

Recent progress in the chemotherapy of Non-Hodgkin's lymphomas has been achieved in the management of subtypes with unfavourable histology. Multi drug regimens like C-MOPP, CHOP and BACOP led to high remission rates and in some cases probably also to a cure of the disease. Disseminated low grade malignancy Non-Hodgkin's lymphomas are probably uncurable and request individual therapy adapted to the natural history of the disease.

[The current status of scintigraphy in oncology (author's transl)].

The clinical usefulness of tumor scanning methods is discussed under the practical considerations of tumor screening, determination of tumor size and noninvasive tumor staging as well as diagnostic follow-up after therapy. Scintigraphy as a primary diagnostic tool is mainly restricted to suspicious enlargements of the thyroid gland whereas scintigraphy with 67Ga-citrate in Hodgkin's disease and bone scintigraphy with 99mTc-phosphates in tumors with tendencies to early bone metastases are widely used non-invasive diagnostic means for the evaluation of tumor propagation. Post therapy conversion of positive findings is of less predictive value than than persistence of a positive scan. Some results of the literature are discussed with our own experiences.

[Evaluation of storage-iron distribution in serum and reticuloendothelial system in patients with rheumatoid arthritis (author's transl)].

Serum ferritin, transferrin and the stainable non-heme iron, were evaluated in 20 patients with rheumatoid arthritis in various stages and with different medications. We found a correlation between the serum ferritin values and the amount of stainable non-heme iron in the bone marrow, which was independent to the stage of the disease and the medication. The pathogenesis of the anemia in rheumatoid arthritis is discussed, together with the reported results in the literature.
