Phenotypic and functional reconstitution of peripheral blood lymphocytes following T cell-depleted bone marrow transplantation from partially mismatched related donors.

Myeloablative chemotherapy followed by transplantation of a T cell-depleted bone marrow graft from a partially mismatched related donor provides a potentially curative option for patients with leukemia and other disorders of hematopoiesis, although the patient is faced with a period of sustained immunodeficiency as well as pharmacologic immunosuppression as a result of prophylaxis against graft-versus-host disease. Thirty patients who received one to three antigen T cell-depleted mismatched grafts were evaluated for immune reconstitution. The percentage and numbers of cells expressing lymphocyte subset antigens were determined by flow cytometry at 14, 28, 60, 100, 180, 270 and 365 days post-BMT and at 6 month intervals thereafter. Lymphocyte reconstitution was characterized by the early appearance of natural killer cells and a low percentage of both T and B cells. During the first year after BMT, the number of NK cells remained constant while T and B cells gradually returned to normal numbers and proportions. Response to the lymphocyte mitogen phytohemagglutinin returned to normal over the course of 2 years, while the response to concanavalin A was slightly depressed and the response to pokeweed mitogen became supranormal at about 1.5 years and continued to increase. These data suggest the need for long-term immunophenotypic monitoring as well as prolonged infection surveillance and prophylaxis.

Clinical measures to assess the practice and efficiency of breast self-examination.

If women examine their breasts thoroughly and regularly they may promptly recognize abnormal tissue changes. The level of skill and motivation required for effective breast self-examination (BSE), however, is very high. In a prospective study of 304 patients with newly diagnosed breast cancer, 165 (54%) claimed to practice BSE. On comparison with the remaining 139, the BSE practitioners reported their symptoms sooner than the nonpractitioners, and presented more often with clinically early tumors. There was, however, no difference in tumor size, and pathologic status of axillary nodes between the two groups. There were 60 patients who discovered the abnormalities during self-examination. On comparison with the remaining 244, significantly more BSE discoverers had shorter patient delay in presentation, but there was no significant difference in tumor size, clinical stage, and nodal status. It is concluded that these or similar clinical measures must be used in assessing the effectiveness of population-based BSE campaigns.

Control of photosynthetic sucrose synthesis in barley primary leaves: role of fructose 2,6-bisphosphate.

Levels of fructose 2,6-bisphosphate (F2,6BP) and related metabolites were measured in 8- or 9-day-old barley (Hordeum vulgare L.) primary leaves throughout a 24 hour cycle. Young barley leaves contained about 0.4 nanomole F2,6BP per milligram chlorophyll at the end of a 12 hour dark period. F2,6BP levels increased rapidly following a dark-to-light transition and then decreased to about 0.1 nanomole per milligram chlorophyll after 5 or 10 minutes of light. Low levels of F2,6BP were detected in barley primary leaves throughout the day. A 10-fold increase in F2,6BP was observed during the first hour of the dark period and then levels of this metabolite decreased slowly for the next several hours. Only small diurnal fluctuations were noted in barley leaf glucose 6-phosphate and uridine 5'-diphosphoglucose levels. There were rapid changes in whole leaf F2,6BP levels when the light intensity was altered. High F2,6BP levels in the dark were not observed after short photosynthetic periods. Results obtained with barley primary leaves support the suggestion that F2,6BP is involved in regulating the flow of photosynthate from the chloroplast to sucrose. Extractable sucrose-phosphate synthase activity was inversely related to barley primary leaf F2,6BP levels. This finding may indicate that the activities of sucrose-phosphate synthase and cytosolic fructose 1,6-bisphosphatase in barley primary leaves are metabolically coordinated.

Breast screening clinic versus health education session as outlets for education in breast self-examination.

A population based programme to educate women in breast self-examination (BSE) was organised as part of the UK Trial of Early Detection of Breast Cancer. Women who responded to an invitation to a meeting were educated in groups and were not routinely screened. Open access clinics offered x ray and clinical examination to all women in the study age group of 45 to 64. Women who presented for screening without prior BSE education were taught individually by clinic staff. Women taught by the two methods were surveyed by post and their BSE practice since education was compared. It was hypothesised that the women taught in clinics, who had been reassured of their breast normality, would practice BSE more regularly and correctly. Results did not confirm this hypothesis. Lower standards of practise and a higher level of anxiety at the time of survey were found in the group who presented at a free access clinic and had received individual teaching at the time of screening.

Measuring family functioning with the MMPI: a reliability and concurrent validity study of three MMPI family scales.

Examined the utility of three MMPI family scales. Three MMPI family scales and three criterion family scales were administered to 110 undergraduates. Internal consistency coefficients were calculated for each MMPI family scale. Relationships between the MMPI family scales and the criterion scales are examined by a correlational analysis. Findings suggest that the MMPI family scales are reliable and concurrently valid measures of an individual's perception of interpersonal family relationships. Procedures for scoring and interpreting the MMPI family scales are examined. The findings also indicate which MMPI family scale is the overall best scale.

Breast self-examination: clinical results from a population-based prospective study.

As part of the Department of Health's National Breast Screening Trial a seven year study is in progress in Huddersfield to assess the effect of an educational programme in Breast Self Examination (BSE) on the mortality due to breast cancer among women aged 45-64. The initial cohort of 22,484 women have completed 3 years in the study and show a higher than expected annual incidence rate of breast cancer. There is no significant difference in the incidence rates between those who attended meetings for BSE instruction and those who did not. Similarly there is no difference in stages of presentation of cancers between attenders and non-attenders at these meetings and also between cancers detected in the first, second and third years. Those who discovered abnormalities during self examination, however, presented with smaller lumps compared to other women. Assessment of prognostic factors do not at this time provide sufficient evidence to show that a community-base BSE campaign will result in a significant improvement in the stage of breast cancer presentation.

Diurnal carbohydrate metabolism of barley primary leaves.

The carbohydrate content of barley (Hordeum vulgare L.) leaves was measured over a 24-hour cycle. Nonstructural carbohydrate accumulation was linear after the 1st hour of light, whereas utilization in the dark was fast initially and slowed as stored reserves were depleted. Sucrose was the most abundant storage form of carbohydrate in the primary leaf. Lesser amounts of starch, fructans, and hexoses were also present. Leaf reserves were almost completely remobilized by the end of the dark period. There was a lag in starch degradation following a light to dark transition. Lower rates of starch accumulation were observed at the beginning and at the end of the day. Fructan synthesis occurred primarily towards the end of the light period as rates of sucrose and starch synthesis decreased. The above results suggested that carbohydrate metabolism in primary barley leaves was controlled by light and by endogenous factors such as foliar sucrose levels. Measurements of specific [(14)C]sucrose activity in steady state labeled 7-day-old barley primary leaves suggested the presence of at least two kinetically separate pools. Sucrose levels were higher and apparent turnover rates were lower in barley leaves in comparison to previous studies with other species.

Validity of a computerized on-line MMPI interpretive system.

Three independent, experienced clinicians trained at the University of Minnesota were asked to rate the quality and accuracy of codetype interpretations and associated rules used in the Psych Systems' automated MMPI program. Results showed that this system is rated highly by clinicians. The authors discuss how this approach can be used in validating other automated test interpretation systems and suggest that feedback by expert judges can be used to improve such systems.

MMPI clinical and content norms for a mixed psychiatric adolescent population.

Presented a set of MMPI norms for a mixed psychiatric group of adolescents (N = 1022). Normative information is stratified according to five age categories and includes means and standard deviations for the validity, clinical, and 48 content scales. The usefulness of these norms for clinicians is discussed.

Radiography of breast biopsy specimens.

The radiological features of breast biopsy specimens can be visualized clearly if the specimens are X-rayed in water. This makes it possible to demonstrate on specimen radiographs lesions causing tissue distortions without calcification. By positioning the specimen correctly in water and by obtaining radiographs in two planes, the exact site of a small lesion within a large tissue mass can be demonstrated and the site marked to guide the pathologist to the suspect area.
