The "long-thumb" brachydactyly syndrome.

A unique variety of brachydactyly associated with skeletal and joint anomalies and possible cardiac conduction defects has been observed in three generations of one family in a pattern consistent with autosomal-dominant transmission. This combination of anomalies has not previously been seen together as a single syndrome. We report clinical and radiographic studies in three of the four affected members.

Problem-solving strategies in consultation.

Techniques for overcoming common stumbling blocks in consultation are outlined, and the advantages to consultants of viewing consultation as a mutual problem-solving process are discussed. It is suggested that the consultant-in-training can achieve confidence in this approach only through experiencing the relationship from the other side by gaining first-hand understanding of the situation of the consultee. Strategies towards this end are offered.
