An unusual presentation of temporal arteritis.

Serious ocular and systemic complications may result from inflammatory processes of the orbit. Temporal arteritis is one such process that can affect the eye. The patient described herein is unusual because she presented initially with signs and symptoms similar to orbital cellulitis. As her disease evolved, she displayed marked, hemorrhagic chemosis with some limitation of abduction and resistance to retropulsion on the involved side. Laboratory and radiologic findings eliminated orbital cellulitis, limited Wegener's granulomatosis, and other systemic inflammatory disorders and led to the diagnosis of temporal arteritis. To our knowledge, this complex of presenting signs has not been previously reported.

The development of a new tissue expander for use in ophthalmic plastic surgery.

We describe the first-time use of the tissue expansion principle in oculoplastic surgery. We have designed a tissue expander for use in eyelid surgery. This instrument was successfully used to treat difficult cases of cicatricial ectropion from multiple causes, avoiding the need for tissue grafting. It was also utilized preoperatively so a large forehead scar could be removed with good resultant cosmesis. The potential for this device in oculoplastic surgery is exciting.

The role of sophisticated radiological testing in the assessment and management of epiphora.

In the management of a patient with epiphora, appropriate investigation is important in arriving at the cause of obstruction, and in helping to outline a plan for management. Standard dacryocystography has been modified by utilizing the techniques of intubation, distension, macrography, and subtraction to produce optimal anatomical information. Lacrimal nuclear scanning with the addition of rapid sequence display and computer interfacings produce the most sophisticated functional information regarding tear drainage. The role of plain films and computerized tomography in lacrimal drainage pathway disease is outlined.

Cicatricial ectropion following blepharoplasty: treatment by tissue expansion.

We describe the first use of the tissue expansion principle in oculoplastic surgery. In a patient with cicatricial ectropion a specially designed tissue expander and reservoir were implanted into a subcutaneous pocket, then the reservoir was filled percutaneously with normal saline until the overlying skin was firm. Saline was injected into the reservoir weekly thereafter. Three weeks following placement the unit was removed; the cicatricial ectropion no longer existed. A week later the patient's ocular symptoms had resolved. The potential for tissue expansion in oculoplastic surgery is exciting.
