[Significance of elevated hemoglobulin A1c for the pathogenesis of diabetic retinopathy].

The hemoglobin of 70 diabetics with retinopathy was analysed. 56 patients had pathologically elevated values, 12 values were within the upper normal limit and in 2 cases the findings were normal. This hemoglobin variant is characterized by an increased affinity of oxygen. Clinically and pathologically speaking the elevated HbA1c value could be a causal factor in diabetic retinopathy. Comparisons with sickle cell anemia and thalassemia seem to indicate that hemoglobinopathy and retinopathy are pathogenetically related.

[Dominant infantile macular degeneration].

In view of its dominant mode of inheritance this condition is to be distinguished from Stargardt Macular degeneration, which - as is known - is transmitted autosomal-recessively. In the fluorescein angiogram a central degenerated area can be seen, whereas the peripheral areas of the fundus are not involved. For this reason dominant infantile degeneration of the macula is a separate condition and should be included in the differential diagnosis of macular degenerations.

[The importance of fluorescein angiography in the diagnosis of Stargardt's mascular degeneration (author's transl)].

A comparison between the fundus photographs and the fluorescein angiogram at the beginning of this disease shows the great informative value of fluorescein angiography. At first the distinct functional losses are most important; ophthalmoscopically, only slight pigment irregularities are discernible. In the fluorescein angiogram the picture of fundus flavimaculatus is already distinguishable at this stage of the disease. These findings led support to the view that Stargardt's macular degeneration and fundus flavimaculatus are identical.

[Differential diagnosis of amaurosis fugax by tonography].

In unilateral stenosis of the internal carotid artery tonography shows the differences in amplitudes in a comparison of the two sides; in bilateral stenosis it shows markedly reduced amplitudes with an intermittent absence of the pulse amplitudes. When extrasystoles occur there are characteristic reductions in the amplitudes. As a result, this method of examination can be very important in the differential diagnosis of amaurosis fugax. Thanks to its simplicity and the fact that an ECG can be made simultaneously the method has a wide range of clinical applications.

[Retinal findings in thalassemia (author's transl)].

Two patients had neovascularizations, bleeding and enclosed vessels or pigmented foci on the retina. The analysis of the hemoglobin showed a raised HbA2 fraction, indicating that they could be considered heterozygous gene carriers of beta-thalassemia. An electrophoretic-spectrophotometric method of determining the hematologic changes is described by means of which thalassemia minima can be diagnosed reliably.

[Fluorescein angiographic findings in retinal venous thromboses (author's transl)].

In addition to documenting pathologic vessel changes fluorescein angiography also makes it possible to show hemodynamic changes in retinal thrombosis. An interruption of laminary flow can be seen in the fluorescein angiogram at arteriovenous intersections. The retinal circulation time can be an aid to prognosis. If the interval does not exceed four seconds the prognosis is generally favorable but when it is considerably longer the prognosis with regard to visual acuity is generally poor and, furthermore, sequelae such as rubeosis iridis and secondary glaucoma must be expected. The therapeutic efficacy of laser coagulation can be judged more accurately by photographic and fluorescein angiographic documentation of the course.

[Observations on the mode of distribution of cholesterol in cholesterolosis bulbi (author's transl)].

Clinical and histological findings are described together with the mode of distribution of the cholesterol crystals that could be traced in the anterior chamber, iris, corpus ciliare, vitreous body, retina and choroid. No crystals were found in the cornea, sclera or lens. In the patient's medical history was a trauma that had occurred 37 years before. Histologically, macrophages were visible in the iron reaction. Since these macrophages contained siderous pigment, an intraocular haemorrhage probably played an important role in the development of the crystals.

[Choroideremie in childhood (author's transl)].

By means of fluorescein angiography extensive defects in the pigmented epithelium found in choroideremia can be shown to be present in the first ten years of life. The degeneration of the vessels of the choroid and the functional damage to the retina do not seem to take place until later.

[The fluorescein angiogram in choroidermia (author's transl)].

The fluorescein angiogram makes a widespread degeneration of the retinal pigment epithelium in choroidermia visible. In the female carriers it shows a spot-like picture. This method of examination contributes to the interpretation of the pathogeny and differential diagnosis.
