Prevention of thromboembolism with low-molecular-weight heparin in orthopedic surgery: a 5-year experience.

A regimen of prophylaxis of thromboembolism with low molecular weight heparin (LMWH) and unfractionated heparin (UFH) in orthopedic surgery is presented. Over a 5-year period, 11,905 patients have been operated in all fields of orthopedic surgery in a big trauma center. In 498 cases, deep vein thrombosis (DVT) was clinically suspected and phlebography was performed: 122 patients had confirmed DVT (rate 0.98%); 59.8% of the DVT were limited to the calf. Nineteen pulmonary embolisms (rate 0.16%) were diagnosed (3 lethal). Heparin-induced thrombocytopenia type II was diagnosed in 12 of 122 patients. Only one of these had received LMWH. The related rates of DVT were highest for tibial head fractures (12.4%), total knee replacement (6.7%), and multiple trauma (6.4%). The extent of DVT was most severe in multiple trauma. The results proved the regimen to be of good clinical use and showed that more attention has to be paid to fractures, especially of the knee; and to patients with multiple injuries.

[The medical profession against Anglicization in medicine. Results of a survey].

BACKGROUND AND OBJECTIVE: The growing Anglicization of the German language especially affects medicine. This is true not only in medical research but also for congresses and publications. This leads to far-reaching consequences also for continuing medical education. This questionnaire study obtained the opinions by a selected group of doctors attending a general medical congress. METHODS: Questionnaires were distributed to 365 attendees of the 51. Doctors' Congress in Berlin in June 2002. In addition 345 questionnaires were sent to doctors working at 3 university clinics, 27 clinics at teaching hospitals and 9 at communal clinics throughout Germany. Answers were divided by work place (clinical or non-clinical) and age. RESULTS: There were 360 respondents (188 non-clinical, 172 clinical; 51.1%): 20% reported good self appraised knowledge of written and spoken English. 84.7% were against English as the only language at meetings of German medical societies, while 98.3% were against it for national congresses (except for foreign guests). 84.3% agreed that German and English should be the languages, both of talks and discussions, at international congresses held in Germany. English as the language of specialist journals was less strongly rejected by young respondents. There was no age-related difference regarding the other questions. CONCLUSIONS: The respondents considered it as self-evident that German be the only language at German national congresses and of journals by German publishers directed at German doctors, but certain exception were acceptable. At international congresses held in Germany, both German and English are equally acceptable in order to avoid a large part of the interested German medical community being disadvantaged in their acquisition of medical-scientific information were English to be the sole language.

[Lactic acidosis: a complication of spinal cord injury in multiple trauma].

Large-dose methylprednisolone has been advocated to lessen neurologic deficits in spinal cord injury for nearly a decade despite confounding statistical results in the Second National Acute Spinal Cord Injury Study (NASCIS-2). Recent retrospective studies found lack of significant functional improvement, increases in the incidence of infectious complications and an increase in ventilated and intensive care days in steroid-treated groups. We report on five cases with severe hyperglycemia and nonketotic metabolic acidosis in otherwise non-diabetic patients with multiple blunt injuries and an associated spinal cord injury. Those adverse effects were induced by epinephrine and aggravated by methylprednisolone. We conclude that high-dose methylprednisolone should be avoided in multiple injured or otherwise compromised patients potentially needing catecholamine support.

[Thoracopulmonary complications of fresh fractures of the thoracic spine with neurologic damage].

The number of thoracic injuries associated to traumatic fractures of the spine is increasing. The multiple trauma rate in this retrospective study of 253 spinal injuries with neurological damage, which needed admission to the Intensive Care Unit, was of 55%. Blunt chest trauma was found in 86% of the multiple trauma group, especially in combination with fractures of the upper and middle thoracic spine (up to 100%), while head injuries appeared in only 58%. Hemopneumothorax was diagnosed in 88%, pulmonary contusion in 48% of the cases, leading to a high rate of recidivating atelectasis and pneumonia. Emergent chest computed tomography should therefore be obligatory in thoracic spine fractures.

Associated malformation with anal and rectal atresiae.

A survey of the frequency of associated malformations occurring with anorectal anomalies reviewing a large number of cases has been carried out. The urogenital tract, the extremities and the skeleton are most commonly involved. Esophageal atresia and hypospadias are the most important associated malformations. It was impossible to classify the associated malformations according to the sex of the patient or the level of the anorectal atresia.

Quantitative aspects of ouabain binding to human erythrocyte and cardiac membranes.

1. [3H]ouabain binding to human erythrocyte membranes is a time- and temperature-dependent process. The association of ouabain to the membrane-bound receptor follows second-order kinetics, while the dissociation is a monomolecular reaction. An association rate constant of 4-6 x 10(4) M-1 sec-1 and a dissociation rate constant of 1-4 x 10(-4) sec-1 were measured at 37 degrees C. The dissociation constant calculated from these data agrees with that determined from equilibrium binding experiments. There is only one type of ouabain binding sites with high affinity for the drug as reflected by the low dissociation constant of 0-28 x 10(-8) M. 2. The dissociation constants of the ouabain-receptor complexes from human erythrocyte and cardiac membranes are identical. 3. The maximal number of membrane-bound ouabain binding sites was measured from equilibrium binding experiments as 288 +/- 28 per single erythrocyte. Thus one receptor site corresponds to less than 1 mum2 of the membrane, provided the receptors are diffusely distributed on the surface of the membrane. 4. Neither the maximal number of ouabain receptors nor the affinity for the drug changes with the age or sex of the blood donor. 5. A maximal transport capacity for sodium of 5-6 m-equiv/hr.1. is calculated from the number of receptor sites per erythrocyte and from the turn-over number of the (Na+ + K+)-ATPase.

[Aspergillus infection in skin transplantation and its therapy].

In a 10 years old girl sustaining a corrosive injury of the lower leg from sulphuric acid, in the region of a skin transplantation colonization with Aspergillus fumigatus (Fresenium) and Aspergillus niger (van Tieghem) took place. This infection endangered the attempt of transplantation and the saving of the foot. Treatment by medication with nystatin (moronal) and canesten (clotrimazol) were ineffective. Pimaricin (pimafucin, natamycin) quickly erradicated the mycotic infection and secured an undisturbed progress for the transplantation. Additionally the epidemiology of infections by Aspergillus is briefly discussed.
