[Appraisal of Lyme borreliosis].

Lyme borreliosis is due to infection with a tick-borne spirochete. The variety of signs and symptoms and also the laboratory tests of this multisystem illness often cause great problems in the appraisal of this disease. Frequently indispositions are associated with positive antibody tests for Borrelia burgdorferi. Terms as fibromyalgia or chronic fatigue syndrome are often connected with the diagnosis of Lyme disease. Outdoor workers such as farmers, foresters, hunters, woodcutters and gamekeepers in areas of endemic disease take a great occupational risk of infection with borreliosis. In the German health and social insurance the appraisal of this disease is of great importance. Affected working people can receive financial compensation. Not only serological investigations with the presence of specific antibodies, but also clinical findings must be considered.

[Modern illnesses from the critical viewpoint].

Psychosomatic illness as "modern diseases" are of increasing interest to the public. Environmental illnesses, for example assumed intoxication with organic solvents, multiple chemical sensitivity, sick building syndrome, chronic fatigue syndrome, fibromyalgia, the influence of amalgam or of electromagnetic waves and ozone are often causes of anxiety. There are many hypotheses about the origin of these diseases. Some scientists emphasize an organic basis; however, this is not generally accepted. Very often with good reason a psychological cause is supposed. Objective diagnostic criteria are not available, therefore these diagnoses may only be applied after sufficient exclusion of other known organic diseases. Mostly a psychological treatment is refused by the person affected, and a scientifically based somatic concept for the therapy does not exist. The medicolegal problems are important and often the reason for prolonged forensic confrontations.

[Bicycle accidents with and without bicycle helmets].

Traffic accidents with bicycles are very frequent, since bicycling has become increasingly popular as a means of transportation and a sports instrument. Therefore, there are many bicycling victims in casualty hospitals. The head being exposed and unprotected, head and brain injuries are the most frequent ones. Just as in professional and sports activities, protection of the head and neck also when bicycling has been urgently recommended and ist effect repeatedly confirmed by recent statistical results. Wearing helmets by motorcyclists has clearly reduced the severity and frequency of head injuries since it became compulsory. A legislative initiative for wearing bicycle helmets as well ist therefore necessary.

[Alzheimer disease--differential diagnosis and modern therapy].

Alzheimer's disease and other dementias are a multidisciplinary challenge and become more and more important because of the growing life expectancy, and also due to the presence in the media. A. Alzheimer published first 100 years ago the disease named after him as an entity marked by the clinical course and by typical histological findings. The last decades brought remarkable progress in demarcation to other forms of dementias for example by CT or MRI. The cause of Alzheimer's disease has not yet been cleared up. The finding of secondary dementias has great importance since causal therapeutic possibilities exist. In the last years new ideas to etiology in respect to the hypothesis of acetylcholine resulted in new therapeutic promises. A rational treatment of this disease seems now possible. Besides psychological and behavioral signs and symptoms of dementia are of great importance in the care of these patients.

[Fibromyalgia--a dispensable disease term?].

The term of "fibromyalgia" has been used increasingly in the last years for chronic widespread pain in particular concerning soft tissues, muscular and extra-articular system including pain in 11 of 18 tender point sites on digital palpation. Scientific proof of an organic disorder could not be established to this day. Psychological causes are more and more considered to be responsible for this problems. Nowadays a psychosomatic disorder is assumed although as well a depression with somatization or a neurosis are discussed. Therapeutical problems and pain coping strategies are described just as the medico-legal assessment for pension scheme. Because the term "fibromyalgia" suggests an organic disorder which does not exist, it seems instead useful to prefer the terms "somatization disorder" or "pain disorder" to make easier the approach to early psychotherapy and to prevent a further chronification.

[Neurological problems in environmental medicine].

It has been tried to demonstrate the role of neurology in the new medical discipline "environmental medicine" or "clinical ecology". Encephalopathy and polyneuropathy are the most common diseases related to neurology following the exposure to low levels of environmental substances. The problems of measuring are discussed, also the difference to other causes of illness. Important are also psychological aspects in the whole context of environmental medicine. The importance of amalgam, organic solvents and ozone is explained in their relation to neurology and clinical ecology. Mentioned is also the difficult concept of "multiple chemical sensitivity". Referring to the environmental consultation an open and evaluating attitude is recommended to avoid damage as a result of environmental poisoning equally to exaggerated medical activities.

[Traffic accidents from the social medicine viewpoint--a medical history connection with present time].

A historically excursus seems sometimes useful to overlook actually problems in medicine. The example of the railway accidents shows the problems of the medical appraisal with regard to the consequences of an accident. After initiation of a third party insurance in Germany 1871 with reference to the new means of transportation a lot of people tried to get a compensation after imaginary or real injury following a railway accident. Very soon physicians had to give an expert's opinion on the complaints of those concerned. 1879 Rigler and 1918 Horn published monographs regarding this problem. Apart of real impairments very often complaints had no organic reason. Oppenheim introduced 1889 the concept of "traumatic neurosis". The scientific discussion remained in the following decades and finally it was rejected until to our times. Today the terms "posttraumatic stress disorder", "psychogenic pain disorder" or "adjustment disorder" are valid. In present times the problem of the whiplash injuries takes the place of the railway accidents. Here also are either organic reasons discussed or psychogenic reactions presumed. The modern techniques in neuroimaging and neuropsychology are inconsistent and the results not generally accepted. In the individual case they are often little helpful. The medicolegal problems are still important and often reason for prolonged forensic confrontations. The discussions more than 100 years ago seem nowadays still actual. The terms have changed, the problems remain.

[Expert assessment of railroad accidents at the beginning of the industrial age--a medical historical excursion with reference to current circumstances].

[Expert assessment of chronic fatigue syndrome].

The Chronic-Fatigue-Syndrome (CFS) has been first described in 1988 and has been also in Germany recently more frequently diagnosed. It is similar to a lot of other terms, especially to "neurasthenia", which has been introduced 1869 from Beard and is now again content of ICD-10. CFS is defined by primary and secondary criteria, which are however largely subjective. There are no objective signs. It is unknown if this syndrome represents a disease entity of its own. The explanation is either exclusive organic based on immunological and virological findings or exclusive psychogenic as a special form of anxiety psychosis. Possibly are both factors involved as part of "psycho-neuro-immunology". CFS is increased subject of medical certification. It has been tried to give a practical guidance to the assessment of CFS.

[Responsibilities and status of the medical expert].

Expertising by assessment is an elementary, specific medical assignment, which in the history of medicine has early been linked to medical activity. No community can do without it, though all along critical voices have accompanied this occupation. The expert is in a field of tension between the claims of the individual and the reasonable interests of the community and the insurance companies. This results in points of attack and criticism challenging for the expert and his function. There should be consensus of opinion in respect of the responsibility and position of the medical expert among the members of the medical profession. This paper will make a contribution to this problem regarding the role of the expert and his self-image.
