[Pulmonary artery arrosion due to parietal endocarditis--a late complication of a haemodynamically ineffective VSD (author's transl)].

A case of late complication after operative correction of a valvular pulmonary stenosis is reported. Chronic pulmonary embolism secondary to bacterial endocarditis situated opposite to an uncorrected small ventricular septal defect in the right ventricle caused arrosion of the pulmonary artery and subsequent severe fatal bronchial hemorrhage. The relation between VSD and bacterial endocarditis and the surgical consequences are discussed.

Relationship between the uptake of 3H-(+/-)metaraminol and the density of adrenergic innervation in isolated rat tissues.

1. The uptake of 3H-(+/-)metaraminol (MA) by tissue slices or pieces was studied in vitro in several peripheral rat organs of varying density of sympathetic innervation (the tissue level of endogenous noradrenaline ranging from 1.7-99.1 nmoles/g). In each individual tissue preparation amine uptake was corrected for entry into the 14C-D-sorbitol space. 2. When the tissues were incubated with 1.4 muM MA, the rate of total amine uptake (i.e., neuronal plus extraneuronal uptake of MA) remained virtually constant for up to 7 min. Therefore, rates of uptake were determined after 2 min of incubation with substrate concentrations ranging from 0.25-12.2 muM. In all tissues the total uptake of MA was saturable. 3. Under the condition of inhibition of neuronal uptake by the presence of 100 muM cocaine, the uptake of MA (considered as extraneuronal amine uptake) was no longer saturable. When tissues were exposed to 1.4 muM MA, the relative contribution by extraneuronal (measured in the presence of cocaine) to total amine uptake (measured in the absence of cocaine) was inversely correlated with the log endogenous noradrenaline content. 4. After correction of the rates of total MA uptake for the cocaine-resistant distribution of the amine, a saturable component of uptake was obtained for each tissue. This uptake was considered to be neuronal; it was subjected to kinetic analysis. 5. Apparent Km values for the neuronal uptake of MA were very similar in all tissues and did not show any dependence on the tissue level of endogenous noradrenaline (average Km = 1.2 muM). 6. V max values for the neuronal uptake of MA were linearly correlated with the endogenous noradrenaline content of the tissues (r = 0.976; P less than 0.001), the V max for the vas deferens being excluded. When related to the content of endogenous noradrenaline, the V max obtained in the vas deferens was lower than that for all other tissues. 7. The results presented here strongly suggest that the membrane site involved in neuronal amine uptake (operationally characterized by the Km of MA) is likely to be identical in all rat tissues and that the number of uptake sites available per nerve terminal does not vary greatly between tissues.

The daily course of the symptomatology and the impaired time estimation in endogenous depression (melancholia).

The importance of 'typical' diurnal variations in mood (depression worse in the morning) as a diagnostic criterion of endogenous depression has been challenged in previous investigations. Disturbance in time experience may contribute towards an understanding of diurnal variation in depressive symptomatology. To examine this hypothesis a series of time estimation experiments was conducted in the course of the day. Twenty-five endogenous depressive patients (according to ICD-9) and 12 healthy controls were asked to estimate prospectively a 30-s interval on two successive days at 7.30 a.m., 11.30 a.m., 3.30 p.m. and 7.30 p.m. Simultaneously the subjects assessed their state of well-being using a visual analogue mood scale. A circadian rhythm of time estimation errors could not be detected; even in daily courses with 'typical' diurnal variations an increasingly favourable self-assessment of well-being was not accompanied by a corresponding diurnal fluctuation of time estimation. These results cast additional doubts on the significance of 'typical' diurnal variations in depressive symptomatology.

Influence of lipid infusion (0.4 g/kg/hr) and positive end expiratory pressure (8 cm H2O) on pulmonary function and hemodynamics in healthy anesthetized pigs.

Fat emulsions are used increasingly for parenteral nutrition in premature infants suffering from various disorders, including respiratory insufficiency necessitating artificial ventilation with positive end expiratory pressure (PEEP). Both PEEP and lipid infusions (LI) may alter pulmonary hemodynamics. The simultaneous effect of LI and PEEP were therefore investigated. Five adult anesthetized Göttinger minipigs were infused with a 20% LI at a high rate of 0.4 g/kg/hr for 30 min, followed by PEEP of 8 cm H2O for 15 min. Catheters were inserted into the upper vena cava, the pulmonary artery, the right and left atrium, and the aorta, and pressures recorded continuously. Ventilation volume, respiratory fractional gas concentrations of O2 and CO2 (mass spectrometer), and blood gases were measured. The following parameters were calculated: total peripheral resistance, pulmonary arteriolar resistance, right-to-left shunt (QS/QT) dead space ventilation (VD/VT) and effective compliance. Total peripheral resistance remained unchanged. Pulmonary arteriolar resistance increased significantly during PEEP, PEEP + LI, but not during LI alone. QS/QT increased significantly during LI and returned to normal when PEEP was applied. VD/VT and effective compliance did not change during LI. The increased right to left shunt, caused by LI, is reduced by means of PEEP, while the pulmonary arteriolar resistance increased with PEEP and LI.
