Amniotic fluid and plasma concentrations of pregnancy-associated plasma protein-A (PAPP-A) throughout pregnancy: comparison with other fetoplacental products.

The development of a sensitive radioimmunoassay has enabled measurements of pregnancy-associated plasma protein-A (PAPP-A) to be performed from early pregnancy. The present paper compares the plasma concentrations of PAPP-A with the levels of two trophoblastic proteins, human placental lactogen (hPL) and the beta-subunit of human chorionic gonadotrophin (beta-hCG), with a steroid of fetoplacental origin, total oestriol (total E3), and with a fetal protein, alpha-fetoprotein (AFP). PAPP-A was also measured in amniotic fluid and in maternal urine. In contrast with the secretion of the other substances studied, which either reach a plateau or even decrease during the last 4 weeks of pregnancy, PAPP-A steadily increased in the maternal circulation from 7 to 40 weeks gestation. It is proposed that PAPP-A production is either not related to placental mass or that PAPP-A is not of trophoblastic origin. The increase of PAPP-A in amniotic fluids parallels the increase in maternal blood; virtually no PAPP-A is excreted in urine.

Endometrial and plasma concentrations of pregnancy-associated plasma protein-A (PAPP-A).

Pregnancy-associated plasma protein-A (PAPP-A), oestradiol and progesterone levels have been measured by radioimmunoassay in plasma and in endometrial homogenates of 20 women undergoing hysterectomy. Results were grouped according to the histological stages of the endometrium. In plasma, oestradiol and progesterone concentrations changed from proliferative to secretory stages in the well-established pattern of the menstrual cycle, but PAPP-A levels did not change. In endometrium, oestradiol levels were high during the proliferative stage and low in inactive and secretory endometrium. Endometrial PAPP-A and progesterone concentrations increased from inactive to secretory stages, but only the increase in PAPP-A was statistically significant. A positive correlation observed between endometrial PAPP-A concentrations and plasma oestradiol/progesterone ratio suggests a possible hormonal control for the presence of PAPP-A in the uterus.

[Detection of tartrate-resistant isoenzyme 5 of acid phosphatase in the white blood cells by disc electrophoresis in a case of hairy cell leukemia].

In a case of hairy cell leukemia the evidence of tartrate-resistant isoenzyme 5 of phosphatase (TRISP 5) developed to findings important for the diagnosis. The evidence was achieved by means of disc electrophoresis by separating the cell lysate of white blood cells. The percentage of TRISP 5 in the total activity of acid phosphatase in leukocytes amounted to 17.6%. In cytochemical respect the evidence of TRISP 5 proved to be negative. The classification of these findings and their differential-diagnostic significance are discussed.

[Clinical significance of HDL-cholesterol].

In a survey the authors deal with the metabolic role of the HDL and the HDL-cholesterol. Via several mechanisms (LCAT-substrate, esterification of cholesterol and transport of cholesterol, competitive LDL-inhibition) the HDL render possible the efflux of cholesterol from the vessels and its catabolism in the liver. HDL-cholesterol possesses a cardio-protective effect. As it was proved by epidemiological, clinical, angiographical and experimental studies, it is reduced in cardiovascular diseases. Its determination, in connection with the calculation of the LDL-cholesterol renders possible diagnostic and prognostic evidence concerning the coronary risk. For males the standard risk is in a HDL-cholesterol of 35-55 mg/dl (0.9-1.4 mmol/l) and for females of 45-65 mg/dl (1.2-1.7 mmol/l). A series of influence factors and diseases which are cited have influence on the HDL-cholesterol-concentration. By physical conditioning, change of nutrition, reduction of weight and different medicamentous measures it is increased.

[Lipoprotein lipase activity in healthy subjects and patients with metabolic disorders].

The activity of the post-heparin-lipase as well as of the cholesterol and triglyceride levels of test persons with normal weight and obese test persons of different age groups was established: 1. In normal persons (males and females) in mature age (30-45 years) a decrease of the enzyme activity takes place which is connected with an increase of the triglyceride content. Above all in older females a repeated increase of the post-heparin-lipase activity was observed, which positively correlates with the triglyceride level. While the decreases of the enzyme activity in mature age may be connected with processes of ageing, a repeated increase of the enzyme activity in older persons is perhaps induced by the increase of the triglyceride level. 2. Obese persons show higher enzyme activities in all age groups examined. Also in obese persons in mature age a decrease of the activity of post-heparin-lipase is to be established. The ageing development of the enzyme activity of the post-heparin-lipase is similar to that of normal persons, takes place, however, at a higher level. 3. The cholesterol and triglyceride levels of the obese persons examined were on an average higher than those of normal persons of the same age group. A dependence of age of these parameters was not observed in the adipose patients.

Pregnancy-associated plasma protein-A (PAPP-A) and hCG in early pregnancy.

Pregnancy-associated plasma protein-A (PAPP-A) is a recently described glycoprotein of unknown biological function. The development of a radioimmunoassay enabled us to measure plasma levels of PAPP-A and human chorionic gonadotrophin (hCG) in 12 non-pregnant volunteers and in 159 women in early pregnancy attending the outpatient clinic for legal abortion. PAPP-A but not hCG was measurable in all non-pregnant women. In pregnant patients (with 36 to 86 days of amenorrhea) hCG reached a peak value (163.1-197.6 ng/ml) between the 9th and the 13th week whereas PAPP-A steadily increased throughout this period of pregnancy. Between the 6th and the 13th week after the last menstrual period, levels of PAPP-A increased proportionally more than hCG. This work provides the first evidence of a PAPP-A production in non-pregnant subjects and the very early marked increase of PAPP-A secretion during pregnancy.

[Atypical course of human salmonellosis (author's transl)].

During the last few years atypical forms of human salmonellosis were observed in 27 patients. Most frequent were acute gastroenteritis with bacteraemia or septicaemia, abscess formations in various organs and septic infections with gastroenteritis Salmonellae but no demonstrable gastroenteritis. In addition there were cases of osteomyelitis, peritonitis, one case of infected hip replacement, one of infected adrenal cortical tumour, and one of infected aortic aneurysm. In 24 of the 27 patients there was an underlying disease likely to have favoured the development of such infections. Diabetes mellitus, cholelithiasis and malignant tumours were the most frequent condition. Isolation of the positive organism was obtained from various materials (blood, abscess pus, gallbladder smear, operative specimen). Chemotherapy is definitely indicated in such Salmonella infections. Often additional surgical measures, e.g. to control spread of septic foci, were necessary. Prophylactic chemotherapy is recommended for patients with risk factors in order to prevent bacteraemic-metastatic events.

[Some parameters of lipid metabolism in aging (author's transl)].

In 150 humans of different ages and sexes total-, LDL- and HDL-cholesterol was tested. The HDL-cholesterol content was 1,41 +/- 0,33 mmol/l with women and 1,18 +/- 0,31 mmol/l with men. Older women show a reduction of the HDL-cholesterol level. There is a significant increase of LDL-cholesterol in both sexes with increasing age. Part of the above experimental group were tested for serum lipoproteins using a four-layer polyacrylamid gel separating the alpha-lipoproteins in a 7% gel into 12 or less fractions. The densitograms of the electrophoreses show differences both in age and sex. Men show a strong decrease of the front fractions, especially in fraction 7, with beginning maturity. There is a slight increase in the older and aged man. Women, however, show no significant changes during their lifespan. The prozentual content of alpha-lipoproteins correlates closely with the behaviour of alpha-lipoprotein-subunits with increasing age. Only with women there is a good correlation between HDL-cholesterol and the prozentual content of alpha-lipoproteins. With 111 subjects of different age groups and sexes the postheparin lipase (pHL) was determined. Both sexes show a strong decrease of enzyme activity with beginning maturity, followed by a different behaviour. There is no permanently negative correlation with the triglyceride level. The results were discussed with respect to arteriosclerotic changes in the blood vessels.

[Primary shape change of platelets in vitro (author's transl)].

Studies with interference contrast microscopy reveal that platelets undergo a typical shape change within 30--60' after venepuncture, i.e. swelling, formation of large tentacles, tiny protrusions and vesicles at the platelet surface. This "shape change" can be observed in citrated blood and PRP, heparinized blood and EDTA-blood as well. It is enhanced by low incubation temperatures (4 degrees C, 10 degrees C) and delayed at 37 degrees C as compared with room temperature. An increased number of primarily shape changed platelets is found if platelets are strongly mechanically irritated at blood sampling. The shape change is partly reversible in vitro, it is completely or almost completely reversible in vivo. Some antiaggregating agents inhibit the in vitro shape change at varying degrees (Bencyclan, SH 869 greater than ASA greater than D-Propranolol). The shape change is partly inhibited after oral or i.v. administration of ASA. A typical transformation of platelets into "spheric" forms can be observed following the addition of Bencyclan, SH 869 and D-Propranolol to PRP in vitro. The spontaneous "primary shape change" which occurs in PRP or blood after blood sampling is probably different from the secondary ADP-induced shape change. The primary shape change may influence the results of different platelet function and aggregating tests. The shape change kinetics of "healthy" subjects and patients with Hodgkin's disease differ significantly. The described method may gain more clinical interest in the future.

[The extragradient of agar-gel electrophoresis in hyperlipoproteinemia].

In the present paper we report on the occurrence of an extra band suggesting a possible M-component in a case of hyperlipoproteinaemia with excessive elevation of lipid values. The enhanced apolipoproteins result in a distinct extra band in the alpha2-beta1-interzone of protein electrophoresis. The extra band disappeared after treatment of the hyperlipoproteinaemia.

[Comparative studies on the determination of uric acid with the continuous flow machine, the manual reduction method and the enzymatic color test].

The manual and automatised determination of uric acid by means of phosphotungstic acid as well as the enzymatic test are compared: 1. When the manual reduction method is performed in the deproteinisation losses of uric acid develop (too low pH-value, absorption). For the precipitation of protein the pH-value 3 must not be undercut. Use uranyl acetate (no trichloro-acetic acid). 2. By dialysing of serum the automatized method avoid errors in precipitation. On the other hand, the simultaneous determination of non-uric acid chromogens is valid as for the manual method. Therefore, the automatically established values are higher. 3. Normal regions for the automatized phosphotungstic acid method are: males 4.8--8.2, females 3.5--7.5 mg uric acid/100 ml serum. 4. Comparisons of the quality of the manual and automatized reduction method for the enzymatic colour test are performed: In the precision the methods tested achieve comparable results (VK 2.1--3.5). The investigations of the correctness by means of control sera achieved acceptable results. The enzymatic colour test gives, since scarcely influenced by pharmaca and nearly specifically, the most reliable results. When using reduction methods analytic and preanalytic sources of error (e.g. pharmaca) must vastly be excluded. The automatised reduction method gives the advantage that there do no more appear any losses of uric acid. There is another normal region for the methods tested.
