[Treatment of thyrotoxicosis with 125-iodine: results in 93 patients 3 to 5 years after treatment, and comparison with 131-iodine therapy (author's transl)].

93 patients were treated with 125-iodine for thyrotoxicosis with a small thyroid (less than or equal to 60 gr) and without palpable nodules in the years 1972--1974. The patients were assayed by: clinical examination, radioiodine-three-phase-study and assay of hormones in the serum: thyroxine, triiodothyronine, triiodothyronine-resin-uptake and thyrotropine. The dosage was individually calculated according to the radioiodine uptake, the thyroid weight and the effective half life time of the radioiodine in the thyroid. In 60 patients the thyrotoxicosis was controlled by a single application, and in further four patients two applications eliminated the thyrotoxicosis. Two out of fifty patients with a follow up between 4 and 5 years are hypothyroid. Compared to 131-iodine therapy in a similar group, the incidence of hypothyroidism is lower after 125-iodine treatment. Only long term follow up will show, whether this advantage of 125-iodine in a very selected group of thyrotoxic patients remains. These results have encouraged us to proceed with this trial.

Follow-Up Studies↗

[Nuclear medical determination of the ejection fraction and the regional wall movements of the left ventricle].

Left ventricular ejection fraction was determined in 114 patients by radionuclide and contrast angiocardiography. The correlation coefficient (r) amounted to 0.66. Exclusion of patients with arrhythmias and with a difference in heart rate exceeding 15 beats per minute between the two determinations of the ejection fraction increased r to 0.80 (61 patients). In patients with coronary artery disease and cardiomyopathies (n = 35), r was higher (0.88) than in patients (n = 26) with acquired valve disease or congenital heart disease (0.60). Comparison of first pass radionuclide angiocardiography and gated blood pool scanning with contrast angiocardiography in 19 patients elicited an r-value of 0.82 for the first pass method and of 0.31 for gated blood pool scanning. Visual assessment of local wall motion by gated blood pool scanning in 15 patients showed agreement with cineangiographically quantitated segmental motion (normo-, hypo- or a-kinesis) in 50 of 60 (= 83%) evaluated segments.

[The place of radiation therapy in the treatment of giant cell tumors of bone (osteoclastomas): results from 32 cases (author's transl)].

Radiotherapeutic results of giant-cell tumors of the bone in 32 own cases are presented. The frequency of local recurrency was 30%, the 10-year survival rate was somewhat about 70% and in nearly 12% of primary benign giant-cell tumors there appeared malignant transformations (inclusive two fibrosarcomas in irradiated tissue). In other radiotherapeutic series, the frequency of malignant transformation is between 14 and 19%, in series with primary surgical therapy the rate of secundary malignant transformation is between 3 and 10%. Despite of the limited comparability of the surgical and radiotherapeutic material, we recommend in accordance with most authors in operable lesions surgical treatment. In inoperable lesions or when the lesion is not radically operable, radiation therapy has a definite place in the treatment of giant-cell tumors of the bone with a curative rate of about 80%.

[Results of radiation therapy in chordoma: data from seven patients and a review of the literature (author's transl)].

Seven patients with chordoma who were treated in the Universitätsklinik für Nuklearmedizin und Radiotherapie, Zürich, since 1950 are presented. In all the cases, high-dose irradiation resulted in symptom-free survival of many years duration. Five years after the first treatment, all the patients are alive, after ten years about 50% of the patients are alive. Our own experiences as well as the already published cases show that radiotherapy has an important role in the management of chordoma.

[Influence of percutaneous radiation therapy upon the large salivary glands of the head: results from functional diagnostics using radionuclide techniques in 95 patients (author's transl)].

We investigated the function of the parotid and submandibular glands in normals and in patients with radiotherapy in the head neck region by scintigraphic methods. The above given doses relate to an application scheme with 200 rd 5 times per week. There is a relatively severe loss of function with small doses from 1000 to 2000 rd. After doses till 3000 rd, there is complete recovery of the gland function in about three months. After 4500 rd, there is in about 80% of the patients a complete recovery after about one year. After irradiation with 5000 or more rd, there is nearly in no person a recovery of the gland function. We did not find a compensatory increase in the function of a spared gland with compensation of irreversibly atrophic glands.

[Therapy and prognosis of testicular tumors: the seminoma, results from radiation therapy in 100 patients (author's transl)].

Hundred patients with seminoma were irradiated in our clinic between 1963 and 1975. The 5-year survival rate corresponds to the rate of freedom from disease. It amounts to 97% for Stage I, 84% for Stage II, 55% for Stage III, O for Stage IV, and to 85% for all stages. These results are obtained by means of consequent radiation therapy. It is not cleared up wether general prophylactic irradiation improves the prognosis of mediastinal and supraclavicular affections. No complications may be expected on account of therapeutic doses, as the sensitivity to radiation is high. It is possible, on these grounds, in generalized stages, to take into consideration the total-body irradiation with low doses as an alternative to chemotherapy.

[Therapy and prognosis of testicular tumors: the teratocarcinoma, results from 52 patients (author's transl)].

In this retrospective study, the course of the disease was investigated in 52 patients having been irradiated because of malignant teratoma between 1963 and 1975. The patients were ranged as follows: four cases Stage I, thirty cases Stage II, six cases Stage III, twelve cases Stage IV. All histological preparations were graded once more, using the classification of Pugh and Collins. The 5-year survival recovery rates amounted to 100% in Stage I, to 64% in Stage II, to zero in Stage III and IV, and to 47% for all stages. A remarkable difference of the recovery rates in relation to histological findings was observed: MTIA Stage II=83%, all stages=40%; MTIB Stage II=57%, all stages=33%.

[Methodology of myocardial scintigraphy with thallium 201: limits of pathological findings].

Thallium 201 is a useful agent for demonstrating ischemic myocardia. It is injected during maximum stress, the stress situation is then continued for a further 2 min and a series of scans are carried out 5-25 min after injection. The series is repeated 3-5 h after injection, thereby obtaining pictures of myocardial resting and stress perfusion. Repitition of the study without stress reveals no further data. In follow-up examinations under the same stress situation the perfusion defects remain the same. 27 normal subjects were evaluated and the limits between normal and pathologic scans established.

Coronary Disease↗

[1 year's experiences with the 201 thallium myocardial scintigraphy for the evaluation of coronary circulation disorders].

68 patients with a history of angina pectoris (AP) and/or myocardial infarction, 35 patients with atypical chest pain and 27 healthy volunteers underwent 201Tl myocardial perfusion imaging (201Tl-MPI). In a preselected group of 71 patients with coronary arteriograms, sensitivity was 98.3% and specificity 71%. In patients with AP and positive exercise ECG, 201 Tl-MPI only serves to confirm the diagnosis of coronary artery disease (CAD) (46 pathological 201Tl-MPI of 47 patients). Perfusion defects are a useful adjunct for the diagnosis of CAD in patients with AP and normal or uninterpretable exercise ECG (20 of 21 patients), and in those with atypical chest pain and positive or uninterpretable exercise ECG (25 of 39 patients). A negative 201Tl-MPI virtually rules out CAD.

[Irradiation therapy of vertebral angionomas: results in 62 patients during the years 1939 to 1975 (author's transl)].

62 cases with vertebral hemangiomas and clinical symptoms from 1939-1975, treated by radiotherapy are reported. In about 60%, radiotherapy resulted in permanent significant improvement or freedom of angioma related symptoms. In 7 cases with symptoms of severe compression of the myelon, decompressive laminectomy was performed, followed by radiotherapy. The radiation dose in the vertebrae were: 3000-5000 rd, 100-200 rd single dose, 5 times per week. Hemangiomas of the vertebrae are an accepted indication for irradiation. We recommend doses about 4000 rd, 150-200 rd single dose, 5 times per week. In cases with severe cord compression decompressive surgery is to be performed first.

[131-I cholesterol (19-C) in the functional and localization diagnosis of adrenal cortex disorders].

Results are reported of quantitative adrenal scintigraphy with 131I-cholesterol in 8 patients with normal adrenal function before and after ACTH-stimulation and cortisol-suppression respectively, in one case with an adrenal metastasis from a pulmonary cancer, in two cases with Cushing's disease and in 7 cases with Conn's disease. In normal cases and in patients with M. Cushing a diagnosis of adrenal cortical function is possible with this method but can usually be replaced by more specific biochemical studies of serum and urine. The scintigraphic method is in addition of value in localization studies and can in unequivocal cases replace the selective adrenal phlebography. In patients with M. Conn increased uptake values (bilateral adrenal cortex hyperplasia) as well as reduced or non-detectable uptakes were observed, one of these cases suffering from a cortex carcinoma with M. Conn.

[Radiotherapy results in brain metastases (118 cases)].

Results of palliative radiation therapy in cerebral metastases of various primary tumors are reported. The radiation therapy consisted in a large-volume irradiation which encompassed the whole cerebrum, using single dose of 200 rd (calculated for the center of the skull) five times a week, amounting to a total dose of 4000 rd, sometimes up to 5000 rd. In some cases, a saturating irradiation of a small volume reached 5000 or 6000 rd. A significant improvement in symptomatics was obtained in about 70% of the patients. The average time of survival after accomplishment of the radiation therapy amounted to six months. Own results and the summarized results of the other authors justify the irradiation of cerebral metastases as a rewarding palliative indication.

Brain Neoplasms↗

[Radiotherapy of neuroblastomas. Results in 45 patients].

Treatment results from 45 patients with neuroblastoma are reported. The total survival rate is 29%. In localized stages the survival rate is nearly 100% in patients under one year old and about 50% in patients more than one year old. The survival rate amounts to 50% in stage IV-S, and there is zero survival in stage IV. Comparison with results reported in literature shows the same influences of age, histology and tumor stage on the prognosis. The own results and the reported ones demonstrate that in the majority of the neuroblastoma-patients curative radiotherapy has definite indications.

Abdominal Neoplasms↗

[Radiotherapy results of nasopharyngeal carcinomas (41 patients)].

5-year cure rates after radiotherapy of nasopharingeal carcinomas in the own patients are reported. The 5-year cure rate in squamous cell carcinoma is about 30% and in lymphoepithelial carcinoma Schmincke-Regaud about 50%. These results are comparable to those which are published in the recent literature. The base of the skull should be included in the high-dose volume even in T1 and T2 cases.

[Place of radiotherapy in therapy of nose and paranasal sinus neoplasms. Results in 63 patients].

Therapeutic results in patients with cancer of the nasal cavity and paranasal sinuses of the own clinic are analysed together with some results published in literature. In early cases with cancer of the nasal cavity and cancer of the infrastructure of the maxillary antrum, the results of radiotherapeutic or surgical treatment are equal. In all the other cases -- the majority of the patients -- combined treatment with surgery and high-dose radiotherapy is superior to either treatment modality alone. The 5-year cure rates are: in early cases about 60% and in the other cases about 30%. The distribution of stages in our patients from 1963 to 1973 shows that the majoirty is in a more or less advanced stage of the disease at the time of diagnosis, comparable to earlier reports.

[Radiotherapy of penile carcinoma, indications and results].

The treatment and prognosis of patients suffering from penis carcinoma who were admitted at the department of radiotherapy of the Kantonsspital Zürich during the last ten years are reviewed. Based on these results which are similar to the results of other centers, we were able to show that primary radiation therapy is to be preferred regarding organ function and survival. Only is a few cases surgical intervention for recurrency was necessary. Indications for radiation therapy of lymph nodes are discussed. The five-year survival rate following primary radiation therapy amounts to 70 or 80% with early stages, a secondary surgical treatment being necessary in 5 to 20%. In more or less advanced stages, a five-year survival rate of 40 to 60% is found after primary radiation therapy, and secondary surgical treatment is needed in 20 to 50%. Thus, survival rates obtained by primary radiation therapy which was followed by an operation only in case of recurrences or tumor persistence are comparable to the recovery rates from primary surgical methods. The advantage of primary radiation therapy is evident in comparison with a surgical treatment: The organ is often saved while the survival rate is the same.

[Carcinoma metastases in neck lymph nodes in occult primary tumor: survival rate depending from localization, stadium, histology and therapeutic method (48 patients)].

In patients with metastatic cancer in cervical nodes without evidence of a primary cancer, radiation therapy eventually supplemented by surgery is indicated. The treated volume should include the regions of the possible primary (pharynx, base of tongue, tonsil) besides the cervical node regions. Five-year cure rates are: about 40% in cases with cervical nodes only, more than 50% in cases with cervical nodes in one region only and not fixed, less than 10% in cases with supraclavicular nodes. In about 30% a primary tumor is found later. About 50% of these primaries is located in the supraclavicular volume and about 50% is located in the infraclavicular volume. In cases with supraclavicular nodes the majority of the primary tumors are located in the infraclavicular colume.
