[The exocrine pancreatic function in chronic renal insufficiency (author's transl)].

46 patients, who suffered from chronic renal insufficiency (serum creatinine: 1,4-14,9 mg%) and had not yet been dialysed, were given a secretin-pancreozymin-test. Statistical evaluation of the results and comparison with a healthy reference group showed amylase secretion, total secretory volume and bicarbonate secretion to be significantly reduced in patients with serum creatinine levels above 4,0 mg%. No signs of secondary hyperparathyroidism (laboratory data, osseous changes) were observed. Blood base excess and increased serum creatinine levels were significantly related. Together with influences of low protein diet metabolic acidosis going along with chronic renal insufficiency is consequently considered to be the pathogenic factor responsible for alterations in the exocrine function of the pancreas and for the occurrence of chronic pancreatitis.

[Therapeutic atmosphere in psychiatric wards--an attitudinal study (author's transl)].

Psychiatric care in the Federal Republic of Germany has been consistently improving in recent years. In many psychiatric institutions therapeutic development has moved away from the "custodian" pattern toward a sociopsychiatric and patientoriented therapy. Those who are mainly concerned with this change, namely, the patients themselves, have so far been approached only rarely to give their opinion about conditions in their ward or to say how they experience and assess the situation in their ward. The present study attempts to explore, with the help of a questionnaire, the attitude of inpatients toward the psychiatric institution or ward. The following results are considered to be important: There is hardly any difference in the attitude of the patients toward the four hospitals investigated in Baden-Württemberg; Patients treated in open wards were significantly more satisfied than patients in closed wards; with increasing age, the patients expressed greater satisfaction with regard to the hopes for the future imparted by aimed therapeutic measures, and also with regard to material aspects of their environment; female patients are on the whole more content than male ones; the socioeconomical level and diagnosis of the patients influence their attitude toward the psychiatric institution.

[Computerized data processing for radioimmunoassay (author's transl)].

The computer programm of Marschner et al. ((1974), Arztl. Lab. 20, 184-191) for the calculation of radioimmunoassay data by spline functions was improved with regard to immediate availability of quality parameters. The existing programm was adapted to a computer Siemens 4004/151. Methods are reported for obtaining an exactly reproducible and objective smoothing of spline functions. The additional modifications enable even unskilled technicians to use the programm.

[Broca's aphasia. The clinical picture and a consideration of the neurolinguistic structure (author's transl)].

Broca's aphasia is characterized by disorders on the phonemic, syntactic and lexical level of linguistic description. It is not only the patient's speech which is impaired; abilities to comprehend, read and write are likewise impaired. Articulatory disorders (dysarthria) which are due to an impaired innervation of the phonatory and articulatory musculature may exist. However these disorders do not account for all the linguistic deficits found in cases of Broca's aphasia. The characteristic feature enabling a differential diagnosis of Broca's aphasia is agrammatism.

Orgotein (superoxide dismutase): a drug for the amelioration of radiation-induced side effects. A double-blind, placebo-controlled study in patients with bladder tumours.

Orgotein, the drug version of Cu-Zn superoxide dismutases is a new and safe anti-inflammatory agent. Animal experiments have shown that it does not interfere with the tumourolytic effects of radiation or chemotherapy. A double-blind, placebo-controlled study has demonstrated that orgotein injected after each daily irradiation session can be used safely and effectively to ameliorate or prevent the side effects due to high-energy radiation therapy (8,400 or 6,400 rads) of bladder tumours. Orgotein significantly reduced the signs and symptoms both in the bladder and the bowel, indicating that it provides a therapeutic regimen for control of these side effects, which to date could only be treated symptomatically.

Evidence of possible association of nephrolithiasis and nephroptosis.

Two separate populations, one consisting of 100 patients with renal stones and one consisting of 81 patients with nephroptosis were examined to determine whether an association may exist between excessive renal mobility and renal stone formation. The statistical analysis of the data suggests a direct relationship between nephroptosis and the occurrence of renal stones.

[Global aphasia. The clinical picture and a consideration of the neurolinguistic structure (author's transl)].

Global aphasia is described as a unitary syndrome which is characterized by a severe impairment of all linguistic capabilities. Speech production is extremely limited and consists of stereotyped phrases, recurring utterances or a few isolated words which are usually neologistically distorted. The patients are unable to express their thoughts in a situationally adequate manner. Language comprehension is restricted to simple questions and commands. A clinical description of the syndrome is given and the neurolinguistic structure of the disorders is discussed.

Glucocorticoid and mineralocorticoid actions on gastric secretion in man.

The effects of a 6-day course of treatment with a glucocorticoid (prednisolone, 1 g per day i.v.), a mineralocorticoid (9-alpha-fluorohydrocortisone, 0.3 mg per day orraly), or both the drugs on gastric secretion of acid, protein, pepsin, and N-acetylneuraminic acid (NANA) containing glycoproteins were investigated in 22 volunteers. None of the treatments produced any statistically significant changes in gastric secretion of aggressive factors, acid and pepsin. However, the output of protective NANA bound to glycoproteins was found to be decreased following prednisolone. The prednisolone effect was not prevented by 9-alpha-fluorhydrocortisone given concomitantly. Gastroduodenoscopy did not reveal any lesions attributable to drug adminstration.

[Recent advances and prospects in periodic hemodialytic treatment for patients with uremia].

Following a brief outline of the history of haemodialytic technique, the most relevant recent progress is illustrated. Advances include the success of the internal artero-venous fistula for access to the vessels, of "less than" single "greater than" haemodialytic units, of three-times-a-week dialysis, and the creation of new disposable dialyzers of high efficiency and low priming volume. Some of the problems posed by the patient in periodic haemodialysis are then examined (the problem involved in the risk of hepatitis, that of persistent severe anemia even after haemodialysis has been begun and that of osteopathy) and the most suitable measures for preventing or limiting these situations are presented. Finally, survival indices of patients undergoing periodic haemodialysis are examined and it is concluded that, although various clinical problems have as yet failed to find a complete solution, the patient under haemodialysis can enjoy a satisfactory state of rehabilitation to family and working life and survive even for more than ten years after the terminal phase of uraemia.

Neuropsychiatric symptomatology with chronic renal insufficiency in the stage of compensated and decompensated retention. I. CNS disturbances.

80 strictly selected patients with chronic renal insufficiency with plasma creatinine values of 1.4--14.5 mg% were examined according to a fixed scheme to determine the presence of symptoms and signs of renal encephalopathy. The general cerebral symptoms complained of were headache in 33.4% of the patient material, dizziness in 30.3%, easy fatigability in 62.5%, giddiness in 18.8% and insomnia in 37.5%. The most prominent neurological findings were hyperactive deep reflexes in 30% and action tremor in 23.8%. The symptoms of organic brain syndrome were impairment of memory in 32.5%, weakness of concentration in 28.8% and lability of affect in 63.7%. Diffuse EEG abnormalities were found in 26.2%. While the clinical neuropsychiatric symptoms did not show any statistically significant correlation with the various internal medical data, a trend was observed in the greater number of pathological EEGs with an increase in the impairment of renal function. Furthermore, there was a statistically significant correlation, (alpha less than or equal to0.015) between the occurrence of pathological EEGs and the plasma creatinine and BUN values. It is remarkable that the patients with abnormal EEGs had a relatively low mean creatinine level of 5.89 mg%. The strict dietetic management of the patients is regarded as one of the deciding factors for the relatively low frequency of neuropsychiatric symptoms in the material studied.

Neuropsychiatric symptomatology with chronic renal insufficiency in the stage of compensated and decompensated retention. II. Peripheral nerve disturbances.

80 strictly selected patients with chronic renal insufficiency with plasma creatinine values of 1.4-14.5 mg% were examined for clinical and electrophysiological signs of nephrogenic polyneuropathy. The motor symptoms complained of were cramps in 43.8% of the patients, "restless leggs" in 18.7%, muscular twitchings in 12.5%. It was emphasized that the first two symptoms do not always indicate the presence of polyneuropathy. 30% complained of paresthesias, 5% of "burning feet". The most frequent clinical finding was the impairment of vibration sense in the feet in 37.5% followed by diminshed appreciation of passive movement of the toes in 30%, weakening or absence of the ankle jerk in 23.8% and finally, weakening of the patellar reflex in 5%...

[Behavior therapy and sex therapy].

The author attempts to situate sextherapy of Masters and Johnson in relation to behaviour therapy in indicating the principles elaborated by old behaviour therapy and its most recent developments which are at the origin of sextherapy. He also considers the problem of indications and complementary relation which can be comprehended between these approaches and the psychoanalytical approach.

Despite the variability of its behavioral manifestations Wernicke's aphasia is considered to be a unitary syndrome. According to the criteria of intelligibility, phonemic and semantic paraphasias in spontaneous speech, 4 forms of Wernicke's aphasia are differentiated: 1) with predominantly semantic paraphasias, 2) with semantic jargon, 3) with predominantly phonemic paraphasias and 4) with phonemic jargon. A severe deficit in language understanding is common to all 4 forms. In addition to phonemic and semantic paraphasias paragrammatism is an outstanding feature of the language production in Wernicke's aphasia. After a survey of views about the localization of the lesion and of earlier descriptive models a neurolinguistic explantation of the characteristic symptoms of Wernicke's aphasia is suggested.
