[Computer angiography in renal transplants (author's transl)].

The results of computed angiography in 12 patients with renal transplants are described. In five patients with normal function of the transplant and normal appearances on a conventional angiogram, under-perfusion averaging 36% was demonstrated. In two out of three patients with a stenosis at the anastomosis, the effect of the stenosis on function could be shown. In two patients with acute rejection of the graft and hypoxia of the tubules, the differential diagnosis could be clarified by means of computer angiography. Computer angiography can therefore be regarded as a valuable addition to conventional angiography for the differentiation between tubular hypoxia and acute rejection and for underperfusion of a renal transplant.

Angiography

[Intermittent hydronephrosis].

An intermittent hydronephrosis was observed in a 40-year old patient. The disease pattern is due to an incongruity between the formation of urine and the transport capacity in the ureteropelvic junction. The latent impediment of flow becomes manifest with increased urine secretion. Irreversible renal damage can be the result of the repeatedly occurring hydronephrotic crises.

[Treatment of prostatic carcinoma stage C with combined perineal and transurethral cryosurgery (author's transl)].

31 patients with prostatic carcinoma stage C were treated by combined perineal and transurethral cryosurgery. Additionally in all patients a subcapsular orchiectomy was performed. A distinct tumour reduction was found in all patients. Six months after the freezing a palpable tumour could be detected only in 4 cases out of 29. At different time intervalls after the operation 57 biopsies of the prostate were obtained with the tru-cut-needle. In 14 patients no tumour tissue could be detected whereas tumour cells were found in the biopsies of the remaining patients. Combined perineal and transurethral cryosurgery therefore is a palliative form of treatment in prostatic carcinoma.

Aged

[A new substance for the therapeutic embolisation of renal tumours (author's transl)].

We report on a radio opaque substance for the embolization of kidney tumours which consists of amino acids mixed up with a contrast medium. It is relatively easy to be injected and can be well directed because of its radio-opacity. Past and recent histological examinations of embolized pig kidneys show that this material provides a durable closure of the vessels and that there is no essential tendency of intravasal dissolution. It is therefore also suitable for palliative embolizations. The first results in clinical trial have proved to be encouraging.

Amino Acids

[Functional efficiency of the vesico-ureteral closure mechanism in female patients with recurrent urinary tract infections without reflux].

On account of different relapses of infections in 15 female patients with relapsing non-obstructive infections of the urinary tract without radiologically provable vesico-ureteral reflux altogether 183 washing out tests of the urinary bladder were performed. 83.6% of the tests resulted in a pure bacteriuria of the bladder and 14.2% in a supra-vesical participation. Of the 51 female patients 38 = 74,5% were always suffering only from bacteriurias of the bladder. In 13 patients = 25.5% we also or always found a supravesical participation. 15 female patients underwent a reflux test with technetium-levelled sulphocolloid. In no patient the tracer could be localised in the supravesical urinary tract. By reason of these results we are of the opinion that in the majority of female patients the vesico-ureteral occlusion mechanism does not allow an ascent of the germs. Therefore, the majority of patients is not imperilled by pyelonephritis. Among the female patients with relapsing non-obstructive infections of the urinary tract without radiologically provable reflux are some with marginally competent ostia. In these more or less frequently an occult reflux with ascent of the germs into the supravesical urinary tract may develop. These patients are potentially imperilled by pyelonephritis.

Adolescent

Selective hypothermic perfusion of the kidney for intrarenal surgery.

Clinical experience and physiological consideration of selective hypothermia for conservative renal surgery are described. While surface cooling is satisfactory for relatively short ischaemia times, continuous renal perfusion via catheters offers the best wash-out and a completely bloodless field for lengthy surgical procedures. This is demonstrated by a comparison of three different methods of renal hypothermia in somewhat similar cases. During the last 6 years we have successfully used a continuous perfusion technique by direct introduction of catheters into the renal vessels in 16 selected cases. Advantages and drawbacks of this method are discussed.

Adult

[Catheter embolisation of renal tumours (author's transl)].

Arterial embolisation of renal tumours with foreign particles is described. This involves occlusion of the arteries supplying the tumour through a catheter introduced by the trans-femoral route. Pre-operative and therapeutic embolisation should be distinguished. During pre-operative embolisation, the kidney should be totally occluded by the foreign particles in order to obtain a bloodless operation zone and in order to avoid the much feared production of tumour emboli. Because of the clinical consequences of renal artery emboli, this procedure should be carried out immediately before the operation. During therapeutic embolisation, which is not followed by an operation, only those vessels supplying the tumour should be occluded in order to avoid the complications of a renal embolus. Ischemia of the tumour and less rapid growth may then be excluded.

Adenocarcinoma

[Abdominal and cremaster reflexes after trauma-induced brain death (author's transl)].

The case is reported of a 42-years-old man who was admitted to hospital with a head trauma following a fall and died 12 hours later. Although irreversible loss of cerebral function had been established neurologically, electoencephalographically, angiographically (carotid and vertebral angiography) and scintigraphically abdominal and cremaster reflexes could still be elicited. The neurophysiological conditions for the reappearance of these reflexes are discussed.

A new kidney transport container: a simple vessel for preservation and transport of kidneys and other organs.

A new vessel which improves conditions of preservation and transport of kidneys and other organs is presented. A Dewar container which provides an almost complete thermal insulation against the environment is chosen. The vessel contains the kidney and a cooling solution which melts at 4 degrees C. When it is closed the organ is exposed to this temperature and measurements show this to be constant for 70 h. The melting point can be lowered or raised as required. By choosing a greater amount of the cooling solution the duration of the working period can be prolonged.
