Carcinoma developing in ileostomies after 25 or more years.

Primary carcinoma of the ileal mucosa in patients who have had an ileostomy has been documented rarely. This report describes 4 patients seen within the last 4 yr with ileal carcinoma adjacent to the mucocutaneous junction of an ileostomy 25, 29, 30, and 34 yr after the stoma was formed. The incidence of this serious complication may be increasing.

Covert inhalation of tablet causing fatal pneumonia.

Inhalation of foreign bodies is usually associated with acute respiratory symptoms. We report a case of an elderly woman who died from pneumonia due to covert tablet inhalation. Clinicians caring for the elderly should consider this as a possible cause for pneumonia which fails to respond to usual therapy.

Foamy myocardial transformation of infancy: an inherited disease.

Five cases of foamy myocardial transformation of infancy, a condition which predominantly affects female children under 2 years of age and which causes cardiac arrhythmia or sudden death, are reported. Of these five cases, four occurred in two sets of siblings, suggesting a possible hereditary basis for the disease. As far as we know, no other familial cases have been reported. The other case was of focal disease of the myocardium, as opposed to the diffuse myocardial changes seen in the four familial cases.

Transitional cell carcinoma in an ileal conduit.

Following cystectomy for transitional cell carcinoma a 63 year old woman developed recurrent tumours in the upper urinary tract and ileal conduit, suggesting that malignant transitional cells can auto-implant in ileal epithelium. This phenomenon has not been previously reported. We illustrate the value of retrograde urogram via an ileal conduit following cystectomy for cancer.

Immunohistochemical demonstration of S-100 protein in salivary gland neoplasms.

A peroxidase-antiperoxidase technique for S-100 protein has been applied to 68 salivary glands. These included 17 pleomorphic adenomas, seven adenoid cystic carcinomas, 23 adenolymphomas and a number of other neoplasms. In addition, five specimens of myoepithelial sialadenitis ('benign lymphoepithelial lesion') and five normal parotid glands were included. Consistent results were obtained, myoepithelial cells and cells in myxoid and chondroid areas in pleomorphic adenomas staining intensely. In adenoid cystic carcinoma, on the other hand, there was no staining. The adenolymphomas possessed intensely S-100 protein-positive cells in the interfollicular lymphoid areas; these were probably interdigitating reticulum cells. In addition, branching structures, probably corresponding to Langerhans' cells, were observed in the epithelium of adenolymphomas.

Idiopathic hydronephrosis. Light microscopic features and pathogenesis.

A histological study of surgical specimens of idiopathic hydronephrosis demonstrated an abnormal muscle arrangement at the ureteropelvic junction in 18 of 26 cases. We believe this abnormality to be the cause in these cases. At this junction, the muscle bundles, instead of displaying the normal interwoven (braided) pattern, are arranged into an outer circular and an inner longitudinal layer. This abnormal muscle arrangement can be attributed to local failure of the physiological uncoiling during growth and development.

A quantitative study of eosinophil polymorphs in Hodgkin's disease.

Eosinophil polymorphonuclear leucocytes (polymorphs) were counted in 45 specimens from patients with Hodgkin's disease and five specimens from patients with reactive follicular hyperplasia. The use of chlorazol fast pink BK, a little known stain for eosinophil polymorphs, combined with image analysis facilitated rapid and reliable counting. Significant differences were found between the mean percentages of eosinophil polymorphs in the Rye subtypes of Hodgkin's disease. The numbers of eosinophil polymorphs in specimens from patients with reactive follicular hyperplasia were very low and could not be counted.
