Mutagenicity tests on epristeride in vitro and in vivo.

AIM: To evaluate the genetic effects of epristeride (Epr), a new prospective drug for treating benign prostatic hyperplasia. METHODS: 1) Assaying reverse mutation in histidine nutritional deficiency strain of Salmonella typhimurium 2) detecting chromosome aberrations in Chinese hamster lung cells (CHL); 3) micronucleus assays of mouse bone marrow; 4) counting sperm shape abnormalities 35 d after first ig Epr. RESULTS: 1) The reverse mutation happened at almost the same rate of the negative control. Epr did not induce bacterial mutation. 2) In vitro, the rates of aberration were all below 3%, thus Epr did not induce chromosome damage in CHL. 3) Micronucleated polychromatic erythroblasts (PCE) were not apparently more than those of sovent control, Epr did not induce the formation of micronuclei in PCE. 4) With Epr 818, 682, and 341 mg.kg-1, the head abnormalities of sperms were 5.3% +/- 2.7%, 5.3% +/- 1.9%, and 5.2% +/- 1.2%, respectively. CONCLUSION: No genetic toxicity of Epr was detected.

[Selective tricuspid annuloplasty].

Six cases of functional tricuspid regurgitation associated with rheumatic valvular disease were treated with selective tricuspid annuloplasty simultaneously with MVR or DVR from Sept. 1990 to Feb. 1993. Five cases were followed up for 8-32 months, the primary results were satisfactory and quite encouraging. Authors sum up the merits of this newly developed method are: (1) the technique is relatively simple; (2) it may correct the dilated anulus effectively; (3) not diminish the valve area; (4) no foreign body remains in the atrium. The technique and key point of the method were described in detail.

[Cardiopulmonary-bypass and myocardial protection in warm heart surgery].

Eighteen cases had successful heart surgery (correction of congenital cardiac deformity in 6, valve replacement in 7, and CABG in 5) with normothermic cardiopulmonary-bypass. The main characteristics of warm heart surgery include normothermic perfusion for keeping the body temperature in normal range, that is core temperature at 35-37 degrees C, and arresting the heart by continuous administration of warm oxygenated blood cardioplegia. The cardioplegia was composed of 1 portion of high-potassium crystalloid solution and 4 portions of oxygenated blood. The crystalloid solution was St. Thomas solution adding some Kcl to make the potassium concentration at 80 or 40mmol/L. In the 18 cases, 17 hearts returned beating from arresting spontaneously, but 2 hearts had resuscitation delayed because of hyperkalemia. There were no low output syndrome, serious cardiac arrhythmia and operative death. No myocardial ischemic and reperfusion periods were noted in warm heart surgery so ischemic/reperfusion injury could be almost eliminated. However there were too much blood in the operative field and hyperkalemia tendency.
