Continuous monitoring of plasma antiepileptic drug levels.

Quantitative knowledge of the fate of antiepileptic drugs in individual patients is useful in the prevention, explanation, and correction of subtherapeutic and toxicity-causing regimens. Several variables, such as age and co-medication, require adaptation of the regimen. An interactive computer-assisted laboratory reporting service provides a data base with epidemiological options that helps in the daily management of patients and can supply information on pharmacokinetic parameters, local, regional, and national drug utilization, and disease frequency. Some treatment variables are quantitatively predictable, but many others require individual monitoring of clinical, behavioral, and laboratory observations by a multidisciplinary team of experts. Intensive monitoring by an epilepsy team in an institution for mentally retarded patients has allowed conclusions on the value of therapeutic substances, reduction of the number of simultaneously administered drugs, and seizure registration.

[Symptomatology and treatment of malformations of the inferior vena cava (author's transl)].

A review of the embryology, pathophysiology semiotics and surgical treatment of malformations of the inferior vena cava is given based on 3 clinical histories. The cases consisted of a typical membrane occlusion, an atresia of the infrarenal segment and of a hitherto not described combination of several malformations of the inferior vena cava: membrane occlusion, aplasia of the infrarenal segment of the vena cava inferior and multiple aneurysms of the iliac vein. The symptomatology of malformations of the vena cava inferior depends on the compensation by the collateral or the persisting embryonic veins, respectively, and on the localization and the degree of the obliteration. The varying hemodynamic reactions are described. The following surgical methods for the treatment of inferior vena cava occlusions in the hepatic segment are available: The so called conservative interventions for the creation of collaterals, the direct or indirect recanalization and the bypass operation. Congenital infrarenal atresias of the vena cava are corrected by homologous or prosthetic interposition. The venous replacement with a bovine heterograft presented here, has not yet been described in the literature.

[Malpractice--unnatural death. Medico-legal problems in the diagnostic activity of pathologists (author's transl)].

According to definitions of medical malpractice and of unnatural death it is established that medical measures under criminal principles of causality come into consideration as causes of death even without proof of guilt. That clinical and scientific sections are turned into means of judicial redress by the provisions of the Cemetery and Burial Act is challenged. It is recommended--as has been the case in other European countries for a long time--that all doctors have a duty of compulsory notification of signs of unnatural death.

A silent gene (C3-) producing partial deficiency of the third component of human complement.

A family is described with 3 members in 3 generations being heterozygous for the silent gene C3-: one girl, her mother and grandfather had half normal C3 levels and were apparently incompatible homozygous. No significant deviation in the total hemolytic complement activity, serum concentration of Bf and C4 component was found in the affected individuals.

[Epidemiological aspects of malignant hyperthermia (author's transl)].

A statistical review is given on cases of malignant hyperthermia in Hamburg. The total number of general anesthetics from 1970--1976 was 960 000. 10 cases of malignant hyperthermia have been observed so that a crude estimate of the incidence would be about 1 : 100 000. Some epidemiological aspects of the patients in Hamburg are discussed.
