Ultraviolet laser-induced fluorescence of human stomach tissues: detection of cancer tissues by imaging techniques.

BACKGROUND AND OBJECTIVE: The background for this work was several literature reports on applications of the fluorescence methods to detection and localization of human cancers. The objective of our study has been to investigate if such an approach could be applied for the detection of gastric cancers. STUDY DESIGN/MATERIALS AND METHODS: Our study was designed in such a way that spectrally resolved images of laser-induced fluorescence of human gastric mucosa were collected and assessed from a point of view of elaborating an algorithm allowing for a differentiation between malignant and premalignant lesions and areas of normal mucosa. The method involved exciting the autofluorescence with ultraviolet light (325 nm, He-Cd laser). The images were recorded in vitro in six regions of a visible spectrum using a cooled CCD camera. The material for study was 21 resected specimens for which altogether 72 surface areas were examined. RESULTS: The main result is the observation that a difference of the fluorescence intensities measured at 440 nm and 395 nm, both normalized to intensity measured at 590 nm, differs significantly for the tissues of interest. CONCLUSION: Using that difference as a diagnostic parameter, it was possible to classify malignant tumor tissues with a sensitivity of 96% and a predictive value of 42%, whereas the same approach applied to abnormal but not tumor stomach tissues gave values of 80%, and 98%, respectively.

[Evaluation of early and late results of radical treatment for perforated duodenal ulcer].

The results of the emergency radical treatment of 285 patients with perforated duodenal ulcer are discussed. The authors compared 4 surgical techniques: 1) truncal vagotomy with traditional antrectomy, 2) truncal vagotomy with mucosal antrectomy, 3) truncal vagotomy with pyloroplasty, and 4) proximal gastric vagotomy. The best early results have been achieved in case of the proximal gastric vagotomy. However, the best late results have been noted in case of vagotomy with mucosal and traditional antrectomy. The overall perioperative mortality was 3.5%. Recurrent ulcer has been mainly observed in patients operated with proximal gastric vagotomy several years after the surgery.

[Epidemiologic and demographic aspects characteristic of patients with perforated duodenal ulcer].

Epidemiologic and demographic characteristics of 285 patients operated for perforated duodenal ulcer in 1972-1991 are discussed. A decrease in the number of patients undergoing surgery for the uncomplicated duodenal ulcer is being noted. However, surgery rate in patients with perforated duodenal ulcer remains constant. Only 14% of patients undergoing surgery for perforated duodenal ulcer is under 60. Most frequently such operations are performed in young and middle aged male patients. The obtained results suggest that there is a strong correlation between perforated duodenal ulcer and long duration of the disease, family ulcer history, and smoking.

[Urease test, microbiologic tests, histologic and serologic tests for the evaluation of Helicobacter pylori infections in persons with peptic ulcer and gastritis].

The presence of Helicobacter pylori in the gastric mucosa of 150 patients with gastric, duodenal ulcer and endoscopic gastritis was investigated by rapid urease-based test, culture, histological examination and serology. Overall, 78% were positive for Helicobacter pylori by rapid urease-based test, 69.3% by culture, 53.7% by histological staining and 93.2% by serology (p < 0.001). The frequency of Helicobacter pylori was higher in patients with gastric or duodenal ulcer compared with those with endoscopic gastritis and those after gastric resection. However, the differences were significant only with reference to rapid urease-based test, (p < 0.02). The EIA (Roche) serological test had the highest sensitivity and the lowest specificity. This indicates the seed for serological kit to be independently evaluated on the population to be studied. The rapid urease-based test was simple, rapid and inexpensive, and it was more sensitive and equally specific compared with culture and Giemsa stain.

[Early results of treating Helicobacter pylori infections in patients with gastric ulcer and gastritis].

In the present study, the effectiveness of a triple therapy for eradication of Helicobacter pylori was evaluated. Therapy consisted of 120 mg tripotassium dicitrato bismuthate q.d.s. for four weeks, 500 mg amoxycillin q.d.s. and 500 mg metronidazole t.d.s. for two weeks. In 77 Helicobacter pylori-positive patients with duodenal ulcers (n = 32), gastritis (n = 18) and after gastric resection (n = 7), rapid urease-based test, culture, histology and serology were used to confirm the eradication, or relapse. The overall eradication rate was 75.3%, ulcers were healed in 82.1% and an improvement of the endoscopic gastritis was observed in 75.3% of the patients. The eradication rates were not statistically different among the subgroups. 1-3 months after the treatment IgG titres had fallen by 25% and over in 67.5% of the patients irrespective of the success of bacterial eradication. Side effects, particularly diarrhoea and nausea, were common (53.2%) but mild. Only 6.5% of the patients' had to discontinue the treatment because side effects became intolerable.

[Tissue spectroscopy. New generation of optical methods for cancer detection].

Recent developments in the field of new, non-invasive, sensitive methods of cancer detection based on measurements of autofluorescence of cells are discussed. Research oriented on a detection of human cancer has been carried on by several groups for last six years only but has already yielded important data pointing to a possibility of both in situ and in vitro detection of cancerous tissues in several human organs, especially lung, gynecological tract, skin and gastro-intestinal tract. At least two such methods have been currently subject to clinical tests.
