[Cyclic sciatica and extrauterine endometriosis].

Cyclic sciatica caused by ectopic endometriosis is an unusual cause of compression of a lumbar root, of the sciatic nerve or its plexus. A 38-year-old patient is presented with a history of menstruation associated with right-sided sciatica, which gradually developed over a 2 1/2-year period into disabling constant pain and a lumboradicular syndrome with signs of S1-root involvement. Suggested compression of the sciatic nerve by ectopic endometriosis in the sciatic notch region could be confirmed by CT and MR, by prompt response to hormonal suppression of ovarian function and by regression of the radiological findings. If the history of cyclic pain, local tenderness on deep palpation and the onset of referred pain in the buttock had been observed at the beginning, unnecessary and costly medico-technical procedures delaying diagnosis and treatment (including unnecessary physiotherapy) could have been avoided. Cyclic sciatica is discussed in a review of literature, and possibilities of treatment are discussed.

[Reiter's disease--a review of an interdisciplinary disease].

Over the past 20 years the term 'Reiter's disease' has been expanded from the classical triad of sterile urethritis, conjunctivitis and arthritis to include a broader range of reactive arthritides. Indeed, the classical syndrome as described by Reiter is rarely seen in its complete form, and the presence or absence of active infection in the joint is being redefined in the light of recent techniques of molecular biology. The terminology of Reiter's syndrome, reactive arthritis and seronegative spondylarthropathy are defined here using case examples.

[Pezzi-balls and ergonomic furniture in the classroom. Results of a prospective longitudinal study].

Faulty posture during childhood is an important causal factor for the development of degenerative conditions of the spinal column, which present in adults in the form of back pain, with or without functional disturbances. The reported prevalence of back pain varies widely, although figures of up to 50% have been recorded. The following controlled longitudinal study compared school children in classrooms fitted with ergonomic school furniture to those in a conventional setting. In one classroom, traditional seating was replaced by large air-filled balls (Pezziballs). Before beginning the study and again after one year, every child (n = 46) completed a comprehensive questionnaire and underwent an orthopedic/rheumatologic assessment. At the beginning of the study, 15% of the children reported having already experienced one or more episodes of back pain. Between 15 and 37% of the children demonstrated measurable functional or morphologic changes in different tests. The study showed that the air-filled balls provided a well-received alternative to conventional seating. On the other hand, a greater effect could be achieved by educating teaching staff in the importance of prophylaxis for the prevention of back pain due to poor posture, e.g. in the form of theoretical and practical back-care instruction for teachers. The potential of this group of professionals to act as examples in this respect should be used and supported by school doctors.
