Primary neuritic leprosy in a black South African.

A case of primary neuritic leprosy in a black South African is described, in which the multiple peripheral nerves were affected. The clinical picture and electrophysiological studies are in keeping with a picture of mononeuritis multiplex. Selective involvement of the facial nerve branches with normal blink reflex latencies was observed. The biopsy of the sural nerve disclosed features most consistent with borderline leprosy.

Linear IgA dermatosis and Hodgkin's lymphoma--report of a case in an African and review of the literature.

Linear IgA dermatosis and Hodgkin's lymphoma were diagnosed at the same time in a 47-year-old Black South African man. Skin changes of linear IgA dermatosis responded to a combined treatment with dapsone and MOPP regimen. Prior administration of dapsone alone did not result in improvement of the skin condition. Four documented cases of linear IgA dermatosis and lymphoma outside the gastrointestinal tract, three of them of Hodgkin's type, have been reported in the literature. Review of the literature suggests that different forms of lymphoma seem to be associated with linear IgA dermatosis and dermatitis herpetiformis. The present patient represents the first immunologically verified case of linear IgA dermatosis in an adult Black African.

Hodgkin Disease

Pustular ulcerative dermatosis of the scalp.

Six young African patients are described with erosive pustular scalp lesions leading to extensive ulceration. No bacterial or fungal cause was found. Biopsies showed non-specific changes of atrophy with subacute or chronic inflammation. Four patients were malnourished and anaemic and three were infested with hookworm. The condition did not respond to antibiotics. Healing was obtained in one patient with systemic steroids. The relationship of this disorder to erosive pustular dermatosis of the scalp and to pyoderma gangrenosum of atypical distribution is discussed.

Palmoplantar keratoderma with amyotrophy.

Four members of a South African Black family who have palmoplantar keratoderma with amyotrophy are reported. No neuromuscular defects have been found and the muscle thinning appears to result from disuse atrophy. Volar hyperhidrosis, nail abnormalities and in 2 cases knuckle pads, were additional features. The condition appears to be inherited as an autosomal dominant trait. We are not aware of a similar syndrome having been previously reported.

Juvenile pemphigus vulgaris. A case report.

A 13-year-old South African Indian boy who presented with oral mucosal lesions and skin blisters due to juvenile pemphigus vulgaris is described. Treatment with high-dose oral corticosteroids produced a prompt clinical response. Pemphigus vulgaris is rare in the young and this may lead to misdiagnosis and inappropriate therapy.

Discoid lupus erythematosus in the Nigerians.

Results of a detailed clinical and laboratory study of 37 Nigerian patients with chronic discoid lupus erythematosus are presented. Patients with chronic discoid lupus erythematosus constituted 0.46% of all out-patients seen in the skin clinic between May 1974 and December 1977. A preponderance of females was noticed (female/male ratio of 5:1), while the age distribution of African patients corresponded to values characteristic for the condition seen in other geographical regions. Several morphological types of the condition have been seen. The vitiligoid variant of chronic discoid lupus erythematosus seems to be common in West Africans. Sixteen out of 37 patients presented laboratory abnormalities considered as markers of the association between chronic and systemic lupus erythematosus. Their significance, however is, uncertain as it has been demonstrated on several occasions that in a tropical milieu heavy parasitic infections produce marked immunological disturbances. The problem of the relationship between chronic and systemic lupus erythematosus is discussed and the literature on the incidence of chronic discoid lupus erythematosus in various African countries is reviewed.

Epidermodysplasia verruciformis in Nigerians.

An analysis of the clinical and pathologic characteristics of 12 Nigerian patients with epidermodysplasia verruciformis (EV) is presented. Two morphological variants have been observed, typical wart-like flat-topped papules and larger confluent plaques. Seborrhoeic keratoses were observed in patients with the second morphological variety of EV. Solar elastosis was found in biopsies taken from EV lesions in this subgroup of patients. A particular rarity of seborrhoeic keratosis in Africans is emphasized as no single African case of seborrhoeic keratosis was found apart from patients with EV. No malignant epithelial skin tumours were found in the whole group. Factors underlying an apparently more benign course of EV in Africans are discussed, and the literature on EV in African is reviewed.

Lichen sclerosus et atrophicus in Nigerians.

Lichen sclerosus et atrophicus has been considered very rare in the native African population. Only seven cases, all among the Bantu, have been described to date. Five cases are now reported and these cases are believed to be the first from West Africa.
