[Long-term experiences with sphincteroplasty using the Wreden-Stone method in the management of ano-rectal incontinence].

After showing the reasons for anorectal incontinence and describing the different methods of surgical treatment, the sphincterplasty technique according to Wreden-Stone is presented, as modified by the author in 1962: (1) a greater distance from the incision to the anus helps to reduce the risk of infection; (2) the strip of fascia lata winding around the anal canal is sutured to the musculus gluteus maximus on both sides. The functional results of the nine patients operated on by this method were subjectively much better than the objective results yielded by electro-myographic and manometric examinations; for only one patient were both kinds of results poor. Other methods are also discussed. At present the new treatment using smooth muscle from the large intestine is preferred, possibly combined with the sphincterplasty presented and additional use of striated muscle.

[Diagnosis and therapy of pancreatic carcinoma (author's transl)].

Report of 143 cases of pancreatic carcinoma and the operations carried out. In 14 cases (operability rate 10%) a pancreaticoduodenectomy was performed. The operative lethality rate came to 14,3%. The longest survival time was 7 1/4 years, the shortest 11 months. Early diagnosis, early laparotomy and operation as radical as possible are emphasized.

[Cystic-stump-syndrome (author's transl)].

When persistent or recurrent pain appears following a cholecystectomy, precise clarification is required (ERCP). A long stump of the cystic duct should be cited as one of numerous different causes. If reoperation is indicated, the stump must be searched for radiomanometry should be implemented in order to ascertain possible choledochus stones or a stenosis of the papilla and redress these in the course of the same operation. The existing stump must be resected in any case. Among 5020 operations for benign diseases of the bile duct (1961 to 1977) 88 cystic duct stump resections were performed without fatalities. With the exception of 2 cases the operation remedied the patients' complaints.

Ampulla of Vater↗

[Pulmonary sequestration (author's transl)].

Pulmonary sequestration is a relatively rare condition in which a part of lung tissue has no or only a secondary connection with the tracheobronchial tree. Blood supply comes from the aorta in anomalous branches. Sequestrations may be intra- or extralobar. Gerle et al. include both types in the general term congenital bronchopulmonary foregut malformation. Two surgically confirmed observations of bilateral intralobar pulmonary sequestrations are reported. The clinical features are recurrent respiratory infections with cough, fever and purulent sputum. X-rays only show nonspecific alterations. Chronic changes in the lower lobes should draw attention to pulmonary sequestration. Thoracic aortography is the procedure of choice in establishing the diagnosis and should always be carried out preoperatively. Segmental resection or lobectomy is the indicated treatment.

[Thyroxine in an amount of 150 micrograms daily for the prevention of the recurrence of goiter].

24 patients, who had undergone a subtotal thyroidectomy, were given 150 micrograms thyroxine daily for 6 weeks for prevention of goiter recurrency. 14 patients had been included in an earlier reported study using 75 micrograms thyroxine daily. Prior to this trial 10 patients had 100 micrograms thyroxine daily; the 14 above mentioned patients had had no therapy for 4 weeks. A significant decrease of the TSH response to TRH is observed when compared with the prestudy regimen. According to this parameter the incidence of hyperthyroidism is increased and the incidence of euthyroidism and hypothyroidism decreased. 2 patients however remained hypothyroid. The before mentioned hyperthyroid patients had only an abnormal TSH-response and were euthyroid when determined by clinical signs or ETR-values. It is concluded, that the dosage of 150 micrograms thyroxin is indicated in patients when reduction of goiter size is desired or a high demand for thyroid replacement is suspected. This (and every comparable) regimen needs a clinical and biochemical control.
