Sorbent membrane dialysis in uremia.

The inclusion of activated charcoal within hemodialysis membranes offers potentially improved plasma clearance of creatinine and middle molecules. However, the carbon becomes saturated with continued use and beyond 1 h removal of solutes is by dialysis alone. Two independently conducted crossover studies, to assess the efficacy of sorbent membrane dialysis (SMD) in the treatment of uremia, found predialysis urea levels increased by approximately 15%, creatinine by 10-15%, and inorganic phosphate levels by 10-18% on SMD compared to conventional hemodialysis. One study also observed "middle molecule' (peak "b') levels elevated. No differences were observable in the clinical status of patients. The results suggest that the charcoal content of the SMD device is too small to effect any advantages over conventional dialysis.

[Formation of metastases in the ovaries in operated squamous cell carcinoma of the cervix uteri (author's transl)].

Formation of metastases in the ovaries in carcinoma of the uterine cervix is a rare occurrence. 749 ovaries were subjected to histological examination in 980 operated patients with squamous cell carcinoma of the cervix. A metastatic carcinoma in the ovary was identified in four patients (0.5%). In three patients, a tumour had formed simultaneously in the corpus uteri. This might point to a metastatic spread of the tumour in the same manner as in carcinoma of the endometrium. In view of the low incidence of metastases in the ovaries adnexectomy would not be generally justified.

[The operated adenocarcinoma of the cervix uteri (author's transl)].

Out of a total of 1092 women operated because of cervical carcinoma, 4.8% had an adenocarcinoma of the cervix. Compared with patients with squamous cell carcinoma, there was no difference in signs and clinical pattern with those patients who had adenocarcinoma of the cervix. Treatment results, too, corresponded with those of patients with squamous cell carcinoma of the cervix.

[Bursting pressure of colon in the rats and proteinase inhibition (author's transl)].

In the early postoperative period (third postoperative day) the colonic enterotomies show 45 per cent higher bursting pressure following administration of Aprotinin than the control group (p less than 0,02). On the fifth postoperative day no difference was noted between both groups. The interpretation of the results is difficult because specific parameters such as collagen content and collagenase activity were not determinated. The relationship between colonic anastomotic breakdown and collagenase and therefore the question of collagenase inhibition have to be discussed. It is suggested that activation of procollagenase is prevented because trypsin and kallikrein are inhibited by Aprotinin.

Prodromal states in schizophrenia.

The vulnerability-stress-coping (VSC) model is the most influential heuristic concept in understanding the course of schizophrenia, whose prodromal status still offers unsolved conceptual and methodological issues. Improved knowledge about the prodromal phase would provide a better understanding of the developing psychopathology and psychophysiology of schizophrenia and could also be of predictive value to attune therapeutic actions to the course of the illness more precisely. To shed more light on the characteristics of prodromal states, data from a German multicenter study on intermittent versus maintenance neuroleptic long-term treatment in schizophrenia (ANI study) were reanalyzed with respect to the prevalence and profile, nature, time course, and predictive value of prodromal symptoms in impending relapse. The results demonstrate that prodromes are a category of symptoms on their own, but they share variance with other symptom domains. Treatment side effects, psychotic symptoms, dysphoric mood, and social dysfunction are all associated with prodromal states--the direction of this association, however, is still to be clarified. Prodromal symptoms are also related to the neuroleptic treatment strategy and its relapse-preventive efficacy--findings that underscore neuroleptic maintenance medication in preventing both overt and subthreshold psychotic morbidity in schizophrenia.

Benfotiamine in treatment of alcoholic polyneuropathy: an 8-week randomized controlled study (BAP I Study).

A three-armed, randomized, multicentre, placebo-controlled double-blind study was used to examine the efficacy of benfotiamine vs a combination containing benfotiamine and vitamins B6 and B12 in out-patients with severe symptoms of alcoholic polyneuropathy (Benfotiamine in treatment of Alcoholic Polyneuropathy, BAP I). The study period was 8 weeks and 84 patients fulfilled all the prerequisite criteria and completed the study as planned. Benfotiamine led to significant improvement of alcoholic polyneuropathy. Vibration perception (measured at the tip of the great toe) significantly improved in the course of the study, as did motor function. and the overall score reflecting the entire range of symptoms of alcoholic polyneuropathy. A tendency toward improvement was evident for pain and co-ordination; no therapy-specific adverse effects were seen.
