A study was designed in order to determine whether waterfilled intragastric balloons are superior to airfilled balloons for facilitating weight reduction. In the first volunteer the small intestine was obstructed by the dislocated waterfilled balloon. This complication was successfully treated by transabdominal fine needle puncture of the balloon. In contrast to airfilled balloons waterfilled balloons offer two advantages: water does not escape through the very thin wall of the balloon; the intraabdominal location of a waterfilled balloon is easily checked by sonography.
The conservative treatment of achalasia of the esophagus with balloon or mechanical dilators may be difficult, or even impossible, it the esophagus in an advanced illness is so tortuous (sigmoid esophagus) that the dilator cannot be placed in the cardia. An endoscope dilator with the aid of which dilation is readily possible even in such cases, has been developed by various individuals. Our own experience and the results obtained in 11 patients with esophageal achalasia treated with the endoscope dilator, are presented. The particular advantages of this method of treatment are simple, low-risk placement of the balloon in the cardia before and during dilation, and the possibility of inspecting the cardia for complications immediately after dilation.
In a double-blind study comprising 33 patients (24 males and 9 females) the effects of sulfasalazine enema (3%) in acute left-sided ulcerative colitis was tested. The success rate of 73.6% is comparable with the results achieved with the oral treatment. The rate of side effects however was markedly reduced (no side effects occurred in our patients). Compared to the SASP-therapy with suppositories, the enema had the advantage of being applicable to more proximal parts of the colon.
In cases of achalasia of the oesophagus where a metal or balloon dilator cannot be placed into the functional stenosis dilatation--which otherwise is the treatment of choice--is not possible and the patient must be operated on. With a new instrument, an endoscope with built-in dilator, dilatation is also possible in these cases. Operation can thus be avoided as is demonstrated by treatment of a 60-year-old female patient with extensive curvature of the oesophagus in the prestenotic area.

Among 1258 polyps from the lower gastro-intestinal tract removed by rectoscopy or coloscopy and examined histologically there were 744 adenomas, 72% tubular, 27% papillary and 1% villous. 96.5% of all adenomas were extracted from patients aged over 40 years. Four fifths of the tumours were found in the rectum and sigmoid colon. Only 6% of the tubular adenomas were more than 15 mm in diameter, compared with 32% of papillary and 57% of villous adenomas. The special significance of the adenomas lies in their potential malignancy (adenoma-to-cancer sequence).

A yoghurt-spoon with a long handle which could not be removed initially from the stomach during several sessions was finally successfully extracted by a new simple device.
