[Ointment treatment of 1st degree hemorrhoids. Comparison of the effectiveness of a phytogenic preparation with two new ointments containing synthetic drugs].

OBJECTIVES: Investigation of the usefulness of a vegetable-based hemorrhoid ointment in comparison with two other ointments containing synthetic substances, one of which additionally contained corticoid. STUDY DESIGN: Treatment of 90 patients with first-degree hemorrhoids in the surgical department of a University Polyclinic, within the framework of a double-blind three-limb study of 21 days duration. Follow-up examinations on the third, seventh, fourteenth and twenty-first day of treatment. TARGET CRITERIA: Four typical symptoms--pruritus, bleeding, burning sensation, sore sensation--graded by both physician and patient. RESULTS: All three ointments proved highly effective. Both during the course of treatment and at the final examination carried out on the 21st day, no major differences were to be found between the three treatment groups. In the case of some of the test parameters, a positive tendency in favor of the herbal ointment was observed.

[Strengthening of cryodestruction by ultrasonic energy].

The ultrasonic treatment of vital tissue with therapeutically qualified ultrasonic intensity immediate before the cryosurgery leads to a significant enlargement of the plane and the depth of the cryodestruction. This knowledge should be used in the cryosurgical treatment of inoperable carcinomas with extensive tumor mass.

[Screening using Krypto-Haem-SSWR for the early detection of colorectal carcinomas and adenomas].

In the last 30 years, colorectal carcinomas have been stated to occur markedly in all industrial countries. From colonic and rectal malignant tumors, the amounts of 4225 in 1961 and 7220 in 1984 were newly registered in GDR. Thus the number of patients with colorectal carcinomas showed the decrease for up to 70.9% starting from 1961. The biochemical Krypto-Haem-SSWR screening test has been implemented during the last few years by members of proctologic section. Colorectal carcinoma was stated in 48 asymptomatic subjects aged more than 45 years, and colonic adenoma in 83, respectively. At present, the yearly screening of occult bleeding into faeces is the unique measure contributing to early recognition of colorectal carcinomas and precancerous lesions.

[Polyps in the rectosigmoid].

A total of rectosigmoidal adenomatous polyps from 1965 to 1987 is scrutinized. Authors refer to 1564 rectosigmoidal adenomas they diagnosed during the 23 years all along with transanal both surgery and endoscopy removal as well as subsequent histology. Patients are life-long submitted to regular rectoscopies, the adenomas being the initiation of adenocarcinomatous transformation.

[Early diagnosis and treatment of small rectal carcinomas].

After a brief review of literary sources, a total of 145 patients with average age 64.5 years is evaluated. Authors report on that the early diagnosed rectal carcinoma (T of Dukes A or CSI + II in accord with Mason) can be removed within the intact tissue either by transanal route or using the posterior proctotomy. The surgery-related mortality is of 0 per cent, the 10-year criterion recovery reaching up to 90%.

[Sacral anesthesia in proctology].

The experience with sacral anesthesia is reported on a total of 513 proctologic patients. From them, 105 cases were submitted to the sacral anesthesia without extradural morphine application, and 308 cases were anesthesized with simultaneous extradural administration of morphine. Easy in applicability, the sacral anesthesia is such an analgesic procedure which results in no complications and may be employed mainly in elder patients. The anesthetics prolonged in action (those similar in type with Bupivacain, Marcain) together with extradural morphine administration are also effective in respect with postoperative analgesia. An average time of postoperative analgesia represents 53 +/- 23.4 hrs. when 2 mg/kg Bupivacain, 0005% Epinephrine and 5 mg Morphinum hydrochloricum are applied, respectively.

[Ozone-oxygen therapy in proctology].

The diffusion of oxygen through the colon wall could be demonstrated in animal experiment. The relevant rise in PaO2 can be evidenced both in blood vessels and in the liver, whereby a significantly high increase of 250% above the normal value was recorded in the region of the intestinal wall. The venous blood values were at 230%, those in the portal vein at 134%, and in the liver parenchyma at 127%. When ozone is applied, these values are even higher. An increase can also be recorded in humans by means of transcutaneous PaO2 measurements. In proctology, we view the indication of rectal insufflation to be valid for colitis. The present report covers initial results and experience: a further short report on the possibility of treating hepatitis in the same way will be following.

[Experiences with sacral anesthesia in combination with morphine analgesia].

Sacral anaesthesia is an anaesthetic method which is easy to learn, has a low complication rate and can be used also in elderly patients. The application of long-lasting local anaesthetics like bupivacaine in combination with morphine provides good conditions for postoperative pain relief following proctological operations. The extradural injection of 5 mg morphine considerably eases the postoperative period for the patient. The mean value of postoperative pain relief amounts to 53 +/- 23.4 hours in patients given sacral anaesthesia with 2 mg/kg bupivacaine with adrenaline 0.005% and 5 mg morphine. Our results are based on 513 patients of whom 105 were given sacral anaesthesia without morphine and 308 with morphine.

[Percutaneous reabsorption of Tc 99m heparinoid].

The transcutaneous absorption of Tc-99 m Heparinoid was tested by animal experimental investigations. The quantified results showed only a small absorption of Tc-99 m Heparinoid (0.002%). This one has not any pharmacologic effect certainly.

[Significance and evaluation of gingival and bony pockets].

The reproducibility of measurements of gingival pocket depth was tested on a total of 200 patients and assessed by statistical analysis. Significantly differing values were observed only in the region of the upper molars, whereas consistent values were found in all the other regions. The measurement of pocket depth is an easy-to-determine reproducible parameter in periodontal diagnostics. A gross information about the presence and depth of intrabony pockets cannot be obtained from clinical values for pocket depth.
