[Stimulation of prolactin secretion by vasoactive intestinal peptide].

Dopamine has already been recognized as an inhibiting factor of prolactin. On the other hand, the stimulators of prolactin are still judged diversely at the moment. In addition to hypothalamic TRH, serotonine as well as endogenous opiates and VIP are under discussion. The object of our studies was testing the function of VIP in directing prolactin secretion on three different groups of patients: 1. hyperprolactinaemic women of WHO-group VI; 2. patients who had undergone a TRH-test along with an endocrine check-up; 3. post partum in childbed. The determination was made from analyzing peripheral blood by radioimmunoassay. In 20 amenorrhoeic patients of WHO-group VI, their prolactin and VIP levels were determined between 8 and 10 a. m. Only 4 patients showed a VIP-concentration below 20 pg/ml (which represents the limit of detectability). The other 16 patients showed an increased concentration of VIP. In 15 cases there was an obvious correlation of VIP and prolactin concentrations according to the linear regression. In the course of a TRH-test to 20 and 40 minutes after initial blood examination prolactin and VIP were again determined. After 20 minutes all patients showed a threefold increase of the initial prolactin value. On the other hand, VIP increased in 3 cases only. Apart from all those patients who wanted to become pregnant, these three women--being postmenopausal--showed increased initial values already in the unstimulated condition. The third group of patients consisted of 26 women in childbed. Their prolactin and VIP were determined between the second and eight day post partum.(ABSTRACT TRUNCATED AT 250 WORDS)

[Naloxone-induced gonadotropin changes in females with amenorrhea].

Along with 51 amenorrheic patients the pituitary reactional situation was checked by means of an Gn-RH-stimulation test. Simultaneously to all these women the opiate antagonist Naloxone was infused in a further test series. In 7 from 51 women a significant increase of the LH was demonstrated in comparison to the basic values. Comparing the kinetics of the pituitary gland it was evident, that in all cases the gonadotropin emanation was detective respectively missing after the second Gn-RH-bolus, which followed immediately after the first one. The findings seem to proof the thesis that the endorphins mainly influence the gonadotropin reserve capacity. In cases, where it was possible to objectify the influence--the second pool was missing as well.

[Serum endorphins in the perinatal period].

In 22 pregnant women beta-ELIR and N-terminal-beta-Lipotropin serum levels were determined by radioimmunoassay (New England Nuclear) in the last three weeks of pregnancy, at the beginning and at the end of delivery and during 48 hours after delivery. The difference between the results of the two examinations (beta-ELIR and N-terminal-beta-lipotropin) represents with close approximation the beta-endorphin concentration. In 12 patients the beta-endorphin level during delivery was significantly higher than during late pregnancy; in three cases an extremely high beta-endorphin concentration was found in pregnant women with a dystrophic infant. Therefore, beta-endorphin is discussed as an indicator for foetal distress.

[Cytologic endometrial changes following high-dose iridium-192 radiation].

By intrauterine exfoliative smear performed twice in each of seventeen patients, the cytomorphologic effect of an iridium-192 after-loading irradiation was demonstrated. The examinations showed that the cytologic findings were characteristic of the effect of ionizing radiation even in case of the histologically unchanged, not radiosensitive cylindrical epithelium. After the iridium irradiation of malignantly transformed cells of the cylindrical epithelium with 1000 rd, vacuolation and pseudoeosinophilia were the predominant lesions, while after 2000 rd this was above all the presence of macronucleoli. A marked cellular diathesis is a characteristic result of the irradiation with iridium-192. Although hyperchromasia is not an absolute sign of malignancy in case of the adenocarcinoma, it should be mentioned that chromatin concentrations are scarcely found after irradiation.

Menopause-associated differences in female fat patterning estimated by dual-energy X-ray absorptiometry.

The aim of the present study was to describe and quantify the typical changes in fat patterning from premenopause to postmenopause. The absolute and relative fat and lean body mass were estimated using dual-energy X-ray absorptiometry in 461 healthy non-obese females between the ages of 18 and 64 years (x = 43.2). Significant differences (p < 0.001) in absolute and relative fat mass, body weight and body mass index between pre-, peri- and postmenopausal females were observed. Postmenopausal women were significantly heavier (BMI, x = 26.8) than perimenopausal (BMI, x = 24.4) and younger and older premenopausal women (BMI, x = 22.8) and showed significantly higher fat percentages (fat% x = 38.1) in comparison to perimenopausal (x = 36.8) and premenopausal females (x = 31.4). Three indices, upper body composition index, lower body composition index and fat distribution index were calculated. Typical differences in fat distribution patterns between females of differential menopausal status were found. During the premenopausal phase a more gynoid type of fat distribution prevailed, during the postmenopausal phase a more android kind of fat distribution occurred predominantly. The fat distribution during the perimenopause can be interpreted as less gynoid than during the premenopause.

[Significance and incidence of risk factors in pre- and dysmaturity].

Anamnestic and clinical data of 10 654 deliveries were related to premature and dysmature pregnancies. The aim of the study was to get a correlation between special data on the one hand and pre- and dysmaturity on the other hand. The risk of dysmaturity is the highest in gravidae with a gestosisscore of more than 4. The risk of prematurity is the highest in gravidae with bleedings during pregnancy. The most frequent symptom in prematurity was a high cervix-score. It occurred in the prematurity collective in 40.7%. In the dysmaturity collective occurred the abuse of nicotine in 32.9%. Some symptoms have a high prognostic value for a pathological pregnancy, but it was impossible to develop a high risk score following these data. An high specificity was connected with a low sensitivity and vice versa.
