Megabore capillary gas-liquid chromatographic method with nitrogen-phosphorus selective detection for the assay of haloperidol and reduced haloperidol in serum: results of therapeutic drug-monitoring during acute therapy of eight schizophrenics.

A gas chromatographic method using a HP-5 megabore capillary and nitrogen-phosphorus selective detection for the quantitative analysis of haloperidol (H) and reduced haloperidol (RH) in human serum or plasma is described. A 3-step liquid-liquid extraction is applied. The extraction yield of this procedure is 63% for haloperidol at 20 ng/ml. The limits of detection are 0.4 ng/ml for haloperidol and 1.0 ng/ml for the metabolite if 2 ml of body fluid are applied. At 10 ng/ml the within-day precision is 4.5% for H and 8.3% for RH. Serum levels of eight schizophrenic patients have been monitored weekly over a therapeutic period of six weeks. Seven patients mainly had metabolite ratios RH/H < 1 over the entire period of investigation. They exhibited a linear correlation between dose and serum concentration of haloperidol. In contrast, one patient had metabolite ratios RH/H > 1 over the entire period of the study. Due to considerable increased serum concentrations this patient did not show a linear correlation between the dose and the serum level of haloperidol.

EEG-brain mapping. A method to optimize therapy in schizophrenics using absolute power and center frequency values.

In this study EEG-spectral parameters like mean alpha-power values and center frequency values are computed as maps and compared using significant probability mapping. The parameters are used to describe changes of the functional state of the brain after neuroleptic depot injection (Haldol-Decanoate) in schizophrenic patients. The absolute alpha-power increased significantly (p < 0.01), especially within the first week after injection mostly in the left hemispheric regions. The center frequency decreased most significantly (p < 0.02) between the first and second week after haldol depot injection at the right occipital regions. Both parameters are efficient to describe functional changes correlated with clinical symptomatology, even 3-5 days earlier. Thus, EEG power and center frequency mapping can be used very efficiently to estimate the optimal time between sequential neuroleptic depot injections.

[Left hemispheric site of EEG findings in patients with schizophrenic psychoses].

It was the aim of our study to examine the interhemispherical and extrahemispherical integration that is possibly disturbed in patients suffering from schizophrenic psychoses, using an easy method of EEG performance spectral analysis. For this purpose, the EEG at rest and the functional EEG were determined in 69 schizophrenic patients (ICD) and 22 healthy subjects and the EEG reactivity was calculated. Differences especially in alpha-2 reactivity were seen both intrahemispherically and interhemispherically. This supports the concept of left hemispheric dysfunction in patients suffering from schizophrenic psychoses.

Multivariate EEG power spectral analysis in acute schizophrenics.

The electroencephalogram (EEG) during information processing is influenced by specific changes in brain electrical activity. Based on the theory of disturbed information processing in schizophrenics, we analysed auditory stimulus induced EEG changes by Fast Fourier Transformation. The most important of the significant stimulation-dependent EEG power changes were measured in the 0.5-3.5 Hz and 10-13.5 Hz frequency bands in the left frontoparietal lead. In a multivariate analysis the separation of the subjects examined into acute schizophrenics and normals was incorrect in only 21% of the cases (resubstitution rate); using pi-method an error of 31% was estimated.

Repeated acoustic stimulation of acute schizophrenic patients and the habituation of EEG power changes.

Habituation in psychiatric patients to repeated stimuli has typically been described in connection with the measurement of the skin conductance orienting response. Here EEG power spectra of acute schizophrenic patients and normals to 4 trials of an acoustic stimulation were examined for evidence of habituation. In response to stimulation the alpha EEG reactivity was less distinct in schizophrenics than in normals. This is in agreement with the theory of hypovariability or hyperstability of the EEG in schizophrenics. Signs of habituation in acute schizophrenics were clearer in fast beta frequencies of the parietal leads.

Effects of eye color and sex on accuracy in archery.

126 subjects were 47 children in Grade 7, 49 students in Grade 8, and 30 adults, who shot arrows at bull's-eye targets from varying distances. Analysis of variance of accuracy scores indicated that there was no difference for eye color, but boys scored more points than did girls.

[Electroconvulsive treatment of psychotic patients with organic brain damage].

It is reported about 2 patients in whom the decision on electroconvulsive therapy was rendered more problematic by proven organic defects of the brain or a seizure disease. In our opinion, the electroconvulsive therapy is, in exceptional cases, also practicable under these conditions after an exact risk-benefit analysis.

[Functional muscular abdominal distension--a case report].

The rare clinical picture of the functional muscular distension must be demarcated from organic changes which are connected with an increase of the abdominal volume such as intraabdominal tumours, accumulations of fluid or gatherings of gas or pregnancy. In this symptom a functional disturbance is concerned which appears in patients with vegetative lability or psychic peculiarities and by changes of tension of the musculature surrounding the abdominal space leads to the intermittent or chronic protrusion of the abdomen. By means of an anaesthesia the presence of a functional cause can be made evident: after injection of anaesthetics the protruding abdominal wall is subsiding. The knowledge of the picture of the disease saves long diagnostic ways which contribute to the fact to fix labile patients extremely to the observation of their abdomen and to favour the manifestation of a neurosis. The often unsatisfying therapeutic long-term effect is in contrast to the harmlessness of this symptom.

[Multidimensional studies of circadian biorhythm in psychoses].

Subject of the report is methodics for a circadian analysis in case of psychotical diseases. Acquisition and filing of psychopathological phenomenona is effected by means of Strukturiertes Psychopathologisches Erfassungssystem (SPES) and other self-or outside-rating-scales. The diurnal variation of body-temperature is taken as physiological reference function. The results of the analysis, which show an internal rhythmic desynchronization in case of phasic as well as in case of non-phasic psychoses, are given by way of exemplary instances.

[Side effects of bilateral electroconvulsive therapy--comparative clinico-computer tomographic study].

The side effects of a five-times bilateral electric shock therapy were clinically and computer-tomographically investigated. A connection exists between the number of electric shock treatments which are carried out and the expression of a transitory organic psychosyndrome. However, no significant differences in relation to the initial findings were established a week after the completion of the treatment series despite great interindividual differentiation. Neither cerebral oedema or other signs of disturbances were computer-tomographically detectable as results of treatment. CT and clinic findings do not correspond to each other.

[Electronic data processing-assisted epicrisis in clinico-psychiatric documentation of findings].

The medical profession is using computers to an increasing extent, and the graphic epicrises described here, which is intended mainly to augment the conventional epicrisis, is another useful application. It is based on a standardised psychopathological examination and documentation procedure and permits the course of psychopathological syndromes to be demonstrated in an illustrative way.

[Multiple recurrence in progressive paralysis].

In case of progressive paralysis of the insane, repeated recurrence can be explained only by the survival of pathogens inspite of optimal therapeutic conditions, the persistence of the pathogens being possibly produced by turning of the treponemas into L-type forms. In order that the possibilities and limits of recurrence verification will become apparent, reference is made to humoral and immunological changes of state given by way of example.
