[Transurethral electroresection--an alternative to the indwelling catheter of old patients with prostatic hypertrophy].

The people become elder in this time, everywhere. The number of patients with adenoma of the prostate is increasing. Most of them have other diseases, too. A retrospective study (2646 cases in 8 years, exactly specially analysed 469 cases from 1975/1976) was done, comparing the results of TUR and open surgery. Well known is the lower risk of TUR, the lower blood loss, the lower discomfort, the lower nursing in the postoperative period ect. There are not news in this study. But it is important, to recall some things, because it's more and more difficult, to classify an old man as an "inoperable case". Nobody knows, where he should stay. The family does not like him at home (his "dripping", his incontinence etc.) In the veterans house it's the same problem, and from the hospital he is removed, because he is an "inoperable case". The situation of this old man is very bad, because he feels there isn't a place for him and nobody likes him. Each man, also if it's the strongest one, will be broken psychically and physically after a short time. Concerning these aspects, must be enlarged the indication of removal the bladder neck obstruction per transurethral resection, more and more.

[Clinical value of multiple praecordial chest leads for evaluating the course of acute anterior myocardial infarction (author's transl)].

Percordial ST-segment mapping using an improved 48-lead system was evaluated 2-to-3-hourly in 322 single measurements on 24 patients with acute anterior myocardial infarction. Changes in the sum of ST-elevation (capital sigma ST) or in the number of leads with ST-elevation (NST) could be distinguished in two different phases: an initial rapid increase and decrease in capital sigmaST and NST was followed after 6 to 12 hours, by a more plateau-like course, which exhibited, especially in uncomplicated cases, only slight further changes. During the initial phase, marked differences in the maximal rate of decrease were observed for capital sigma ST and NST. In this phase, interpretation of therapeutical interventions using changes of capital sigma ST or NST as reference is therefore very critical. However, during the second plateau-like phase, extension of myocardial necroses is significantly associated with a new steep increase in capital sigma ST. Thus precordial mapping is suitable for rapid and sensitive detection of infarct extension in patients with anterior myocardial infarction. The development of pericarditis or bundle-branch block, which principally can disturb the evaluation of capital sigma ST and NST, occurred only sporadically in a few patients and therefore caused no major problems.

[Experiences with the lengthening of the inferior oblique muscle (author's transl)].

After an introductory survey of the methods used up to now for lengthening the muscle the technique, dosage and indication of the method described by Gonin and Hollwich is described and the results obtained with it on 137 patients with hyperfunction of the inferior oblique muscle on adduction with associated and paralytic squint are communicated.

[Preoperative embolization of the renal artery in kidney neoplasms].

In 15 patients with extensive malignant renal tumours before operation the artery of the kidney concerned was closed by foam substance particles (ivalon) after percutaneous selective catheterisation. The occurring ischaemic pain was controllable by medicaments and eased off after hours. In most cases the nephrectomy followed on the same day, but it can be performed also on the following day. The operation was principally performed from the transverse flank cut in practically bloodless kidney. Compared with the major interventions (transthoraco-abdominal or transabdominal) of the tumour nephrectomy the procedure described in this paper as a more careful one which at the same time meets the principles of modern surgery of cancer and which can also suggested to patients suffering from a severe disease.
