Factors contributing to erythrocyte lithium-sodium countertransport activity in lithium-treated bipolar patients.

Clinical and biochemical factors related to the activity of the erythrocyte lithium-sodium countertransport (LSC) system were investigated during lithium prophylaxis in 27 patients (13 male, 14 female) with bipolar affective illness. No relationship was found between erythrocyte LSC and such factors as age, gender, duration of lithium prophylaxis, quality of prophylactic lithium response, and family history of affective illness. There was a significant negative correlation between the activity of LSC and the magnitude of the erythrocyte lithium ratio both in whole group and in female patients. In seven patients with concomitant hypertension, the relationship between high activity of LSC and hypertension was not demonstrated. The levels of total cholesterol, HDL cholesterol, triglycerides, and potassium related neither to LSC activity nor to hypertension. Erythrocyte LSC in patients with lower TSH levels were significantly reduced compared to patients with higher TSH. The values of TSH were negatively correlated with T4 but not with T3. Concentrations of T3 were positively correlated with plasma total cholesterol levels. These results are discussed in the view of recent findings on erythrocyte LSC.

Alexithymia and somatic conditions in patients with affective illnesses during lithium prophylaxis.

The personality trait of alexithymia was assessed in 95 patients (35 male, 60 female) with affective illnesses, receiving lithium prophylactically, by means of Schalling-Sifneos Personality Scale (SSPS). The mean SSPS score was similar to that of the healthy population: only 8 patients had a score below 50 points. No relationship was observed between alexithymia score and age, diagnostic category (bipolar I, bipolar II, unipolar) and the duration of lithium therapy. Lower SSPS scores (i.e. high alexithymia) were found in subgroups of male patients with essential hypertension and female patients with lithium-induced obesity. The results suggest that alexithymic traits have no relationship to the pathogenesis of affective illness but may play some role in somatic symptoms emerging in affective patients on lithium prophylaxis.

A review of nasal cancer in furniture manufacturing and woodworking in North Carolina, the United States, and other countries.

Nasal adenocarcinoma in the High Wycombe furniture industry of England during 1956-1965 had an annual incidence of 500 to 1,000 times greater than that of the general population. Excesses of nasal cancer have also been described in France, Australia, Denmark, Finland, Italy, and Holland. Interestingly, one limited study in Canada revealed no excess, whereas a more recent one showed a slight excess. In contrast to the strikingly large excesses of nasal adenocarcinoma in other countries, there has never been any evidence of similarly large excesses in the US woodworking and furniture industry. Modern manufacturing conditions may not present the same degree of risk of developing nasal cancer as was present in the English furniture manufacturing industry. The incidence of nasal cancer associated with furniture manufacturing in the United States is examined in considerable detail in North Carolina, the leading furniture manufacturing state. Furniture manufacturing in the state began around 1890 and has grown steadily since. Utilizing statistics available from the North Carolina Department of Vital Statistics, the absolute mortality of nasal cancer in North Carolina was calculated from 1964 to 1977. The average mortality was approximately 3.5 times greater in the furniture manufacturing industry than in the general population.(ABSTRACT TRUNCATED AT 250 WORDS)

Ileocecal endometriosis presenting with abdominal pain and gastrointestinal bleeding.

A 26-year-old woman with enteric endometriosis presenting with cecocolic intussusception, a cecal mass on barium enema, and gastrointestinal hemorrhage is described. Laparotomy revealed cecocolic intussusception, ileocecal endometrial implants, and cecal mucosal ulceration presumed secondary to ischemia of the intussuscepted bowel. Histopathology showed serosal and subserosal endometrial implants without mucosal invasion. A review of the literature of endometrial bowel disease is presented.

Furniture manufacturing: a step-by-step profile.

Standard Industrial Classification (SIC) 25 -- furniture and fixtures manufacturing -- covers a variety of furniture products. These include household furniture, office furniture, and institutional furniture as well as partitions and fixtures. Statistically, household furniture accounts for two-thirds of the establishments, sales, and employment in this SIC category and will be emphasized in this presentation. A profile of the household furniture industry (Table 1) indicates that it is very appropriate to discuss this industry in the context of small plants. With 320,000 employees in 5400 plants, the average plant has less than 60 employees. The largest companies in this industry have fever than 7500 employees and account for less than three percent of the total sales each. Except where furniture manufacturing is a subsidiary or division of a larger company, full-time industrial physicians and hygienists are not employed in this industry. A few of the larger companies have full-time safety managers, and nurses are found in some of the larger plants.

The erythrocyte lithium-plasma lithium ratio in patients with primary affective disorder.

Increasing attention has been given to the significance of intra-cellular concentrations of the lithium ion in patients treated with this drug. The erythrocyte has been the most common cell investigated because of its accessibility and certain similarities between the ion transport mechanisms of this cell and the neuron. Intraerythrocyte lithium is expressed as the ratio of lithium in the cell to the plasma lithium concentration (lithium ratio). The lithium ratio has been reported to be related to a number of clinical variables, including treatment response, clinical state, side-effects, toxicity, diagnosis, and electrophysiological effects. We have investigated the lithium ratio in a large series of patients with a primary affective disorder and in a smaller control group. We found a significantly higher mean lithium ratio in the bipolar diagnostic group than in the unipolar and control groups. There was a trend, not statistically significant, in the unipolar and bipolar groups for females to have higher lithium ratios than males. While not diagnostic, the lithium ratio appears to be another biological variable where bipolar patients, as a group, differ from normals and others with an affective disorder.
