[Inhibition of spontaneous uterine contractions by gestagens].

So far, in spite of the well known pregnancy maintaining effect of progesterone, not one of the many trials could succeed, in using progesterone or synthetic gestagens for tocolytic purposes. Therefore we have investigated the tocolytic effect of gestodene, which is supposed to be the most potent gestagene available at this time and which has, in addition, properties very similar to natural progesterone. In 26 uterine strips, obtained during cesarean section, we found a highly significant decrease of spontaneous uterine activity 10, 20 and 60 minutes after application of gestodene 10(-6) M.

Calcium antagonists for uterine relaxation.

The uterus-relaxant effect of diltiazem, a recently developed calcium antagonist, was investigated in vitro. 24 myometrial strips dissected from the lower uterine segment during cesarean section showed a statistically highly significant (p less than 0.001) decrease of their spontaneous contractility in the presence of Diltiazem 10(-6) M: 82% after 10 min, 100% after 20 and 100% after 60 min (median values). Diltiazem, which is considered to be the most balanced calcium antagonist as far as cardiovascular side effects are concerned, could be of considerable interest also for clinical use.

Determination of light-chain myosin in pregnancy and under tocolysis.

Cardiac toxicity of tocolysis containing hexoprenaline (0.3 microgram/min) and the beta 1-blocking agent metoprolol (0.01 mg/min) was investigated using a recently developed monoclonal antibody arised against light-chain myosin in 15 patients and compared with the results in 51 control subjects. There was an increase in the median and 80th percentile values 2, 6, 12 and 24 hours after the beginning of tocolysis but no statistically significant difference between these values and the normal ones could be shown.

[Magnesium deficiency caused by epirubicin/cisplatin].

In 19 patients receiving a cytostatic therapy with epirubicine and cisplatinum, 31 times in total the renal loss of magnesium was determined using the colorimetric xylidile-blue-method. 16 patients, whose magnesium input postoperatively was equally well known, served as a control. In all these patients a negative magnesium balance could be demonstrated being significantly higher (p less than 0.01) in those receiving cytostatic therapy.

[Keratoacanthoma centrifugum marginatum].

Keratoacanthoma centrifugum marginatum (KCM) is a rare distinct variant of keratoacanthoma. Based on three personal observations and a review of the literature, the authors describe the clinical and histological features of this neoplasm. Clinically KCM is characterized by the lack of a tendency for spontaneous remission and by continuous centrifugal spread. Histologically there is a subclinical, iceberg-like growth pattern. Like keratoacanthoma, KCM is a highly differentiated, biologically benign, non-metastasizing tumour. The treatment of choice is early excision of the tumour.

[Therapy of malignant melanoma with beta-interferons].

Since June, 1985, we have been treating 18 evaluable patients suffering from high risk melanoma with human beta-interferon (HBIF), 10 of the patients received HBIF infusions, 8 patients got subcutaneous injections into the regional lymph nodes. In some of the cases, this treatment resulted in obviously prolonged survival times; at present, however, the effect of HBIF is still beyond calculation.

Combined Modality Therapy

[Chlorinated hydrocarbons in breast milk].

Chlorocarbons, which are used for cleaning purposes in both the home and industry, are of growing interest with respect to environmental pollution. We have therefore, compared the concentrations of chloroform (CH CL3), carbon tetrachloride (C CL4) and tetrachloroethylene (C2 CL4), determined by gas chromatography, in breast milk from 13 puerperal mothers from Innsbruck and the surrounding Tyrol with the respective data in 20 mothers from Linz and the surrounding industrial area. In fact, no elevation in chlorocarbon levels was detected in either group.

[Lead content of breast milk in heavy traffic and light traffic areas of Tyrol].

The lead content of the milk of 28 breast feeding mothers from various regions in Tyrol was examined using flameless atomic absorption spectrometry. These examinations showed that the milk of women living in places with heavier traffic had a seven-fold higher lead level as compared with the data in women from regions with little traffic. This difference was statistically significant. We also determined the lead level of specimens of drinking water from 7 towns in Tyrol. The mean level was half the value found in maternal milk from women living in low-traffic regions.

[Pemphigus vulgaris with involvement of duodenum and colon].

Pemphigus vulgaris should be numbered among the many causes of gastrointestinal hemorrhage. In our present case, the lesions of the intestinal mucosa occurred some time before those of the oral mucous membranes which finally led to the diagnosis. At the beginning of our century, the possibility of intestinal involvement in pemphigus vulgaris was well-known among dermatologists. We suggest that patients with pemphigus vulgaris should be endoscopically examined in order to confirm the clinical observations made in former times by modern scientific methods.

[On the problem of the cardiotoxicity of epirubicin-cisplatin].

Cardiotoxicity is supposed to be the most important limiting factor in chemotherapy with anthracycline derivatives. The left ventricular ejection fraction was measured several times by means of radionuclid - scintigraphy in 24 patients treated with epirubicin - cisplatinum. Pathologic values have been found only in 3 patients, but no correlation with age, cumulative dose or dose/m2 of body surface could be demonstrated.

Antineoplastic Combined Chemotherapy Protocols

[One year after Chernobyl--radioactive cesium in breast milk].

Radioactivity in human breast milk was determined in 32 women in February and March 1987 by means of a Germanium detector. At this time radioactivity increased again by feeding the hay to the cattle harvested in the weeks following the disaster of Chernobyl. 8 (25%) of the values were above 0.3 nCi/l for babies milk recommended by the local authorities.

Accidents

[Treatment of venous-induced ulcer].

The therapy of venous ulcer comprises a variety of qualified methods applied either singly or in combination (Table 1). As many cases require catching up on necessary investigations, we recommend thorough examination before treatment in order to exclude causes other than venous ones; to reveal insufficient transfacial communications. The basic therapy consists of mobilisation and compression. According to the diagnosis, insufficient transfacial communications may be disrupted by means of surgical or sclerosing measurements. Supporting drug therapy, topically or internally may follow.

Clothing

The relation between saliva estriol levels in pregnancy and infant birth weight.

Specimens of saliva were obtained from 106 women with normal pregnancies and from 42 women, whose children turned out to small for dates (birthweight below the 10th centile). Saliva estriol was highly significantly (P less than 0.0001) decreased in the group with a newborn with a birthweight below the 10th centile (500 +/- 340 ng/ml, mean +/- SD) when compared with the group with infants whose birthweight was over the 10th centile (813 +/- 449 ng/ml). Estriol determination in saliva seems to be a simple, non invasive method of aiding the detection of fetal growth retardation.
