[Acute foreign body aspiration as a respiratory emergency in childhood].

From 1957 to 1987 altogether 206 cases of tracheobronchial foreign body aspiration were diagnosed. Two third of the patients were one or two years old. Boys prevailed with 57 per cent. 55 per cent of all foreign bodies were nuts. Only 10 per cent were radiopaque (screws, nails, needles or pieces of bones). The attempt to eliminate the foreign body via the bronchoscope was successful in 96 per cent and failed in seven cases. Six children had to undergo a thoracotomy. In one third of the cases the foreign body remained two weeks or longer in the bronchial tree. 66 children with such a "chronic" foreign body were later on examined by bronchography, which showed in 29 per cent severe deformations of the bronchial wall and in 14 per cent even bronchiectasis. An acute foreign body aspiration should always be considered and handled as an emergency.

[Fatty acid pattern in children with mucoviscidosis in relation to the degree of exocrine pancreas insufficiency].

Fatty acid composition were determined by gas chromatography in serum triglycerides and cholesterol esters of 32 patients (aged 7-17 years) with cystic fibrosis. According to their exocrine pancreas function (determined with secretin-cholecystokinin test), patients were divided in 3 groups. In comparison with previously measured values in 30 metabolically healthy children, patients with cystic fibrosis showed a significantly increased level of palmitic, palmitoleic, stearic, oleic, and linolenic acid in the triglyceride fraction and a significantly increased level of myristic, myristoleic, palmitoleic, stearic, oleic, and triene acid in the cholesterol ester fraction. However, in both fractions, concentration of linoleic acid was significantly decreased. It seems unlikely that malabsorption of ingested fat is responsible for the abnormalities in fatty acid pattern, because no significant differences could be found between the 3 patient's groups.

Bronchial secretions and bronchial mucosa in children with cystic fibrosis: comparison of bronchoscopic, biochemical, bacteriological, microscopic and ultrastructural findings.

In children (mean age 12.1 +/- 2.9 years) with cystic fibrosis, 44 bronchoscopic examinations were done under general anaesthesia with muscle relaxation using a Friedel type ventilation bronchoscope. The endoscopic picture of the mucous membranes was compared with the state of the bronchial secretions, its bacteriologic findings and content of acid mucopolysaccharides and DNA fibres (semiquantitative estimations). In all patients biopsy of the mucous membrane (central part of the bronchial tree) was performed for light and electron microscopy. The degree of reddening, swelling of the mucous membrane and hypersecretion was in some agreement with the intensity of the cellular infiltration and the production of pus (microscopic investigation). Secondary ultrastructural changes were detected in nearly all children, consisting of cellular oedema, swelling of mitochondria, dilatation of the endoplasmatic reticulum, protrusion of cells and fusion of cilia, enlarged intercellular spaces, thickening of the epithelial basal membrane, increased number of goblet cells, microtubular abnormalities of the cilia, lesions of the apical cell membrane with loss of cilia and microvilli. These ultrastructural changes were not correlated with the above-mentioned signs of inflammation.

[Bronchial secretion and bronchial mucosa in children with mucoviscidosis: results of comparative bronchoscopic, biochemical, bacteriologic, light and electron microscopic studies].

Twenty two patients with cystic fibrosis, 7 to 17 years of age, underwent each two bronchoscopic investigations, the second six months later than the first, in general anaesthesia with muscle relaxation using a ventilation bronchoscope and taking small biopsies of the mucous membrane. The degree of reddening, swelling of the mucous membranes and hypersecretion was in some agreement with the intensity of the cellular infiltration and the production of pus (microscopic investigation). Secondary ultrastructural changes were detected in nearly all children: e.g. cellular oedema, protrusion of cells and fusion of cilia, microtubular abnormalities of the cilia, lesions of the apical cell membranes and increased number of goblet cells. These changes of ultrastructure were not correlated to the mentioned signs of inflammation.

[Results of outpatient clinic care of children with bronchial asthma and chronic recurring bronchitis].

More than 4000 children with chronic and recurrent bronchopulmonary diseases have been cared for in the Dresden district, GDR (just under 2 million inhabitants) in 28 out-patient clinics over the past 7 years. The prevalence of this disease group is thus around 1.2%. There was a marked increase in children with bronchial asthma from initially 1000 to 1600, corresponding to 26 and 40%, respectively, within the disease group. By contrast, there was a continuous decrease in children requiring bronchological investigations (24 to 13%). Bronchiectasis was also encountered with diminished frequency. One third and one quarter of the patients dropped out of care over two three-year periods, respectively. Of these 70 to 75% were discharged symptom-free. The remaining patients were referred to adult out-patient clinics for continued care. Whereas nearly 80% of the discharged children with chronic or recurrent bronchitis became symptom-free, only 30% of the asthmatics showed this satisfactory result. The analysis of this large group of patients confirms the clear-cut success of out-patient clinic treatment of children with chronic and recurrent bronchopulmonary disease.

[Detection of the incidence of increased sensitization to molds in patients with bronchial asthma and allergic rhinitis].

The incidence of sensibilization against extracts of moulds produced by VEB SSW Dresden was investigated by intracutaneous tests in 354 persons (controls and patients - adults and children - with bronchial asthma and allergic rhinitis). Even in the nonallergic controls the tests were positive in 20-30%. Patients with asthma and rhinitis were significantly more often positive than controls with Mucor, Cladosporium and mixed-mould. There was no difference between the groups with Aspergillus and Penicillium. Testing with extract of mixed-mould is not effective. Positive results with the individual moulds were not excluded although the test with mixed-mould was negative. There was no essential difference in the incidence of sensibilization between adults and children with bronchial asthma using mould extracts of VEB SSW Dresden.
