Characterisation and molecular typing of Burkholderia pseudomallei: are disease presentations of melioidosis clonally related?

Eighteen cases of culture positive melioidosis caused by Burkholderia pseudomallei, were seen in four geographically separate communities in North Queensland, Australia. The genetic inter-relatedness of the clinical isolates were compared utilising random amplification of polymorphic DNA (RAPD) and multilocus enzyme electrophoresis (MEE). The isolates segregated into two groups that correlated with clinical presentation rather than geographical location. This is the first described association between the varied clinical presentations of this condition and specific molecular type. If proven on larger studies, this may further our understanding of the pathogenesis of this important condition.

[Aging and cancer. An analysis of cancer mortality in Germany 1900 to 1990].

The unification of Germany rendered possible the comprehensive overall calculation and subsequent analysis of cancer mortality of an entire population of 60 to 80 million people over a period of 90 years. 1. The total mortality rates (all forms) in the various age groups are largely constant. The increase in absolute terms is a consequence of the changing age distribution in the German population resulting in a larger number of people in higher age groups. 2. The age-related constancy of total cancer mortality can be confirmed statistically via longitudinal and cross-sectional approaches focussing on changing subject groups per calendar year as well as in a diagonal manner using the same group of subjects per year of birth. 3. Each age group has its own characteristic and exponentially-at higher age-increase cancer mortality rate which is independent of influences by e.g. personal environment or "generation". 4. The constancy of total cancer mortality implies a change in the distribution of organ manifestations which equilibrates any increases or decreases. Available data on organ manifestation of cancer from the year 1950 on are the basis of this observation. 5. Age-related constancy of total cancer mortality is a basic principle of cancer epidemiology.

[Reflections on the 90 year existence of the German Central Committee for the Control of Tuberculosis].

The Deutsche Zentralkomitee zur Bekämpfung der Tuberkulose (German Central Committee on the Fight against Tuberculosis) is one of the oldest institutions of this kind in the world. Already at the end of the past century, it was the starting point of the fight against tuberculosis at the international scale. It created a cooperation between state and private initiatives, and between scientific work and management, based on voluntary congregation of all partners. In spite of considerable political and social changes, its effective work could be continued during 90 years. As future task remains the world-wide fight against tuberculosis. Its elimination has nowadays become feasible.

Communicable Disease Control↗

[Control priorities in tuberculosis in the endemic stage].

The author derives the consequences of his study for tuberculosis control in FRG from the statement, that the self-eradication of tuberculosis will begin, when a lower number of new sources of infection will be produced by the actually existing infectious cases. Self-eradication may be accelerated by measures of control, above all by chemotherapy, nevertheless, the epidemiological regular course will not be broken but the transition from tuberculosis epidemic to endemic will occur only quicker. The trend of tuberculosis in the different areas, countries, and counties of the FRG is being investigated by comparing the dates of the years 1894/97, 1977, and 1980. The development is characterized by a west-east-trend of the disease and by the formation of centres of relatively higher frequency. Consequences for the future tuberculosis control are drawn emphasizing it in the still existing areas of relative accumulation.

Disease Outbreaks↗

[Epidemiology of carcinosis. 105 years of mortality statistics (author's transl)].

Cancer mortality in Hamburg, calculated per 10,000 living inhabitants of each age group of men and women for the years 1873 to 1978 proved to be essentially constant after eliminating sources of statistical error. Calculation or organ involvement by the same method for 1933 to 1978 showed that the increase in mortality for one type of location is compensated by a decrease elsewhere, so that altogether the constancy remains. This behavior can be explained if it be assumed. This behavior can be explained if it be assumed that a certain constant percentage of the population is prone to cancer and - possibly due to the action of exogenous noxae - becomes ill when the necessary age is reached. The nature of a determining factor which is most probably present is not known. Comparison with international figure shows that the results of the Hamburg Study can be considered a model case.

[The tuberculosis situation in the German Federal Republic (author's transl)].

Based on a statistical anlaysis of the records the present tuberculosis situation in Germany and how it has developed during the past years are reviewed. The main means of control are: BCG vaccination, mass radiography and public health measures. Suggestions made by the Central Committee for the Control of Tuberculosis concerning surveillance and protection against occupational exposure to tuberculous infections are discussed. Recommendations regarding the assessment of incapacity for work and degree of disablement are evaluated. The aims and plans of the Committee for the next years, also in the international field, are reviewed.
