Dynamic activity of bladder neck and external sphincter in ejaculation.

The bladder neck shows a typical electromyographic pattern in ejaculation. It consists of periods of heightened activity rhythmically alternating with intervals of reduced activity signifying muscular contraction and relaxation. This behavior of the bladder neck is confined to its ventral part. Concurrently, the external sphincter shows a short period of increased activity before the typical rhythmical pattern which corresponds to that of the ventral bladder neck.

Vesicourethral continuity in bladder neck activity.

Electromyographic study of the bladder neck during vesical filling and emptying was performed in anesthetized dogs. Increased electrical potentials denoting active contraction of bladder neck were obtained on vesical filling. The bladder was then completely divided just above the vesical orifice, and the resulting apertures were closed to form two separate compartments. Still increased electrical potentials of bladder neck were recorded on filling the proximal compartment. We conclude that vesical neck activities are not directly dependent on the detrusor or on any anatomic continuity between bladder and urethral muscles.

Mercaptopropionylglycine: a progress in cystine stone therapy.

During a 4-year period 9 cases of severe recurrent cystine stones and cystinuria were treated with mercaptopropionylglycine. The oral long-term administration revealed the high effectiveness of the drug and resulted in no further stone formation. Mercaptopropionylglycine is free of side effects and more effective than D-penicillamine.

Urogenitodynamics of the male bladder neck. 1. During continence.

The dynamic behavior of the male bladder neck during urinary continence was studied by electromyography in dogs and rabbits. On vesical filling, increased electrical activity in the form of biphasic potentials was recorded from the anterolateral segment of the bladder neck. These biphasic potentials became more frequent and higher in voltage with further filling of the bladder. Concurrently, the posterior segment showed no significant electromyographic changes. These findings indicate muscular contraction of only the anterolateral part of the bladder neck during vesical filling. We conclude that the sphincteric function of the bladder neck during urinary continence is achieved by the active muscular contraction of its anterolateral segment. We postulate that the posterior segment is mainly concerned with the genital function of the bladder neck.

Urogenitodynamics of the male bladder neck. 2. During voiding.

The dynamic activity of the male bladder neck during voiding was studied by electromyography in dogs and rabbits. On vesical filling, increased electrical activity in the form of high voltage biphasic potentials was recorded from the anterolateral segment of the bladder neck. With the onset of voiding these high voltage potentials disappeared completely and only a low voltage base line activity was recorded. This indicates muscular relaxation of the anterolateral bladder neck. Concurrently, the posterior segment of the bladder neck showed insignificant electromyographic changes. We conclude that opening of the vesical outlet during voiding is achieved by the muscular relaxation of its anterolateral segment. The dynamic activity of the bladder neck is reciprocally coordinated with the detrusor muscle to produce continence and voiding of urine.

Diagnosis of renal tumor by gamma-glutamyltranspeptidase (EC 2.3.2.2).

A urinary enzyme pattern and kidney tissue pattern were investigated simultaneously in 117 urologic patients. In contrast to all other renal disorders only the sixteen malignant tumors of the kidney showed a significant drop of gamma-GT in tumor tissue and urine. So far urinary enzymology has been used only as screening test. Measurement of gamma-GT in urine, however, permits the diagnosis of kidney tumors.

Calcium oxalate stone disease: congenital defect of metabolism?

Kidney tissue enzyme patterns from patients with calcium oxalate stone disease were compared to enzyme patterns from healthy and inflamed human kidneys. In the case of oxalate stone disease there was a significantly decreased activity of the glycolytic enzyme aldolase, resulting in a cumulation of oxaloacetate and of oxalic acid. This decrease of the aldolase activity does not completely reveal the cause of calcium oxalate stones but provides a new aspect to the disease.

Bleomycin effects on malignant tumors of the male genitalia.

Bleomycin was applied alone or in combination with actinomycin D, vinblastine, radiation and/or surgery in cancer of the penis (6) and testis (6). In early stages of penile cancer (T2 N0 M0) a total response was noted. The failure in advanced stages is discussed. In terato-, embryonal- and choriocarcinomas of the testis (T2-4 N0-2 M0-1) remarkable responses of short and long duration were observed in all patients. The reasons for different biological action are discussed.

[New aspects in the treatment of oxalate lithiasis (author's transl)].

The causes of hypercalciuria and simple diagnostic criteria for the various forms of hypercalciuria are outlined. Indications, effectiveness, limitations, and side effects of cellulose phosphate are described. Emphasis is placed on the biochemical pathogenesis and classification of hyperoxaluria. The problems of measuring and controlling oxalate excretion in patients with hyperoxaluria and calcium oxalate stones are discussed. Succinimide offers a partly successful approach to the reduction of endogenous oxalate synthesis.

Regeneration of the renal calyces after calycectomy: an experimental study.

On 14 rabbits, 21 kidneys were subjected to a calycectomy or the tip of the papilla was cut off by caudal nephrotomy. During an observation period of 2, 4, 6, 8 and 10 days, and further after 3 and 6 weeks the macroscopic and microscopic investigations led to the following results clearly answering the question put at the beginning of the inquiry: (1) even if parts of the renal calyces are removed, a complete regeneration of the renal pelvic mucosa takes place in the damaged area; (2) regeneration of the mucosa begins in neighbouring renal pelvic epithelium and the epithelium of the collecting tubes; (3) regeneration starts on the 4th day and is essentially ended by the 3rd week; this regeneration extends over the whole of the injured area; after the 6th week the process is finished and no inflammatory changes are to be found in the damaged area, and (4) in the process of healing no fibrous tissue is formed at the occlusion of the lamina cribrosa in the area of the papilla; there is no demonstrable necrosis of the renal parenchyma where the mucosa was removed. Hydronephrosis in this area is evident, beginning on the 3rd or 4th day and is to be regarded as the result of inflammatory process or of the operation itself. It is partly in evidence after 6 weeks but is clearly in a regressive stage. Morphologically a functional loss of the affected area of renal tissue cannot be demonstrated. The four questions made and answered by this animal experiment allow the following conclusion: calycectomy can be successfully undertaken. Regeneration of the mucosa is almost finished after 3 weeks and finally completed after 6 weeks. Considerable cicatrisation in the area of the mucous membrane injury or of its nephrons is morphologically unimportant. Calycectomy should be preferred to polar resection as there is a smaller parenchymal loss.

Reconstruction of the ureter: a review of operative and functional aspects.

The importance of the urodynamic characteristics of the ureter are considered after plastic and reconstructive operations. Choice of the most suitable method is as important as good operative technique. The partial replacement of the lower end of the ureter by a bladder flap shows good results. Different types of bladder flap procedures do not influence the dynamics of the ureter as its continuity is preserved. Irrespective of the material used, failures of autoplastic or alloplastic segmental replacement are due to the lack of functional adaptation of the prostheses. By partial replacement good results may be obtained if vesico-ureteral reflux is avoided.
