Psychological factors involved in maxillofacial prosthetics.

1. No significant difference existed between the MMPI means of the test group and the MMPI means of the Mayo Clinic patient group. 2. The means of the test group and the Mayo Clinic patient group deviated from the MMPI normative data. 3. No apparent change in personality of the test group was suggested by the MMPI. 4. A much larger percentage of the patients than expected failed to complete the initial MMPI. One patient refused, and four patients had a raw score of "cannot say" responses greater than 120, thus invalidating the test. 5. The changes in the means of the ACL scales of total number checked, autonomy, change, and abasement were significant (p less than .05). 6. The patient became more outgoing and optimistic, as measured by the ACL, during the immediate postoperative period. 7. Speech and mastication were problems for some patients even after prosthodontic rehabilitation. 8. Most of the rehabilitated patients resumed their normal social habits. 9. The patients in the study seemed to respond to the new situation according to their already existing personality structure.

Adolescent

Results of behavior modification in the treatment of chronic pain.

Two hundred patients with chronic pain were treated in an in-patient program in which behavior modification was the major therapeutic modality. The patients were characterized by having had pain of many years' duration, multiple operations, treatment failures, prolonged disability, compensation factors, and dependency on medication. At hospital dismissal, 59% of the patients had achieved moderate improvement or better. At a 3-mo follow-up, 40% of the admitted patients (75% of those successfully treated) were still doing well, and after 1 yr, 25% of those originally admitted continued to do well (65% of those successfully treated).

Carotid endarterectomy in a community hospital.

One hundred consecutive patients underwent carotid endarterectomy in a small community hospital in North Dakota during a period between 1969 through 1976. All operative procedures were performed by the same surgeon using the Javid shunt. A 1 per cent operative mortality and permanent morbidity resulted. These findings are comparable with those reported from large medical centers having extensive experience in carotid endarterectomy. It is concluded that patients with carotid artery disease can be safely and effectively treated in small medical centers by well trained experienced vascular surgeons.

Is there a characteristic personality profile in amyotrophic lateral sclerosis? A Minnesota Multiphasic Personality Inventory study.

A comparison of the Minnesota Multiphasic Personality Inventory profiles of 21 men and 17 women who had amyotrophic lateral sclerosis (ALS) with the profiles of 50,500 general medical patients showed no marked deviations. The validating scales of the men and the women were similar to those of the general medical population. There was no evidence of increased defensiveness in the group with ALS, as measured by either the L or the K scale, when compared with the general medical population. Individual patient interviews also showed no characteristic personality profile for ALS patients.

Intellectual assessment in a Midwestern alcoholism treatment population.

A study of 867 patients at a midwestern alcoholism treatment center which used Shipley-Hartford Institute of Living Scale scores and WAIS scores indicates that intellectual differences in this population are due to age and socioeconomic differences than to alcoholism. However, these real differences can be used to plan dismissal and rehabilitation policies for individual patients.

Muscle tension and personality.

Studied the relationship between skeletal muscle activity and personality variables as measured by the MMPI in 17 male and 51 female patients (most with chronic pain) in a large general hospital. After administration of the MMPI, muscle activity was measured from the frontalis. A biserial correlation coefficient was calculated between muscle tension and each of 12 MMPI scales. The high-tension group obtained a significantly higher score on scale L and a significantly lower score on scale PD.

Ovarian arteriovenous fistula. An unusual cause of abdominal pain.

Arteriovenous fistulae of the ovarian artery may develop after gynecologic surgery. A case that occurred 9 years after hysterectomy with bilateral oophorectomy is presented and discussed. It illustrates an unusual cause of left lower quadrant abdominal pain. Surgical removal of the ovarian arteriovenous fistula resulted in complete relief of the patient's symptoms. This uncommon entity must be considered when dealing with lower quadrant pain and a past history of hysterectomy.

Low back pain patients in a psychiatric population.

Thirty-one patients with low back pain resistant to medical and surgical treatment were studied. On admission to the psychiatric service, these patients revealed apparent features of conversion and hysterical personality, as characterized also by the MMPI profile. During hospitalization, however, there was increased appearance and recognition of anxious-depressive features. We suggest that these patients should be treated by the combined approach of supportive psychotherapy and physiotherapy, with appropriate use of antidepressants.

Program for managing chronic pain. II. Short-term results.

A program for treating inpatients who had chronic pain resulted in significant improvement in 79% of them by the time of their dismissal. At the time of short-term follow-up (6 months), however, this improvement rate had diminished to 50%. It was observed that this type of chronically disabled person can return home and increase work-related activity without returning to previous patterns of drug use and repeated hospitalizations.

Spouse response to a self-administered alcoholism screening test.

A self-administered, expanded version of the Michigan Alcoholism Screening Test was completed by a group of hospitalized alcoholics and by their spouses and counselors, describing the patients. The data obtained from spouse responses was accurate in 90% of the cases; the spouse may be a more reliable source of information about drinking problems than the alcoholic.
