Developments of the past decade have greatly improved the likelihood that patients can control their asthma. Inhaled medications are basic to a regimen that may include bronchodilators only, or bronchodilators along with cromoglycate and steroid to the extent required to achieve and maintain control. The regimen is modified for the individual and designed to control symptoms while avoiding an overdose of any one agent and overuse of inhaled bronchodilators (a sign of their lessening effectiveness). The regimen outlined emphasizes controlling asthma day to day and providing effective intervention early to prevent attacks from becoming severe.

The training and utilization of New Health Practitioners (NHP's) has moved within a decade from a small tentative beginning in the mid-1960's to a major health manpower policy issue. Its discussion transcends a number of important and emotionally charged areas including the role of various professions in providing health care services, the renaissance of primary care, the geographic and economic maldistribution of medical services, and the cost of care. It is valuable to highlight what is known, what is professed and what needs to be studied further about this issue.
Three classes of Medex were followed during their preceptorship training and subsequent employment to assess the impact of adding such physician assistants on 13 private practices. The data suggest that the Medex are readily incorporated into the practice, seeing a similar spectrum of patients and generally functioning as a semicolleague. Although the Medex was almost universally a contributing addition to the practice, increasing patient volume, the specific effects varied with the setting.

The MEDEX approach to the training and deployment of physician extenders is described and contrasted with other physician-extender training models, and results of a survey of 277 MEDEX graduates and 207 trainees are presented. Practically all MEDEX graduates and trainees are employed in full-time practice and are working with fee-for-service family physicians in the rural areas of the United States, which suggests that the strategy used to place Medex practitioners into communities requiring additional health services has been successful.
Programs designed for the control of asthma are complex. Implementation of a number of modalities may be necessary, and the mean and level of treatment may need to be varied from time to time. Medical treatment is a primary concern but preventive measures and education of the family are essential as well.
