Studies on the presence of magnesium in visceral amyloid.

The Magneson and Titan yellow tests were used to detect Mg in amyloid deposits in primary and secondary amyloidosis, in a pituitary tumour and in a case of equine cutaneous amyloidosis. Especially vascular amyloid deposits turned out to contain high levels of Mg. The significance of these findings remains unclear at present. Competition between Mg and Ca, the relationship between Mg in elastic fibres and amyloid P-component, and the high water content of amyloid along with the interaction of Mg with water are discussed.

[Low malignancy MALT lymphoma of the stomach: H. pyloric eradication as a therapeutic concept?].

A 63 year old patient came to admission because of abdominal pain. A stone disease of the gallbladder was known. Gastroscopy showed active gastritis of the antral-mucosa with some erosive lesions. Histology revealed surprisingly a low grade MALT lymphoma. Helicobacter pylori colonization (H. pylori) was found in the mucosa. While additional lymphomas were not found, a therapy aiming at the eradication of H. pylori was started. A subsequent control biopsy showed only slight lymphoplasmacellular inflammatory infiltration of the mucosa but no evidence of lymphoma. Our case report shows, that regression of a gastric MALT lymphoma can be achieved by eradication of H. pylori only. So far we don't know for how long this regression will continue and if permanent healing is possible. Further studies will have to show whether eradication of H. pylori can be established as a new therapeutic concept of low-grade MALT lymphomas at early stages.

[Pneumatosis intestinalis: a rare and difficult problem].

We present a case of a 74-year-old-woman who developed 3 episodes of intestinal pneumatosis, 2 times complicated by pneumoperitoneum, with consequent lethal outcome after 4 months. Intestinal pneumatosis, according to the literature, is a difficult and rare problem, associated with a large number of current diseases, can be complicated by pneumoperitoneum, mostly without clear etiology, whose treatment remains empirical (antibiotics, O2-therapy), not excluded laparotomy, if the clinical situation requires it.

[Mesenteric infarct in acute mesenteric vein thrombosis].

Mesenteric venous thrombosis is an infrequent but distinct form of intestinal ischemia. We report 2 cases of acute mesenteric venous thrombosis, treated successfully by carrying out intestinal resection and postoperative anti-coagulation (Heparin, Sintrom). Both patients were diagnosed in the operating room. Improvements in computed tomography may identify patients earlier in their clinical course, heparin has both a primary therapeutic role in early disease and a postoperative adjunctive role in advanced disease. With such care, these patients can expect an acceptable prognosis.

[The bipolar needle electrode in surgical laparoscopy. Initial technical experimental and clinical results].

To date the tremendous spread of operative laparoscopic procedures associated with monopolar high-frequency techniques has lead to an increase in complications caused by monopolar leakage currents. Experimental-surgical measurements on minipigs have shown no significant leakage currents in clinical relevant power range during laparoscopic application of a bipolar needle electrode as compared to monopolar electrodes. Practicability and safety of the bipolar electrode was proven in 100 operative laparoscopies for organ preserving or reconstructive surgery. In summary, with the new laparoscopic bipolar needle electrode a secure surgical instrument is available, with which finest tissue handling and minimal thermal damage feasible.

[Status of portal perfusion in colorectal cancer. Swiss Study Group for Clinical Cancer Research].

533 patients with diagnosis of operable colorectal carcinoma were randomized to receive either a single course of portal infusion with Mitomycin-C (MMC) and 5-Fluorouracil (5-FU) starting immediately after operation, or no adjuvant treatment. Of these, 505 (94%) were evaluable. Over the median follow-up of 8 years, the adjuvant therapy reduced the risk of recurrence by 22% (Hazard ratio = 0.78%, 95% CI 0.61-0.99; P = 0.045). The relative reduction of relapse on death was similar in all subgroups (i.e. nodal status, localization). However, adjuvant portal chemotherapy proved to be most efficient in the subgroups of patients with tumor involvement of the regional lymph nodes (Dukes C) and of patients with colon cancer. Analysis of the pattern of relapse showed that most of the difference in overall and disease-free survival is to be attributed to a consistent reduction of all kinds of tumor recurrences (i.e. local relapses, liver metastases and/or other distant metastases) in the treated group, rather than to liver relapses alone. We conclude therefore, that part of significant benefit obtained for patients with operable colorectal carcinoma treated with a single course of adjuvant chemotherapy via the portal vein might be due to the additional systemic effects of the portal chemotherapy and further study of perioperative treatment with and without prolonged chemotherapy appears worthwhile.

[Menisci and knee stability].

The menisci are of primary importance for the knee and its stability. They assure functional congruence guided by the crucial ligaments. They break up the greatest amount of contact-impact force and limit "elastically" rotation and translation movements. They are polyfunctional. That is the reason why it is so difficult to measure their physiologic behavior and mechanical effects. The medial and lateral compartments, and therefore the medial and lateral meniscus function and anatomy, are not symmetric but are quite different. The menisci are important for "fine tuning" in the control of normal motion and its limits. By achieving functional congruity they create stability ex vacuo, as in the shoulder joint.

[Diagnosis of lung function in intensive care of newborn infants].

We report our initial experience with pulmonary function testing in neonates during intensive care, using an automated, computerized system (PEDS), which allows non-invasive and rapid determination of pulmonary mechanics and energetics such as compliance, resistance and respiratory drive, and measurement of lung volume (functional residual capacity). This method is used for better physiological characterization and quantification of the respiratory status, to assess the effectiveness of mechanical ventilation and pharmacological therapy, and to guide weaning and extubation. Despite certain limitations with regard to direct evaluation of the respiratory status especially in very sick neonates, pulmonary function testing has contributed to a better understanding of the pathophysiological mechanisms of pulmonary dysfunction and can also be useful to optimize clinical management of neonates in intensive care.

Attempts to mimic docking processes of the immune system: recognition-induced formation of protein multilayers.

The assemblage of protein multilayers induced by molecular recognition, as seen, for example, in the immune cascade, has been mimicked by using streptavidin as a docking matrix. For these experiments, this protein matrix was organized on liposomes, monolayers at the air-water interface, and self-assembled layers on gold, all three containing biotin lipids. The docking of streptavidin to biotin at liposomal surfaces was confirmed by circular dichroism. Mixed double and triple layers of streptavidin, concanavalin A, antibody Fab fragments, and hormones are prepared at the air-water interface and on gold surfaces and were characterized by fluorescence microscopy and plasmon spectroscopy. With the use of biotin analogs that have lower binding constants it has been possible to achieve multiple formation and competitive replacement of the oriented protein assemblages.

Interleukin-10-deficient mice develop chronic enterocolitis.

Interleukin-10 (IL-10) affects the growth and differentiation of many hemopoietic cells in vitro; in particular, it is a potent suppressor of macrophage and T cell functions. In IL-10-deficient mice, generated by gene targeting, lymphocyte development and antibody responses are normal, but most animals are growth retarded and anemic and suffer from chronic enterocolitis. Alterations in intestine include extensive mucosal hyperplasia, inflammatory reactions, and aberrant expression of major histocompatibility complex class II molecules on epithelia. In contrast, mutants kept under specific pathogen-free conditions develop only a local inflammation limited to the proximal colon. These results indicate that the bowel inflammation in the mutants originates from uncontrolled immune responses stimulated by enteric antigens and that IL-10 is an essential immunoregulator in the intestinal tract.

Anemia↗

[Castleman's disease].

A 17-year-old girl developed acute colicky periumbilical pain with haematuria. On examination a tightly elastic space-occupying lesion about 10 cm in diameter was palpable around the umbilicus. Erythrocyte sedimentation rate (ESR) was raised to 113 mm in the first hour. There was a microcytic anaemia (8.7 g/dl) and a positive Coombs test. Ultrasonography revealed a periumbilical space-occupying lesion of decreased echogenicity and left hydronephrosis. Computed tomography demonstrated a homogeneous noninfiltrative tumour, about 7 cm in diameter, with increased contrast medium concentration. The tumour, which was covered by peritoneum and well circumscribed, was excised from the base of the mesentery. Histologically it proved to have hyaline-sclerotic changes in the blood vessels and lymphoid and plasma cellular infiltrations, corresponding to a mixed form of Castleman's disease. In a second operation a left nephroureterectomy was performed. The kidney, the regional lymph nodes and the tissue which had caused the stenosis of the ureter all showed the same changes as the tumour. Postoperatively the ESR and the blood count became normal, but the Coombs test remained positive. During a subsequent pregnancy the ESR again rose. The patient has remained symptom-free for 38 months after the second operation.

[Serotonin concentration in serum of patients with generalized tendomyopathy (fibromyalgia) and chronic polyarthritis].

The serum concentration of serotonin (S-5-HT) was measured in 31 patients with primary fibromyalgia, 21 patients with rheumatoid arthritis (RA) (15 of them with secondary fibromyalgia) and 20 healthy volunteers. Both patients with primary fibromyalgia and rheumatoid arthritis had significantly lower S-5-HT levels when compared to healthy controls, and S-5-HT concentrations in patients with secondary fibromyalgia were even significantly lower than those of RA-patients. Unlike the patients with rheumatoid arthritis, a significant correlation between S-5-HT level and the number of "tender points" as well as mean pressure tenderness at 24 different points was found in patients with primary fibromyalgia. Conversely, in patients with rheumatoid arthritis the S-5-HT level correlated significantly with erythrocyte sedimentation rate. These results suggest different pathological mechanisms of S-5-HT decrease in patients with primary fibromyalgia and rheumatoid arthritis. On the other hand, they raise the question whether secondary fibromyalgia may be a pathogenetically different syndrome mimicking symptomatically primary fibromyalgia.

Immunoglobulin heavy and light chain genes rearrange independently at early stages of B cell development.

The compartment of mouse B cell progenitors can be resolved into five developmentally related fractions by multicolor flow cytometry. Using this system and employing mutant mice in which the membrane exon of the mu chain, the lambda 5 gene, or the JH locus was inactivated by gene targeting, we found that expression of the pre-B cell receptor complex is necessary for the transition from the large CD43+ to the small CD43- pre-B cell stage. We report the occurrence of immunoglobulin heavy and light chain gene rearrangement at the stage of large B cell precursors. We show that neither the pre-B cell receptor complex nor any gene rearrangement in the heavy chain locus is required for the induction of kappa light chain gene rearrangement in early B cell progenitors.
