[Arterio-coronary venous difference in lactate concentration--a new objective and topical available parameter for the estimation of the energy metabolism status of the heart].

Cardiosurgical operations were tested experimentally by means of a perioperative metabolic-energetic monitoring and by the conventional method of the hemodynamic monitoring (Swan-Ganz-catheter). By that could be shown that a perioperative monitoring by means of the arterio-coronary venous difference of the lactate level is possible. Intraoperative information about the topical metabolic-energetic condition of the myocardium is to receive as the decisive advantage of this method. An improvement of the therapy results is possible in that way by an optimum of the treatment. A metabolic recovery of the heart during the operation was substantial more favourable for the postoperative myocardial restitution than an early application of catecholamins. This method for perioperative observation should be used specially in risk operations (NYHA-stadium III-IV, reoperations, etc.).

[Procedures and results with preoperative individual AO reconstruction plates].

As a rule continuous bony defects of the mandible after tumour resection are temporarily bridged with the AO reconstruction plate. To precontour the plate, CT-scans of the mandible at a scale of 1:1 6 mm above and parallel to the inferior margin are needed. Corresponding layers further cranial show the anatomy of the chin. Using a lateral cephalograph, the mandibular angle and the length of the plate in the ascending ramus are measured. Using the CT-scans, the lateral cephalograph, a prebent template and a corresponding model of the mandible, the plate is bent into shape. Of the nineteen preshaped reconstruction plates implanted, eleven did not need any intraoperative adjustment. The other eight plates needed only slight modifications of contour. Preoperative bending of the reconstruction plate reduces the time taken for the operation, protects the material and the plate is precisely shaped.

A mortality study of a cohort of slate quarry workers in the German Democratic Republic.

Following reports from other countries indicating an excess risk of lung cancer among silicotics, a historical cohort of workers employed for at least one year at a company in charge of slate extraction and processing during the period 1953-1985 in the German Democratic Republic has been constructed and followed up for mortality from 1970 to 1985. The results of the study show a mortality excess for infectious and respiratory diseases. The overall lung cancer mortality is not in excess but shows a tendency to increase with time since first exposure. A mortality excess from lung cancer is concentrated among workers receiving compensation for silicosis, suggesting a possible carcinogenic risk for individuals suffering from this pathological condition.

Differential antagonism by 1,3-dipropylxanthine-8-cyclopentylxanthine and 9-chloro-2-(2-furanyl)-5,6-dihydro-1,2,4-triazolo(1,5-c)quinazolin-5-im ine of the effects of adenosine derivatives in the presence of isoprenaline on contractile response and cyclic AMP content in cardiomyocytes. Evidence for the coexistence of A1- and A2-adenosine receptors on cardiomyocytes.

The antagonism by the A1-adenosine receptor antagonist 1,3-dipropyl-8-cyclopentylxanthine (DPCPX) and the A2-adenosine receptor antagonist [9-chloro-2-(2-furanyl)-5,6-dihydro-1,2,4-triazolo (1,5-c)quinazolin-5-imine] (CGS 15943A) of the effects of the A1-adenosine receptor agonist (-)-N6-phenylisopropyladenosine (R-PIA) and the A2-adenosine receptor agonist 5'-N-ethyl-carboxamideadenosine (NECA) in the presence of isoprenaline on contractile response and cyclic AMP (cAMP) content in cardiomyocytes from guinea pig cardiac ventricles were studied. In electrically driven (1 Hz) guinea pig ventricular cardiomyocytes R-PIA concentration-dependently (0.0001-100 microM) reduced the stimulatory effects of isoprenaline (0.01 microM) on contractile response and on cAMP content. The A1-adenosine receptor antagonist DPCPX (0.3 microM) antagonized the effects of R-PIA on contractile reponse and on cAMP content, whereas the A2-adenosine receptor antagonist CGS 15943A (0.01 microM) was ineffective. NECA (0.0001-100 microM) reduced the effects of isoprenaline (0.01 microM) on contractile response to about the same extent as R-PIA. However, NECA did not change cAMP content. DPCPX (0.3 microM) antagonized the effects of NECA on contractile response and evoked a cAMP-increasing effect of NECA, which was 38% of the isoprenaline value at most. In contrast, CGS 15943A did not affect the reduction of contractile response induced by NECA, whereas CGS 15943A revealed a cAMP-decreasing effect of NECA (0.1-10 microM). This study provides functional evidence that both, cAMP-decreasing A1- and cAMP-increasing A2-adenosine receptors are present on ventricular cardiomyocytes.(ABSTRACT TRUNCATED AT 250 WORDS)

[Fundamental research in ultrasonic surgery. IX. Splenic surgery using cavitation ultrasound--an experimental animal study of aspiration and sealing technics with ligament FIMOMED].

A farther alternative for the conservation of the spleen represents the ultrasonic aspiration and sealing technique by means of the monomeric tissue adhesive Ligament Fimomed. The effectiveness of the method was proved by histomorphologic investigations in animal experiment with the porcine spleen. The method was effective in nearly all injuries across to the longitudinal axle of the organ even with a larger parenchymal defect and also in case of splenic tumor. The control of hemorrhage succeeded promptly and surely. Not any postoperative complication appeared.

[Heart surgery interventions in risk patients. New possibilities for improving therapeutic results].

Perioperative therapy in conjunction with cardiac surgery had so far been guided by functional, paraclinical, and clinical parameters which only conditionally enabled objective appraisal of the present condition of the myocardium. Perioperative recording of the arterio-coronary-venous concentration difference of lactate (acDL) has now enabled the use of an objectivated parameter for accurate description of the metabolic-energetic situation of the myocardium. This will be helpful in metabolic-energetic condition monitoring (MESM) to optimise perioperative cardiotonic therapy. Stabilisation of the metabolic-energetic situation of the myocardium, objectivated by MESM, has proved to be superior to premature cardiac support, using catecholamines. It has been associated with improved restitution with high significance (p less than 0.005).

[Bronchopulmonary diseases in primary immune deficiencies].

Data of 124 patients with primary immunodeficiencies were examined in a retrospective study in respect of incidence and severity of bronchopulmonary immunodeficiencies. Relapsing pneumonias were seen in 40 patients (32.3%) and bronchiectases in 15 cases (12.1%). Children with severe antibody deficiencies or combined immunodeficiencies were involved particularly often. Avoidance of pulmonary complications is of special prognostic importance with these patients.

[The use of disposable draping materials in the operating room and its effect on the postoperative wound infection rate].

In a consecutive study we analyzed the influence of gown and drape material on the postoperative wound infection rate. 1033 operations were included in this study. In 354 operations conventional cotton gowns and drapes were used, while in 679 operations, a disposable gown and drape system was utilized. The use of disposable gown and drape material reduced the postoperative wound infection rate in our surgical department. The greatest reduction -43% - was seen in clean wounds. Within this group gallbladder operations showed a reduction of 59% and hernia operations showed a reduction of 46%. A slight reduction was also seen in clean-contaminated and contaminated wounds, while in dirty wounds there was no difference. However, due to the relatively low numbers involved the results were not of statistical significance. An analysis of the costs over a 12-month period showed a reduction of 7.5% when the disposable gown and drape system was used. With respect to ease of handling and wear the disposable material was preferred by the operating-staff, both doctors and nurses.

[Methods for determining antinuclear antibodies and their clinical significance].

Antinuclear antibodies are directed against different nuclear antigens such as deoxyribonucleic acid, ribonucleic acid, nucleoproteins, histones and non-histones-antigens. The importance of ANAs in the characterization of different systemic rheumatic diseases, e.g. autoimmune diseases, has been recognized. The most commonly used assay for ANAs is indirect immunofluorescence. Several methods, such as radioimmunoassays, double immunodiffusion in agarose gel, counterimmunoelectrophoresis or immunoblotting, have been developed and serve to identify antibodies of different specificities. This overview describes the clinical importance of ANAs with reference to the methods of detection. The immunologic diagnosis aspect of different autoimmune and collagen diseases is presented.

Evaluation of advanced silica packings for the separation of biopolymers by high-performance liquid chromatography. VI. Design, chromatographic performance and application of non-porous silica-based anion exchangers.

The linear solvent strength model of Snyder was applied to describe fast protein separations on 2.1-micron non-porous, silica-based strong anion exchangers. It was demonstrated on short columns packed with these anion exchangers that (i) a substantially higher resolution of proteins and nucleotides was obtained at gradient times of less than 5 min than on porous anion exchangers; (ii) the low external surface area of the non-porous anion exchanger is not a critical parameter in analytical separations and (iii) microgram-amounts of enzymes of high purity and full biological activity were isolated.

Carbachol reduces IK,baclofen, but not IK,GABA in guinea pig hippocampal slices.

In the presence of bicuculline and/or picrotoxin (-)-baclofen and gamma-aminobutyric acid (GABA) induced outward currents (IBac and IGABA) at holding potentials of -55 to -75 mV in guinea pig CA3 neurones of hippocampal slices. Zero potentials for these currents were at the K-equilibrium potential indicating that they were carried by K-ions (IK.Bac, IK.GABA). IK.Bac was strongly reduced by carbachol (Cch) in low concentration (0.1 0.3 microM), while IK.GABA was not affected by Cch concentrations even up to 20 microM. The K-dependent late inhibitory postsynaptic potential (IPSP) was reduced significantly by Cch concentrations higher than 1 microM, but with these concentrations the early, Cl-dependent IPSP was reduced as well. The baclofen-derivative phaclofen, considered a selective antagonist of both the postsynaptic action of baclofen and the bicuculline - and picrotoxin - resistant action of GABA, exhibited, in our hands, partial agonistic effects and effects on non-transmitter gated K-currents. Our findings cast some doubt on the assumption that IK.Bac, IK.GABA and the late IPSP are all mediated by the same receptor and generated by the same mechanism.

Lithium discriminates between muscarinic receptor subtypes on guinea pig hippocampal neurons in vitro.

In CA3 pyramidal neurons of guinea pig hippocampal slices an outward current activated by the GABAB agonist, baclofen (0.3 microM, Ibac) was reduced by low concentrations of carbachol (Cch, 0.1-0.3 microM). The effect of Cch desensitized suggesting that the receptor subtype involved in this muscarinic effect of Cch was of the M1 subtype. The receptor subtype was also characterized by its equilibrium dissociation constant for pirenzepine (10 nM) as an M1 receptor. Li+ applied extracellularly (1 mM) or intracellularly blocked the suppression of Ibac by Cch without affecting the Cch blockade of a current termed IAHP, which is mediated by M2 receptors. While the effect of intracellular Li+ application was immediate, it developed very slowly with extracellular application. Since Li+-salts are used effectively in the treatment of mania and depression, the selective effect of Li+ on M1-mediated muscarinic neurotransmission might be important for the cholinergic hypothesis of affective disorders.
