Fate of late complications in type I diabetic patients after successful pancreas-kidney transplantation.

The success rate of pancreas transplantation allows us to study in more detail the potential beneficial effects of normoglycemia on secondary complications in diabetes mellitus. We report a prospective follow-up (mean 26 mo) of metabolic control, neuropathy, retinopathy, and peripheral microcirculation in 31 patients with type I (insulin-dependent) diabetes (mean age 33 +/- 1 yr; mean duration of diabetes 21 +/- 1 yr) after combined kidney and segmental pancreas grafting. All patients had normal HbA1 levels. Glucose tolerance (GT), insulin, C-peptide, and glucagon were normal in 22 patients, and impaired oral GT with reduced insulin secretory capacity was seen in 9 patients. During follow-up, there was no deterioration of GT and insulin release. Vascular risk factors, e.g., hypertension, cholesterol, and triglycerides, decreased after grafting. Autonomic neuropathy improved clinically, and R-R variation increased significantly in 3 of 18 patients. Peripheral neuropathy improved clinically in 46% of patients and did not deteriorate in the others. Motor nerve conduction velocity increased greater than 20% in 8, less than 20% in 12, and was unchanged in 8 of 28 recipients. Most of the patients (n = 30) had pretransplant laser treatment of their advanced retinopathy. Posttransplant visual acuity improved at least more than one line in 56%, stabilized in 32%, and deteriorated in 12% of patients. Patients with functioning grafts for greater than 1 yr had no further deterioration of visual acuity. Vitreous hemorrhage frequency and severity dropped markedly from pretransplant (from 69 to 24%) 10 mo after grafting. Retinal morphology remained stable in all eyes except two.(ABSTRACT TRUNCATED AT 250 WORDS)

Quantitative immunohistochemistry: standardization and possible application in research and surgical pathology.

We describe three different methods of calibration of quantitative immunohistochemistry, two of them based on self-made dye-agarose or antigen-agarose substrates. Quantification of immunostained objects (immunosignals) can be done by (1) microscope photometry (plug method) or scanning-photometry; (2) and densitometry of a television transformed microscope image. Both approaches are subjected to the different methods of calibration and three examples of application.

[Initial results of adjuvant portal liver infusion following radical surgery of colorectal cancer (Swiss Study Group for Epidemiologic and Clinical Cancer Research Study 40/81)].

Between July 1981 and June 1987, 533 patients from 7 participating institutions have been entered in a prospective randomized trial to assess the value of adjuvant portal infusion (5-Fluorouracil 500 mg/m2/d x 7 continuous infusion + Mitomycin-C 10 mg/m2 on day 1 as a bolus injection through portal venous catheter) compared to radical surgery alone. The portal venous catheter was placed through any side-branch of the mesenteric venous system during laparotomy for the primary tumour. Using the transabdominal route, there have been no catheter-related complications. Overall hospital mortality in the study was 1.75% and was not influenced by adjuvant treatment. Analysis of 469 eligible patients at a median follow-up of 48 months revealed 39.1% recurrencies in the control group and 31.8% in the infusion group (p = 0.09, logrank). Median survival of control patients is 72 months, of chemotherapy treated patients not yet reached. Significant survival advantages have been detected for those 195 (85%) patients who received full-dose adjuvant chemotherapy (67% versus 53% 5-year survival). Due to the low number of deaths in this trial, prolonged follow-up is needed for definitive survival conclusions.

[Measurement of keratan sulfate levels in synovial fluid in joint diseases].

The authors describe their own modification of the ELISA method for the assessment of the epitope of keratate sulphate (KS) in synovial fluid. Considerably elevated values of KS were found in patients with gouty arthritis and reactive arthritis, as compared with patients with rheumatoid arthritis. The KS levels in the exudate of patients with osteoarthritis were significantly higher than in patients with rheumatoid arthritis. In patients with rheumatoid arthritis a negative relationship was found between the KS level in the exudate and the activity of the disease. A relationship was found between KS in the exudate and the interleukin-1 level in the exudate. Finally the authors discuss the importance of assessment of cartilage fragments in body fluids by immunochemical methods and future development trends of this discipline.

[A new multi-channel LDR/MDR-afterloading-system (Inter-Pal-C38)].

The methods of temporary interstitial implantation of iridium-192 seed chains have been applied in Erlangen for two years in many cases of squamous cell carcinoma in the head and neck area, the cervix uteri or the anal duct. The afterloading procedure which at the beginning had to be performed manually involved partially considerable radiation-exposures of medical and nursing staff. Therefore we have developed a 38-canal machine for remote control afterloading with iridium-192 seed chains in LDR and MDR therapy which is working in a similar way as the already established methods of remote control afterloading in HDR therapy. Special technical features are the automatic afterloading procedure, the disconnecting of patient and machine for any length of time, the possibility to use the machine for several succeeding patients, and the modern computer control by an IBM PC with color monitor and alpha-numerical keyboard. The new machine is fast and easy to handle, protects the staff completely against radiation and, due to the possibility of disconnection and free mobility, makes it easier and more comfortable for the patient to stay within the radioprotection room.

[The effect of noise on the feto-placental unit (epidemiologic and experimental study)].

Epidemiologic investigations on pregnant working women with noise exposure in ear damaging range didn't astonish in regard to essential parameters like average birth weight, average duration of gravidity, hyperemesis gravidarum, abortion frequency and disablements in comparison with a control group. In an experimental study 13 pregnant women in the 18th to 21st, 28th to 31st and 34th to 37st week of pregnancy were tested against a load of 75, 85 and 95 dB(A) via earphones and by means of cardiotocogram the derivations of foetal cardiac actions were taken. During all observation phases, all derived parameters are signated within the physiological deviations considering the gestational age.

[Blood transfusions and prognosis following curative resection of colorectal cancer: is there an association?].

More recently a number of retrospective analyses in rather ill defined patient populations demonstrated an association between perioperative blood transfusion and recurrence after curative resection of colorectal cancer. In the randomized trial (SAKK 40/81) (adjuvant cytotoxic intraportal infusion versus no further treatment) we evaluated the transfusion status in a well defined, prospectively documented and controlled patient population. Of 457 patients, 353 (77.2%) received either pre-, intra- or postoperatively blood transfusions. After a median follow-up of 4 years, the transfused patients developed significantly more recurrences (38.2%) than patients without blood transfusions (23.1%), the death-rate being 33.7% versus 23.0%, respectively. Patients without transfusion but treated with adjuvant intraportal chemotherapy are strikingly doing better (10.5% recurrences) than patients with perioperative blood transfusion not having an adjuvant treatment (44.5% recurrences).

[Cardiac resuscitation using glucose-insulin--a case report].

A report is given on a 52-year-old female patient who showed catecholamine-refractory cardiac failure after double valve replacement. Maximum dosages of dopamine, dobutamine and epinephrine could not effect a sufficient left ventricular function. As "ultima ratio", the application of 1 g/kg b.w./min glucose and 1.5 IU/kg b.w./min insulin reversed the apparently hopeless situation. The decisive advantage of cardiac nutrition using glucose and insulin lies in an anabolic action with resynthesis of high energetic phosphates for improving the inotropic capacity of the heart.

[Work formation and experience in flexible automated production plants--analysis of an example from the VEB Combine Carl Zeiss JENA].

Considering the production range pressure casting of the Combine VEB Carl Zeiss JENA as an example it will be made clear that--due to the development of low--service flexible automatic solutions--not only a lot conventional industrial-hygienic exposures/loads could be reduced but it is made a personality--furthering activity. The effect of work on the psychic good health of workers, the experience of the activity in order to the connected work contentment and the subjective analysis of the activity by the workers themselves are examined.

[Studies on the long-term exposure of operating room personnel to 2-bromo-2-chloro-1,1,1-trifluorethane (Halothane)].

Different operation theatres of the county of Gera were examined to quantify the exposition against halothane which is used as an inhalation narcotic, and also against ethanol. Besides the short time exposition (0.5 h) determined with gas sampling tubes the long time exposition (up to 8 h) was monitored personally by means of charcoal passive samplers. The analytical method of choice was gas chromatography, in the second case after thermal desorption of the enriched pollutants. Generally, the concentrations are lower than the MAKD if the workplace is equipped with an exhaust type NAREX 101. Without it the MAKD is reached quickly. A statistical evaluation of the results shows that there exist remarkable differences between as well the degree as the scattering of the individual exposition levels depending from the task in the operating theatre. Obviously the percentage of anesthesia using halothane is increasing.

[Can the thesis of higher noise resistance in working women be maintained?].

Using the data of 2,000 medical opinions concerning hearing loss due to noise the thesis of a higher noise resistance of exposed working women is tested on the base of the hearing loss comparison of female and male individuals of a paired random sample. The results of the examination show that there is a weak not statistically significant difference of noise susceptibility depending on sex which is negligible small comparing it with the high interindividual differences of the noise susceptibility of men and women. This possibly existing very small difference of hearing loss unfavourable for noise exposed men doesn't authorize to consider women as more resistant against noise as men.
