[Bacterial flora of lower extremity ulcerations in patients with chronic sclerotic arterial obstruction].

Investigations were carried in 70 patients with ulceration of lower extremities during chronic aortal obstruction. The patients were subjected to angiography, ultrasonographic determinations of pressure and quantitative and qualitative bacteriological studies. Quantitative studies were carried on by taking a smear from one square centimeter of ulceration surface. The isolated microorganisms were cultured on standard media for bacteria growing in aerobic conditions. Cultures for detection of anaerobic bacteria were performed on thioglycolate media (qualitative study). It was found that clinical symptoms of infection occur when for 1 cm2 of ulceration surface more than 5 x 10(4) of bacterial cell are present. In all patients reconstructive procedures improving blood flow were performed. Comparison of consecutive bacteriological studies performed before and after surgery permitted for evaluation of dynamics of infections and enabled choice of proper treatment and evaluation of its effectiveness.

[Function of platelets in patients with occlusive atherosclerotic arterial disease of the lower extremities].

The platelet function as well as parameters of lipid metabolism and glucose tolerance were investigated in 30 men with occlusive atherosclerotic arterial disease of the low extremities (IIIB or IV Fontaine's stage). The platelet aggregation, platelet survival, activity of intraplatelet metabolism of arachidonic acid, radiofibrinogen binding to platelet, circulating platelet aggregates and both the activity of factor VIII and the concentration of von Willebrand factor antigen in the plasma were measured. In the majority of patients the impairment of platelet aggregation with ADP, enhancement of radiofibrinogen binding to platelets and an increase of factor VIII level in the plasma were established. There was an interrelationship between platelet dysfunction and disturbances of lipid metabolism. Platelet survival was shortened in patients with moderate hyperlipidemia and correlated with a concentration of HDL-cholesterol in the serum. The radiofibrinogen binding to platelets was the most pronounced in patients with severe hyperlipidemia and correlated with a concentration of total cholesterol in the serum. The results may suggest the potential usefulness of antiplatelet drugs in patients with occlusive atherosclerotic arterial disease.

C-reactive protein and alpha-1-acid glycoprotein in monitoring of patients with chronic arterial occlusion of the lower limbs.

C-reactive protein (CRP) and alpha-1-acid glycoprotein (AGP) levels were studied in the sera of 107 patients with chronic arterial ischaemia of the lower limbs. Depending on the degree of ischaemia, patients were divided into four groups: (i) intermittent claudication--20 patients; (ii) rest pain and ankle pressure above 50 mmHg--12 patients; (iii) rest pain and ankle pressure below 50 mmHg--25 patients; (iv) peripheral necrosis--50 patients. Reconstructive surgery was performed in 84 patients and primary major amputation in 23 cases. Levels of CRP and AGP increased with the severity of ischaemia. In uncomplicated cases, CRP and AGP concentrations decreased to normal values 7-10 days after surgery. In a group of 23 patients with postoperative necrotic complications, significant increases in CRP and AGP levels were found 7-10 days after surgery. These proteins, especially CRP, may be clinically useful in monitoring patients during the postoperative period with a CRP level greater than 50 mg/l predictive of complications with a sensitivity of 83% and a specificity of 94%.

Concentrations of calcium, magnesium, zinc and copper in relation to free fatty acids and cholesterol in serum of atherosclerotic men.

The concentrations of calcium, magnesium, zinc, copper, free fatty acids and total, HDL- and LDL-cholesterol in the sera of atherosclerotic men were determined. The subjects with femoral atherosclerosis were divided into two age groups: 35-59 years (N = 8, I) and 60-75 years (N = 8, II). The LDL-cholesterol concentration was increased in the elderly atherosclerotic group. Lower concentrations of calcium and magnesium, a higher concentration of copper and decreased Ca/Cu, Mg/Zn, Mg/Cu and Zn/Cu ratios were found in atherosclerosis I as compared to controls of the same age. High and positive correlations for Cu vs. total cholesterol and LDL-cholesterol (r > 0.89), but low correlation for Cu vs. HDL-cholesterol (r = 0.61) were calculated in atherosclerosis I, and negative correlations for free fatty acids vs. Ca and Mg (-r > 0.77) in atherosclerosis II.

C-reactive protein and alpha 1-acid glycoprotein in monitoring of patients with acute arterial occlusion.

C-reactive protein (CRP) and alpha 1-acid glycoprotein (AGP) levels were studied in sera of 75 patients with acute arterial occlusion. Depending on the degree of ischaemia, patients were divided into two groups: grade I--26 patients--and grade II--49 patients. All patients were treated surgically; 42 embolectomies, 17 endarterectomies and 16 bypasses were performed. After surgery in 19 patients various complications were observed. The concentration of both proteins at the time of admission was higher in the serum of patients with grade II than with grade I ischaemia. Similarly the concentration of both proteins was significantly higher in the sera of the patients admitted after 8 h than in patients admitted within 8 h of the onset of ischaemia. CRP and AGP levels were significantly higher in the serum of patients with ischaemia of the lower limb than in those with ischaemia of the upper limb. In all patients 2-3 days after surgery a significant increase in serum CRP and AGP was observed. In uncomplicated cases on days 7-10 the values of both proteins decreased below the level observed at the time of admission. However, in patients who experienced postoperative complications high levels of both serum proteins (especially CRP) were found on days 7-10. Complications were detected with a sensitivity and specificity of 84 and 95%, respectively, using a CRP level of 49 mg l-1 as the cut-off point.

Rapid miniaturized chromatography for Tc-99m IDA agents: comparison with gel chromatography.

Miniaturized chromatography systems for determining free pertechnetate and hydrolyzed reduced 99mTc levels in commercial 99mTc-labeled iminodiacetate (IDA) hepatobiliary radiopharmaceuticals were evaluated and the results compared with gel chromatography column scanning (GCS). Commercial IDA agents were evaluated including 2,6-dimethyl IDA, p-isopropyl IDA, p-butyl IDA, and 2,6-diisopropyl IDA. Of all the chromatography systems evaluated, only Gelman ITLC-SA with 20% NaCl and Gelman ITLC-SG with distilled water correlated with GCS in evaluating free pertechnetate and hydrolyzed reduced 99mTc levels for all IDA radiopharmaceuticals. The miniaturized chromatography procedure, as outlined, is rapid, taking less than 4 min, and can easily be incorporated into the daily quality control program in any nuclear medicine facility.

[Degradation of sodium carboxymethylcellulose in different media].

Aqueous solutions of CMCNa change the rheological properties in the presence of electrolytes. It was stated that the greater the ionic strength, the weaker the dynamic viscosity. The tixotropy of CMCNa solutions decreases with time and those changes are proportional to the ionic strength of the electrolyte. No significant changes of the IR spectra of CMCNa were observed, which means that changes of the rheological properties do not influence the chemical structure of CMCNa.
