Factors influencing survival following fractures of the upper end of the femur.

Patients undergoing treatment for fractures of the upper end of the femur in a district general hospital over a 2-year period were analysed to identify preoperative illness, operative factors or postoperative complications associated with increased mortality. Both the mean age and mortality rate were higher than in many other reported series. The elderly diabetic was identified as a particularly high-risk patient. Dislocation following hemiarthroplasty was associated with advanced age and carried a formidable mortality.

Pelvic osteotomy retractor.

A retractor which displaces soft tissue from bone in deep wounds and improves lighting and instrument manipulation at its tip is described. Its particular advantage in pelvic osteotomy is emphasised.

A free bone block operation for recurrent anterior dislocation of the shoulder joint.

An operation to prevent recurrent anterior dislocation of the shoulder by insertion of a free iliac bone graft into an anterior glenoid osteotomy is described and has been performed on 16 shoulders in 15 patients. No additional fixation of the graft was used and the divided subscapularis tendon was not sutured. The average follow-up was 7 years and 8 months, no serious complications occurred, there were no recurrences, there was no evidence of degenerative osteoarthrosis and only 4 patients had any detectable loss of power. Only 2 patients had noticed any restriction of movement. There was some loss of external rotation in 13 shoulders. All patients returned to their desired levels of work and sporting activity after operation. The operation is technically easy to perform and has comparably excellent results to other published series.

Prosthetic arthroplasty of the shoulder.

Twenty-six patients, 18 with rheumatoid arthritis, underwent arthroplasty of the shoulder between 1973 and 1980, with an average follow-up of 3 years. The Stanmore and Kessel arthroplasties and the Neer hemiarthroplasty were used. Twenty-one patients undergoing 22 elective arthroplasties considered that the operation was worthwhile. Pain was completely relieved in 12 shoulders. Movement was improved in 18 shoulders. The priority of restoring movements which improved independence and self-respect is emphasised; the results were far superior to those obtained by an arthrodesis.
