Images of the invisible-prospection methods for the documentation of threatened archaeological sites.

To understand the development of prehistoric cultural and economic activities, archaeologists try to obtain as much relevant information as possible. For this purpose, large numbers of similar sites must be identified, usually by non-destructive prospection methods such as aerial photography and geophysical prospection. Aerial archaeology is most effective in locating sites and the use of digital photogrammetry provides maps with high accuracy. For geophysical prospection mainly geomagnetic and geoelectrical methods or the ground-penetrating radar method are used. Near-surface measurements of the respective contrasts within physical properties of the archaeological structures and the surrounding material allows detailed mapping of the inner structures of the sites investigated. Applying specially developed wheeled instrumentation, high-resolution magnetic surveys can be carried out in a standard raster of 0.125 x 0.5 m covering up to 5 ha per day. Measurements of ground resistivity or radar surveys in a raster of 0.5 or 0.5 x 0.05 m, respectively, are used to gain information on archaeological structures and on the main stratigraphic sequence of sites covering up to 0.5 ha per day. Data on intensities of the Earth's magnetic field, apparent resistivities of the ground or amplitudinal information of radar reflections are processed using a digital image processing technique to visualize the otherwise invisible archaeological structures or monuments buried in the ground. Archaeological interpretation, in the sense of detecting, mapping and describing the archaeological structures, is done using GIS technology by combining all relevant prospection data. As most of the Middle European archaeological heritage is under a massive threat of destruction, dramatically accelerated by intensive agriculture or industrial transformation of the landscape, the prospection techniques presented here represent an approach towards an efficient documentation of the disappearing remains of our ancestors.

[Mental competence of dementia patients to execute a will].

Instruments allowing to give a psychiatric expertise on the ability of demented people to give a will are needed. Criteria for an estimation of the course of dementing illnesses and for an increased suggestibility in coming to a decision are critically reviewed.

[Willingness to give informed consent in elderly demented and confused (delirious) patients].

In the treatment of elderly patients the question often arises whether they are able to give an informed consent to medical operations. According to German jurisdiction the patient must be able to comprehend the kind, the aim and the consequences of the projected surgical operation as well as to declare his will. If this is not the case, he cannot give an informed consent, being "under disability" and in Germany a guardian has to be installed by the court. E.g., moderately demented or delirious patients are unable to give an informed consent to medical operations. A discussion on this topic is under way, as to whether a patient has the right to give an informed consent in advance in case of becoming unable to declare his will (in dementia etc.).

[Aseptic necrosis of the lunate bone. Etiology, clinical aspects, stage-adjusted therapy].

The development of the aseptic necrosis of the lunate bone is so far not fully understood. Therapeutical problems result from the very late discovery of the disease and from the central position in the carpal bone system. Bad long-term-results come out of therapies not able to revitalize the lunate bone. Therefore the therapeutical goal attempted must be to keep the structures of the bone alive and to avoid major pathological changes of the whole carpal bone system. More or less good results could be shown for these patients with a total break-down of the lunate bone treated with the intercarpal-arthrodesis described by Graner.

[Graner's intercarpal arthrodesis as therapy of aseptic lunate bone necrosis].

From 1978 till 1989 at the Department of Orthopaedics (University of Graz, Faculty of Medicine, Division of Surgery) 25 patients were operated for the reason of an avascular necrosis of the lunate bone when the bone structures were already destroyed. The form of therapy was the intercarpal-arthrodesis, developed by Orlando Graner 1966, in a slightly modified way. On the one hand through creating an intercarpal block it is possible to create more or less plain sides of the radiocarpal joint and on the other hand one can stop the carpal collapse what explains long-term-results rather acceptable. The most important disadvantage of this form of therapy is the often found loss of range of movement in the operated radiocarpal joint.

[Surgical therapy of malacia of the semilunar bone with a modified Graner method].

All operations for lunatomalacia are linked to considerable alteration and functional loss of the afflicted wrist joint. The risk of secondary arthrosis due to incongruity is very high, and not only because of the complicated biomechanics of the carpal bones. The authors present their experiences and results with seventeen patients treated for Kienböck's disease, of whom fourteen have been reviewed, by a modification of the operation described by Orlando Graner. On the basis of this evaluation and comparison to the results described by other authors, the method of Graner seems to be definitely suitable, even for younger patients. The disadvantage of the relatively obvious restriction in mobility is offset by a nearly complete freedom from pain, full strength, and only minor secondary arthrosis, even with long postoperative periods.

[Intercarpal pseudarthrosis as a complication of Graner's operation].

Since 1978 eighteen patients were treated for Kienböck's disease by the Graner procedure. The satisfactory long-term results can be explained by the preservation of the radiocarpal joint. By restoring fairly congruent joint surfaces, secondary radiocarpal joint arthrosis, which affects long-term results, may be avoided. Employing Graner's procedure, the authors observed two cases with pseudarthrosis within the intercarpal arthrodesis. They interpret this complication to be a result of micro-movements with the new bone block in the former midcarpal joint level. These movements may be avoided by careful removal of all the cartilage from all joint parts which are to be fused.

[Carpometacarpal joint arthrosis and its surgical therapy with the de la Caffinière endoprosthesis].

In the area of the hand the carpo-metacarpal joint of the thumb together with the metacarpo-phalangeal joints is the most important joint according to the function of the hand. Its reduction of movement leads to misfunction of the thumb and the whole hand. Osteoarthritis of that very special joint commonly occurs in females after the menopause. The increasing severeness of the arthrosis is often complicated through pathological positions as well as through contractures (so called Z-deformities). In these patients operative treatment for improvement of function is very often not avoidable. Especially in the elderly patient with reduced manual work the implantation of a cemented carpo-metacarpal prosthesis (created by de la Caffinière) seems to be a useful treatment. As in all cemented implants the disadvantage is the danger of aseptic loosening of the prosthesis.

[Information about medical procedures involving elderly patients].

Physicians are obliged to inform all patients about the diagnosis and course as well as the purpose and risks of diagnostic and therapeutic procedures. Difficulties in informing elderly, especially cognitively impaired patients are discussed. The legal aspects in Germany are shortly reviewed.
