[Dorsolateral approach to the left subphrenic area, to the bursa omentalis and to the cauda pancreatis].

We are reporting on a dorsolateral access left to the bursa omentalis and to the left subphrenium carried out in order to relieve infectious processes in this region. The access has proved a success above all in the later postoperative period, e.g. when the protecting conglutinations towards ventral isolate the process from the abdominal cavity. At the lying patient this access allows the drainage of the exsudation over the deepest point. The results of 23 surgical interventions are being demonstrated with references to possible complications caused by the intervention.

[Surgical indications in bronchopulmonary malformations in childhood].

Especially in childhood, the diagnosis of bronchopulmonary malformations should be carried out to such an extent as to enable an appraisal of the probable further development of symptoms. If increasing complications and secondary damage are impending, there should be no hesitation in having surgical treatment carried out by an experienced thorax surgeon even in early childhood, if possible before an absolutely life-threatening situation is present. Without exception, there were no complications in the postoperative courses, and the good development of symptom-free children with normalized lung function document the rational justification for thorax surgery in various bronchopulmonary malformations.

[Biocompatibility of various surgical drainage materials in the cytotoxicity and implantation test].

The biocompatibility of 10 materials used for surgical drainage was evaluated in a cytotoxicity test and in rat subcutaneous tissue implantation test. All rubber materials and silikolatex were found to be cytotoxic. There was no correspondence of the results of the cytotoxicity test with those of the implantation test. Therefore various procedures for biocompatibility-testing should be used.

[Does the elastic rapid wound dressing Ankerplast Spray modify wound healing?].

It had been tested if the elastic wound-dressing Ankerplast Spray reduces the risk of an infection of wounds, and if any influences could be observed on the formation of scars after spray-sealing of fresh surgical wounds. In a prospective, controlled study of 597 patients 567 could be postoperatively surveyed at all fixed terms of control up to the 56th day. The infection rate of wounds after general surgical operations amounted to a total of 7 per cent. The rate of hypertrophic scars within the control group was with 11 per cent respectively 7 per cent significantly below the rate of 34 per cent of hypertrophic scars within the Spray group. Are fresh surgical operation wounds being sealed with the elastic wound-dressing Ankerplast Spray no influence can be observed on the postoperative risk of wound infections. Hypertrophic scars must be taken in consideration in about 34 per cent.

[Mucosa protection--an assessable value? An experimental study in rats].

Reduced mucosaprotection is a main factor for development of peptic ulcer disease. The quantity is difficult to estimate. We did it by measuring the acid output as the aggressive factor and we sized the resulting mucosalesions. In the gastric corpus we could not find any change in mucosaprotectionindex after cimetidine, pirenzepine and vagotomy. But in the duodenum protection was increased after pirenzepine and vagotomy.

[Evaluation of reflux behavior of esophago-jejunal anastomoses following gastrectomy].

Functional results of 48 gastrectomies are reported. The applied techniques of the construction (jejunoplicatio) are judged for the safety of the anastomosis. 29 patients (60.5%) were examined for the degree of jejunooesophageal reflux after a survival time of 1 year. Antireflux techniques of anastomosis had good functional results measured by manometry and pH-metry.

Immunohistology of gastrointestinal carcinoid tumors.

DAKO quick staining kits were used to immunostainings for Epithelial Membrane Antigen (EMA) Neuron Specific Enolase (NSE) and S-100 protein (S-100) in 18 carcinoid tumors of the gastrointestinal tract classified according to Soga and Tazawa. EMA was always absent in carcinoid tumors, being at the same time present within glandular epithelium. 88% of cases showed positive immunostaining for NSE. S-100 immunostaining showed immunopositive stellate cells present within the tumor especially within type A carcinoids. In addition in one carcinoid tumor thick, strongly S-100 positive bundles were noticed at the periphery of nests of tumor cells. Combined immunohistochemical and ultrastructural studies are needed to elucidate interrelation of neoplastic and neural elements within carcinoid tumorsa.

[Short bowel syndrome and glucose tolerance].

There are different signs after extended small bowel resection. The length of the resected small intestine (size of resection), the location of the removed part (type of resection), the time after operation (stage of adaption) and the age of the patient define the severity and nature of complaints following small bowel resections. We examined the dynamics of glucose- and insulin level after oral and intravenous glucose administration to recognize the relation to the size of resection. We found a decreased basal insulin concentration and a diminished response of insulin after intravenous glucose load in patients with extensive small bowel resection. This reduced response after parenteral administration of glucose load we did not find in the ileum-resected group compared to control group. We can conclude, it is possible to get information about the size of small bowel resections by the intravenous glucose tolerance test.

[Gastric perfusion in the rat--a method for determining gastric secretion].

The response of the 3 postulated receptors in the parietal cell was investigated by means of a perfusion model in the rat. It was compared the effect of cimetidine and atropine on pentagastrin, histamine and carbachol stimulated gastric acid secretion with a control group. Atropine blocked the carbachol stimulation totally but did not effect pentagastrin and histamine stimulation. Cimetidine inhibited all stimulations. This method is a model for study of drugs influences of the gastric secretion.

Symptomless postoperative thrombosis of the deep calf veins--treatment and prognosis.

Deep calf vein thrombosis in the rule can be diagnosed by objective methods like 125I fibrinogen up take test only. But if the diagnosis is done, we have to cure because the risk of the single case is not calculable prospectively. After our therapy we never saw neither progressive development of the thromboembolic process, nor haemorrhagic complications.

[Syndrome following injury of the ventromedial hypothalamic nucleus (VMH syndrome) and its relation to increased secretion of gastric acid (hyperchlorhydria)--an experimental study with rats].

In rats a stereotactic electrocoagulation of the Nucleus ventromedialis hypothalami (NVM) leads to a VMH syndrome (ventromedial hypothalamic syndrome) with hyperphagia, adiposity, and hyperinsulinaemia. A less known fact in this connection is the increased secretion of gastric acid (hyperchlorohydria). We could prove that the basal gastric secretion increases after NVM lesions by two times (from 18 mumol H+/hr to 37 mumol H+/hr) and that this hyperchlorohydria can be removed by a vagotomy. From this fact one can derive conclusions concerning the acute gastric mucous secretion after brain lesions with regard to their pathogenesis and therapy.

[Increased cholinergic stimulating gastric secretion after truncal vagotomy in rats].

The reasons of the relapses after vagotomy are not clarified completely. The reactivity of the 3 postulated receptors in the parietal cell was investigated by means of a perfusion model in the rat in a control group and after truncus vagotomy 28 days before the experiment. Not any difference was shown between the control group or the vagotomy group after histamine and pentagastrin stimulation. A significant greater secretion (!) with negative insulin test was observed in the vagotomy group after carbachol stimulation.
