Effect of long-term therapy with nipradilol on esophageal varices in patients with compensated cirrhosis. Results of a multicenter open study.

The effect of long-term administration of nipradilol (NIP, Hypadil Kowa, CAS 81486-22-8), a beta-blocker with a vasodilatory action, on esophageal varices was studied in 66 patients with compensated liver cirrhosis. Administration of NIP (6-12 mg/d) for 3-12 months produced progressive improvement of endoscopic findings over time (30% for C, 25% for F, and 40% for the R-C sign after 12 months). At the last examination (mean: 9 +/- 4 months), the improvement rates were 16.7%, 16.7% and 22.7%, respectively. No significant relationship was found between endoscopic improvement and the Child-Pugh score or the dose of NIP. Gastrointestinal bleeding occurred in five patients: one had bleeding esophageal varices, three had bleeding gastric varices, and one had a bleeding gastric ulcer. The systolic blood pressure was decreased significantly (4.6-12.3%) at 2 weeks as well as 1 and 2 months, and the heart rate showed a significant decrease throughout the study (10-18.4%). With the exception of the patients who had gastrointestinal bleeding, no symptoms of decompensation appeared, and there was no deterioration of laboratory parameters including ammonia. Adverse effects occurred in about 10% of the patients, most of which were related to bradycardia and/or hypotension, and they improved when the drug was withdrawn or the dose reduced. These results suggest that long-term administration of NIP is useful in the treatment of esophageal varices.

Histological reconstruction of a Von Meyenburg's complex on the liver surface.

We report here a case with four black nodules on a smooth liver surface. The cystically dilated part of the intralobular bile duct of the microhamartoma contained bile pigments. Neither liver cysts nor cystic kidney were found. Spicular processes from the dilated bile duct were attached directly to the liver cell cords, seemingly to receive the secreted bile juice from the bile canaliculi. A convoluted tubular bile duct communicated with an interlobular terminal bile duct in the neighbouring portal triad. Thus, the so-called microhamartoma had not developed separately in the liver acini but had arisen in connection with both the liver cell cords and the terminal bile duct as part of the still functioning bile duct.

Laparoscopically disclosed findings of insidious portal hypertension in a hepatitis B surface antigen carrier.

In a 37 year-old male, a chronic asymptomatic carrier, laparoscopic examination disclosed a thickening capsule of the liver with a whitish gray hue and congestive branches of the portal vein scattered on the thickened hepatic capsule. The congestive branches of the portal vein stood out in bold relief against the whitish gray hue and they were suggestive of portal hypertension. The subserosal vein of the gallbladder was also congested and the spleen was enlarged. These findings were related to insidious portal hypertension, though no manifestations of portal hypertension could be detected clinically. Moreover, no early history of liver disease or very few abnormal laboratory findings could be found in this case. The histology of the biopsied liver showed markedly proliferated fibrous tissue, especially in the portal areas, as well as somewhat thick septal formation. To date, many laparoscopic findings of portal hypertension were given, but this is the first report of laparoscopically detected findings in which congested branches of the portal vein were scattered and stood out in bold relief against the thickened hepatic capsule.

Kupffer cell hyperplasia in liver diseases. Demonstration by scanning electron microscopy of biopsy samples.

Kupffer cells were observed in liver biopsy tissues of 9 cases of liver diseases by scanning electron microscopy to prove Kupffer cell proliferation numerically. Kupffer cell count per 0.01 mm2 of cracked surface of liver lobule was 1.2 +/- 0.3 in the convalescent stage of a mild acute hepatitis case and 1.2 +/- 0.1 in a chronic persistent hepatitis case with slight inflammation. Whereas it was increased to 2.4-5.5 (P less than 0.01) in the convalescent stage of moderate to severe acute hepatitis cases, 1.8 +/- 0.1 (p less than 0.05) in a chronic active hepatitis case, 2.5 +/- 0.3 (p less than 0.001) in an alcoholic portal fibrosis case and 2.2 +/- 0.4 (p less than 0.001) in a liver cirrhosis case. Kupffer cell count per mm3 of liver lobule was estimated roughly 3,500 in the convalescent stage of a mild acute hepatitis case and in a mild chronic persistent hepatitis case and 7,000 to 16,000 in the convalescent stage of moderate to severe acute hepatitis cases.

Adaptative changes of activity of enzymes involved in fructose metabolism in the liver and jejunal mucosa of rats following fructose feeding.

The adaptative response of a diet containing 60% fructose on the activity of those enzymes which are involved in the metabolism of fructose was measured in the liver and in the jejunal mucosa of rats over a period of 12 days. Control animals received isocaloric amounts of glucose or starch. Under fructose feeding there was a marked increase in the activity of fructose-1-phosphate aldolase (3-fold), ketohexokinase (2--3-fold), and triokinase (3-fold) in the jejunal mucosa. In the liver, however, a significant increase in enzyme activity could only be seen for triokinase (2--3-fold), whereas the activity of the other enzymes measured were only slightly or not at all altered. The activity of the three enzymes mentioned above were elevated to a maximum within 3 days after feeding the fructose diet. In the following time of observation no major further changes occurred. The results show that fructose feeding in comparison to a glucose or starch containing diet leads to a marked adaptative increase in the activity of those enzymes, which are involved in the breakdown of fructose, only in the jejunal mucosa.

Scanning electron microscopy of the rat bile canalicular-ductular junction.

Scanning electron microscopy revealed two types of bile canalicular-ductular junctions in rats. 1. Several bile canaliculi converged to enter the terminal bile ductule by direct continuity, forming an ampulla just before the junction. The ampulla was about 2.5x6 micrometers, whereas the canalicular diameter was about 1.5 micrometer just before the ampulla and the ductular diameter was 1.2--4.0 micrometers. The ampulla was mainly walled by several hepatocytes and partly by a few ductular cells. The hepatocytes abutted on the bile ductular cells. 2. Bile canaliculi connected directly to the bile ductule as a side branch without forming the ampullary dilation. In this case, a part of the bile ductular canal was formed by the hepatocytes at the junction of the bile canaliculus with the bile ductule. This study confirmed the light microscopy observation of Lettule and McIndoe.

Scanning electron microscopy of the bile ductule.

Scanning electron microscopy of a liver biopsy specimen from a patient 6 months after the onset of acute hepatitis revealed a normal appearing bile ductule. The bile ductule was 13 micron in outer diameter and 1.5-2.7 micron in inner diameter. The ductular lumen was surrounded by two ductular cells and ampullary dilated at the canalicular side. In the lumen, intracytoplasmic diverticles were observed with an orifice diameter of 0.9 micron. About 15 microvilli 0.4-0.6 micron in length and 0.1 micron in thickness were observed on 1 micron2 of luminal surface. The number of microvilli was calculated to be approximately 1,500 per ductular cell. A single cilium 0.15 micron in diameter at the base and 0.10 micron at the trunk, and 7-15 micron in length was found on each ductular cell. The cilium arose from a recess at the canalicular side of the ductular cell, and the free-end was on the bile-duct side. The cilia seemed to play an important role in bile flow to the duct. Sometimes between the ductular cells at the edge of the basal portion a cell was intercalated with thin processes. Such thin processes were not observed on ductular cells.

Peritoneoscopic diagnosis of liver surface--using a new code number system--.

A new peritoneoscopic classification of liver surface using a three digit code was developed. The first digit represents the lobular change from normal structure to nodules. The second digit represents the size of the nodule and the internodular distance. The third digit represents the pathological findings, such as inflammation, degeneration, necrosis or regeneration. Furthermore, peritoneoscopic findings of necrosis of lobular size around the portal vein and patchy markings of the liver were presented. These findings were the most prominent in the course of hepatitis progression to cirrhosis.

Lupus cystitis and performation of the small bowel in a patient with systemic lupus erythematosus and overlapping syndrome.

We describe a patient with systemic lupus erythematosus (SLE) and overlapping syndrome who had repeated gastrointestinal (GI) symptoms such as nausea, vomiting and malabsorption. With a subacute process, she developed paralytic ileus and contracted bladder, and died of perforation of the ileum. When 13 reported cases with lupus cystitis were reviewed, an extremely strong correlation between GI and urinary tract symptoms was observed, indicating the presence of a unique subgroup of SLE.
