Does serum protein binding inhibit tissue penetration of antibiotics?

Accumulated data on the penetration of 15 antibacterial agents into peripheral human lymph are reviewed. The major factor determining extravascular penetration is the level of serum protein binding. The penetration ratios of extravascular and serum levels diminish gradually as the level of protein binding increases. However, even at the highest protein-binding levels, distinct lymph concentrations are observed. The degree of penetration ranges from 100% for the antibiotic with the lowest level of serum protein binding (gentamicin, no drug bound) to 20% for that with the highest level of protein binding (flucloxacillin, 96% bound).

Cytodiagnosis of acinic cell carcinoma. Ultrastructural study of material obtained by fine needle aspiration biopsy.

The material obtained by fine needle aspiration biopsy from a metastatic acinic cell carcinoma was evaluated by light and electron microscopy. A preliminary diagnosis of acinic cell carcinoma established by light microscopy was confirmed by electron microscopic study. Glandular arrangement of cancerous cells and the presence of a specific type of secretory granules in their cytoplasm were the most characteristic ultrastructural features of the acinic cell carcinoma.

Studies on acute lung lymphatic edema.

The outflow from lymphatic vessels running along with the right cardinal bronchus was interrupted in six dogs by the removal of the corresponding hilar lymph nodes. After 6-8 days the right lung showed slight subpleural areas of atelectasis and edema. Edema of endothelial cells of the lung septal capilaries was observed electron-microscopically. This lesion is similar to the findings in cases of mitral stenosis.

Fine needle aspiration cytology of rare malignant tumors of the breast.

This report describes the fine needle aspiration (FNA) cytologic findings in 17 rare malignant breast tumors. The series consisted of invasive cribriform carcinoma, papillary carcinoma, apocrine carcinoma, carcinoma with pseudosarcomatous metaplasia, carcinosarcoma, fibrosarcoma, malignant phyllodes tumors, primary malignant lymphomas, plasmocytoma, metastatic melanoma and metastatic renal clear cell carcinoma. Besides cytomorphology, the results of immunostaining in eight cases are presented, as is a review of the literature. It is important for rare primary malignancies, as well as for metastatic tumors, to be diagnosed, or at least have the diagnosis suggested, preoperatively by FNA and immunocytochemistry, permitting better therapy planning.
