[Double blind comparison of the antidepressives viloxazine and imipramine (author's transl)].

In a double blind trial in 88 patients, 71 of whom were included in the fin al evaluation, viloxazine (Vivalan ICI) showed an effectiveness corresponding to imipramine. Both antidepressives show properties for lysis of depression and improvement of mood, the target symptom is vitally depressive disturbance of mood. There was clear improvement of depression, anxieties, restlessness, loss of initiative and activity as well as pain syndromes in psychosomatic disorders. In cases of psychomotor inhibition viloxazine was superior to the older drug. Effectiveness of viloxazine commenced rapidly as demonstrated by control assessment after 10-14 days. Results improved further in a second equally long treatment period. Tolerance of viloxazine was superior to imipramine.

A clinical trial comparing nomifensine and nortriptyline.

A double-blind study which compared the effects of nomifensine and nortriptyline was carried out in outpatients with retarded depressions. It demonstrated that nomifensine has distinct antidepressive and activating effects. 40 patients (17 male and 23 female) with either endogenous, endogenous/reactive or reactive depression were divided into two treatment groups of 20. During the 21-day study, all patients were given three 50-mg capsules of either nomifensine or nortriptyline per day. With the aid of a depression scale, devised by Pöldinger, a significant improvement in the psychic symptoms was noted both in the nomifensine and in the nortriptyline group. There was no evidence of a significant difference between the two drugs. From the results of the study, it can be concluded that nomifensine is suitable for the treatment of patients with retarded depressions.

[Pharmacotherapy of aggression].

Various methods are available for the treatment of aggression, sexual aggression and auto-aggression. There can unfortunately be no talk of an actual "anti-aggressive" drug, so that the choice of individual and combined medication will depend on the nosological classification of the underlying illness, on the phenomenology of the manifestation, and also on the result of weighing the therapeutic expectation against the possible risk. Drug treatment of aggression will, however, only be successful when it is integrated into an overall psychotherapeutic sociopsychiatrically oriented plan of therapy.

Neurotropic drugs as co-medication to psychotropics. Combined administration of a neurotropic drug and a tetracyclic antidepressant.

Interactions of EMD 21 657 - a neurotropic drug - and maprotiline were studied in 40 depressive outpatients. Patients were assigned at random to an experimental or a control group. 20 patients of the experimental group received EMD 21 657 (3 X 400 mg/day p.o.) in addition to maprotiline (75 mg/day p.o.) during an initial treatment of 10 days. 20 patients of the control group were treated during this period with maprotiline of the same dosage but did not received EMD 21 657 as co-medication. During the 10-day period of treatment and observation, subjective information on the first day of improvement of depressive symptoms and severity of side effects were recorded. Under supplementary treatment with EMD 21 657 the onset of antidepressant action occurred sooner than in the control group receiving maprotiline alone. The earlier improvement in the experimental group was statistically significant in the target symptom 'obsessive rumination' (p less than 0.01). Undesirable effects of maprotiline treatment were seldom observed or were mild in terms of severity. A decrease in the rate of side effects in the experimental group could not be shown statistically.

The long-term treatment of periodical and cyclic depressions with flupenthixol decanoate.

In a 2- to 3-year open study 30 patients were treated with flupenthixol decanoate (depot form). These periodic and cyclic depressive patients had side effects during treatment with lithium salts. In comparison with the depressive and manic phases before the treatment, the prophylactic effect of flupenthixol decanoate on periodic and cyclic forms of depression was similar to the effect of lithium salts.

[Problems with hypnotic drug abuse and its therapy (author's transl)].

Barbiturate dependence leads to serious withdrawel symptoms requiring first hospitalisation and later intensive outpatient treatment. This treatment can be supported with neuroleptic and anti depressant drug therapy. Adequate supportive aftercare is essential for a successful treatment of drugdependence.

[Position of behavior therapy among the psychiatric therapies].

In the fourth table, we have classified the most important measures of behaviour therapy: desensitivization, operating conditioning, aversive therapy and negative learning. These courses of action can play an important role in psychiatry and can enrich the therapeutic repertory through their rational use, placed in a general plan of treatment. Moreover, these methods are usually excluded from the tought theories and they do not see in the neurotic troubles a consequence of unconscious conflicts; these conflicts must become conscious again, but they must appear, then, as the result of the learning of bad behaviour.

Differences in effect between nomifensine and nortriptyline.

A double-blind study comparing the effects of nomifensine and nortriptyline, which was carried out exclusively on outpatients with retarded depressions, showed nomifensine to have a distinctive antidepressive and activating effect. 40 patients (17 male and 23 female) with either endogenous, endogenous/psychogenic or psychogenic depression were divided into two treatment groups (20 patients to each group). During the 21-day study, all patients were given three 50 mg capsules of nomifensine or nortriptyline per day. With the aid of a depression scale, a significant improvement in the psychic symptoms was noted both in the nomifensine and the nortriptyline group. There was no evidence of a significant difference in the two drugs. The data obtained from the dropouts suggest that nomifensine had a better effect. From the results of the study, it can be concluded that nomifensine is suitable for the treatment of patients with retarded depressions.
